    배경:
    순환종양 DNA(circulating tumor DNA, ctDNA)를 이용한 미세잔존질환(minimal residual disease, MRD) 평가는 근치적 절제 후 대장암 환자에서 재발 위험을 예측하고 보조항암치료(adjuvant chemotherapy) 전략을 조정할 수 있는 중요한 도구로 부상하고 있다. 현재 가장 널리 사용되는 종양-정보(tumor-informed) 기반 MRD 검사들은 높은 민감도를 보이지만, 종양 조직 확보와 맞춤형 패널 설계가 필요하다는 한계가 있다. 반면, 혈장만을 이용하는 종양-비의존(tumor-naïve) 검사는 접근성이 우수하나, 아직 임상적 성능과 활용 가능성에 대한 근거가 부족하다. 본 연구에서는 전장유전체 메틸레이션과 fragmentomics를 이용한 종양-비의존 MRD 검사의 임상적 성능을 검증하고, 상용 종양-정보 기반 검사와 직접 비교하고자 하였다.
    방법:
    근치적 절제를 받은 병기 II–III 대장암 환자를 대상으로 후향적 검증 코호트를 구축하고, 수술 전(P0), 수술 직후(P1), 수술 1년(P2)에 혈장을 채혈하여 cfDNA를 분리하였다. 효소 기반 메틸레이션 시퀀싱(EM-seq)을 통해 대장암 특이 차등 메틸화 부위로부터 tumor methylation score(TMS)를 산출하고, cfDNA 절편 길이 분포로부터 fragment length score(FLS)를 도출하였다. 사전에 정의된 절단값을 이용하여 TMS-high/low, FLS-high/low로 분류하였으며, 질병-무진행 생존(disease-free survival, DFS)을 주요 평가 변수로 하였다. Kaplan–Meier 분석과 기준 시점(landmark) Cox 회귀, 임상병리 인자를 보정한 다변량 Cox 회귀, 그리고 연속 채혈을 반영한 시간의존(time-dependent) Cox 회귀를 시행하였다. 일부 환자에서는 상용 종양-정보 기반 MRD 검사(CancerDetect™)를 병행하여 두 검사 간의 예후 예측 능력 및 재발 검출률을 직접 비교하였다.
    결과:
    총 131명 중 102명(78.6%)에서 TMS와 FLS를 산출할 수 있었고, 중앙 추적기간 약 43개월 동안 48명(36.6%)에서 재발이 발생하였다. 수술 직후 시점(P1)에서 TMS-high 환자는 TMS-low 환자에 비해 유의하게 불량한 DFS를 보였으며(중앙 DFS 18.4개월 vs. 도달 안 됨), 단변량 위험비는 2.60(95% 신뢰구간 1.40–4.83), 다변량 분석에서 보정 후 위험비는 4.19(95% 신뢰구간 1.92–9.14)로 독립적인 예후인자로 확인되었다. 이때 재발 예측에 대한 민감도와 특이도는 각각 28.3%와 92.8%였다. FLS는 보다 약한 경향성을 보였으나, P2 시점에서 TMS-low/FLS-low 조합을 보인 환자군은 그 외 환자군에 비해 현저히 우수한 DFS를 보여, 장기 추적 시 저위험군 선별에 기여할 수 있음을 시사하였다. 시간의존 Cox 회귀 분석에서는 TMS의 종단적 증가가 재발 위험의 유의한 상승과 연관되어 있어, 반복 측정에 기반한 동태적 평가의 중요성을 뒷받침하였다.
    47명에서는 종양-정보 기반 검사 결과가 동시에 확보되었다. 이 아형 분석에서 종양-정보 기반 검사는 P1 시점에서 민감도 60.0%, 특이도 85.2%, 단변량 DFS 위험비 5.84를 보여 종양-비의존 검사보다 우수한 예후 예측력을 나타냈고, 다변량 모델에서도 유일한 독립 인자로 남았다. 그럼에도 종양-비의존 검사는 특히 간 이외 장기 재발 등 일부 재발 사례를 종양-정보 검사와 상보적으로 포착하여, 메틸레이션·fragmentomics 기반 신호가 돌연변이 기반 신호와 상이한 생물학적 정보를 제공할 수 있음을 시사하였다.

    Background:
    Circulating tumor DNA (ctDNