CAD-CAM과 전통 제작 방법으로 제작된 가철성 국소 의치의 치아 및 내면 적합도 비교

목 적: 본 연구의 목적은 가철성 국소의치 제작 방식에 따라 교합면 정확도와 조직면 적합도의 차이를 평가하는 것이다. 전통적 제작 방법인 Pack & Press, Fluid resin, Injection mold와 디지털 방식인 CAD-CAM 방법을 비교하여, 각 방법의 제작 정확도와 적합도를 3차원 표면 분석과 레플리카 테크닉을 이용하여 측정하고, CAD-CAM 기술의 임상적 적용 가능성을 검증하고자 한다.
방 법: 본 연구에서는 상악 우측 제1, 2소구치 및 제1대구치가 결손된 Kennedy class Ⅲ의 부분 무치악 마스터 모델을 제작하였다. 우측 견치와 제2대구치는 서베이드 크라운으로 제작하였으며, 프레임워크는 구개판을 주연결장치로 설정하였다. 실험실용 스캐너(Identica T500; Medit Co.,Korea)로 마스터 모델을 스캔한 후, CAD 소프트웨어(exocad DentalCAD; exocad GmbH)를 이용하여 프레임워크를 설계하였다. 메탈 프린터(NCL-M2150X; Nanjing Chamlion Laser Technology Co., China)를 사용하여 Pack & Press, Fluid resin, Injection mold, CAD-CAM의 네 가지 제작 방법에 사용할 60개의 프레임워크을 제작하였다(n=15).
CAD-CAM 방법은 CAD 소프트웨어(Meshmixer; Autodesk Inc., USA)를 이용하여 프레임워크와 인공치아가 정확히 안착될 수 있는 geometric guide를 설계하였다. 석고 디스크를 제작하여 5축 밀링 장비(ARUM 5X-500; Arum Dentistry, Korea)로 밀링하였으며, 해당 geometric guide에 프레임워크와 인공치아를 안착시켜 접착제로 고정하였다. 이후 다수의 유지구를 형성하고 의치상 레진(Press LT; Retec, Germany)을 주입하여 가압식 중합장치(Palamat elite; Kulzer GmbH, Hanau, Germany)를 이용해 중합하였다. 중합 후 CAM 소프트웨어(ARUM Apexmill; Arum Dentistry, Korea)를 사용하여 가공경로를 계산하고 최종 가철성 국소의치를 가공하였다. 전통적인 방법은 Pack & Press, Fluid resin, Injection mold의 세 가지 방법으로 통상적인 제작 절차에 따라 제작하였다.
제작된 의치의 정확도 평가는 교합면 정확도(Occlusal trueness)와 조직면 적합도(Tissue adaptation)로 구분하여 실시하였다. 교합면 평가는 CAD-CAM 방법의 경우 CAD 디자인 파일을 참조(reference) 데이터로, 밀링된 최종 의치의 스캔 데이터를 비교 데이터로 사용하였다. 전통적인 방법은 제작 전 왁스 의치와 제작 된 최종 의치의 스캔 파일을 각각 참조 및 비교 데이터로 사용하였다. 두 데이터 간의 평가를 위해 3차원 표면 분석 소프트웨어(Geomagic Control X; 3D Systems Inc., USA)를 이용하여 초기 중첩 후 프레임워크의 주연결장치인 구개판을 기준으로 best fit alignment를 적용하였다. 이후 제1, 제2소구치 및 제1대구치의 협측과 설측 교두 총 8개 포인트에서 shell to shell deviation을 측정하였다. 편차는 3D compare 기능을 이용해 색상 맵으로 시각화하였으며, 색상 스펙트럼은 ±0.5 mm를 임계 값, ±0.1 mm를 공칭 값으로 설정하였다.
적합도 평가는 실리콘 재료(Fit Checker II; GC Corp., Japan)을 이용하여 무치악 부위를 기록한 뒤, 실리콘 인상재((Examixfine; GC Corp., Japan)를 주입하여 복제하였다. 이후 실리콘 퍼티(Putty Type; GC Corp., Japan) 베이스를 사용하여 레플리카를 제작하였다. 제작된 레플리카를 절단하여 협측 경사면, 치조정, 설측 경사면, cingulum, rest seat 부위의 단면 중 15개 포인트에서 실리콘 층 두께를 30배율 실체 현미경 (SZX-168-TL; Motic Inc., Germany)으로 측정하였으며, 이미지 분석은 소프트웨어(ImageJ v1.45; National institues of Health, USA)를 이용하였다.
통계 분석은 SPSS 통계 프로그램(SPSS v28.0; IBM Corp., USA)을 사용하여 정규성 검정(Shapiro-Wilk test), 등분산 검정(Levene test) 및 Welch ANOVA 검정을 실시하였으며, 사후 검정은 Games-Howell을 시행하였다, 유의수준은 a=.05로 설정하였다.
결 과: 각 제작 방법별 교합면 정확도는 통계적으로 유의한 차이를 보였다(p<.001). 평균 교합면 편차는 Fluid resin 그룹(248.06 ± 50.55 μm)이 가장 높았으며, Pack & Press 그룹(193.01 ± 92.63 μm), Injection mold 그룹(-28.44 ± 18.98 μm), CAD-CAM 그룹(9.92 ± 12.94 μm)의 순으로 나타났다. 특히 전통적 방법인 Fluid resin과 Pack & Press 그룹은 CAD-CAM 및 Injection mold 그룹에 비해 유의하게 큰 편차를 보였다(p<.001). 조직면 적합도 또한 그룹 간 유의한 차이를 보였다(p<.001). 평균 적합도는 Fluid resin 그룹(192.30 ± 45.14 μm)이 가장 높았으며, Pack & Press(189.61 ± 39.16 μm), CAD-CAM(150.32 ± 30.56 μm), Injection mold(143.99 ± 28.02 μm) 순으로 나타났다. 사후 검정 결과 Pack & Press 및 Fluid resin 그룹이 Injection mold 및 CAD-CAM 그룹보다 통계적으로 유의하게 높은 적합도 편차를 나타냈다(p<.01).
랜드마크별 부위 분석은 치조정과 설측 영역에서 통계적으로 유의한 차이가 관찰되었다(p<.05). 치조정 부위에서 Fluid resin 그룹(194.82 ± 51.49 μm)이 Injection mold 그룹 (147.49 ± 39.85 μm)보다 유의하게 높은 편차를 보였으며(p=0.042), 설측 부위에서는 Pack & Press 그룹(203.09 ± 58.67 μm)과 Fluid resin 그룹(206.8 ± 48.47 μm)이 Injection mold 그룹(140.67 ± 36.07 μm)과 CAD-CAM 그룹(145.71 ± 36.52 μm)보다 유의하게 높은 편차를 보였다(p<.05).
결 론: 본 연구의 한계 내에서 다음과 같은 결론을 얻었다.
1. 교합면 정확도는 CAD-CAM 그룹이 가장 낮은 편차를 보이며, 가장 높은 교합 정확도를 나타냈다(p<.001).
2. 조직면 적합도는 CAD-CAM 및 Injection mold 그룹이 Pack & Press 및 Fluid resin 그룹에 비해 유의하게 낮은 편차를 보였다(p<.01).
3. CAD-CAM 및 Injection mold 그룹은 다른 두 그룹보다 교합면 정확도와 조직면 적합도에서 낮은 편차를 나타냈으며, 이는 의치의 해부학적 적합성 향상에 기여할 수 있음을 시사한다.

Purpose. The purpose of this in vitro study was to compare the occlusal trueness and tissue surface adaptation of RPDs (removable partial dentures) fabricated using CAD-CAM milling with those fabricated using three conventional techniques: pack & press, fluid resin, and injection mold.
Methods. A partially edentulous master model was fabricated for a Kennedy class III patient missing the maxillary right first and second premolars and first molars. The right canine and second molar were restored with surveyed crowns, and a palatal plate was selected as the major connector. After scanning the master model with a laboratory scanner (Identica T500; Medit Co., Korea), and the framework was designed using CAD software (exocad DentalCAD; exocad GmbH., Germany) and fabricated using a metal printer (NCL-M2150X; Nanjing Chamlion Laser Technology Co., China). Sixty RPD frameworks were produced and divided into four groups according to fabrication method (n = 15 per group).
For the CAD-CAM group, a geometric guide was designed using CAD software (Meshmixer; Autodesk Inc., USA) to ensure accurate positioning of the framework and artificial teeth. A stone disk was fabricated and milled using a 5-axis milling machine (ARUM 5X-500; Arum Dentistry., Korea) to produce the geometric guide. The framework and artificial teeth were seated onto the guide and fixed with adhesive, after which multiple retention holes were created. Denture base resin (Press LT; Retec., Germany) was injected and polymerized using a pressure polymerization unit (Palamat Elite; Kulzer GmbH, Hanau., Germany). Following polymerization, the final removable partial denture was milled using CAM software (ARUM Apexmill; Arum Dentistry., Korea) to complete fabrication.
Conventional RPDs were fabricated using three traditional processing techniques (Pack & press, fluid resin, and injection mold) according to standard laboratory procedures recommended for each material system.
The accuracy of the fabricated dentures was evaluated in terms of occlusal trueness and tissue adaptation. For occlusal trueness assessment, the CAD-CAM group used the CAD designed denture STL file as the reference dataset, while the scan of the milled final denture served as the comparison dataset. For the conventional groups, the scanned wax denture was used as the reference dataset, and the scanned final denture was used as the comparison dataset.
All datasets were imported into 3D surface analysis software (Geomagic Control X; 3D Systems Inc., USA). Initial alignment was performed, followed by best fit alignment using the palatal major connector of the framework as the reference region. Shell to shell deviations were then measured at eight predefined points corresponding to the buccal and lingual cusps of the first premolar, second premolar, and first molar. Deviations were visualized using color maps generated by the 3D compare function, with tolerance thresholds set at ±0.5 mm and nominal values set at ±0.1 mm.
Tissue adaptation was evaluated using a silicone based replica technique. The edentulous ridge surface was recorded with a silicone registration material (Fit Checker II; GC Corp., Japan), followed by injection of a silicone impression material (Examixfine; GC Corp., Japan) to duplicate the internal space. A putty type silicone material (Putty Type; GC Corp., Japan) was used as a supporting base to fabricate the replica. The replicas were sectioned, and silicone thickness was measured at a total of 15 predefined points located on the buccal slope, alveolar crest, lingual slope, cingulum rest, and occlusal rest areas. Measurements were obtained using a stereomicroscope at 30× magnification (SZX-168-TL; Motic Inc., Germany), and image analysis was performed using ImageJ software (version 1.45; National Institutes of Health., USA).
Statistical analysis was conducted using SPSS software (SPSS v28.0; IBM Corp., USA). Data normality was assessed using the Shapiro–Wilk test, and homogeneity of variance was evaluated using Levene’s test. Because the assumption of equal variances was violated, Welch’s analysis of variance (ANOVA) was applied. Post hoc comparisons were performed using the Games–Howell test. The level of statistical significance was set at α = 0.05
Results. Significant differences in occlusal trueness were observed among the four fabrication methods (P<.001). The fluid resin group exhibited the greatest occlusal deviation (248.06 ± 50.55 μm), followed by pack & press (193.01 ± 92.63 μm), whereas the injection mold (−28.44 ± 18.98 μm) and CAD-CAM groups (9.92 ± 12.94 μm) demonstrated substantially lower deviation values (p<.001). Tissue surface adaptation also differed significantly among groups (P<.001). The fluid resin (192.30 ± 45.14 μm) and pack & press groups (189.61 ± 39.16 μm) showed significantly greater internal gap thickness compared with the CAD-CAM (150.32 ± 30.56 μm) and injection mold groups (143.99 ± 28.02 μm) (p<.001).
Conclusions. Within the limitations of this study, RPDs fabricated using CAD-CAM milling and injection mold demonstrated superior occlusal trueness and tissue adaptation compared with those produced by pack & press or fluid resin techniques. These findings suggest that CAD-CAM and injection mold workflows enhance the overall fabrication accuracy of removable partial dentures.
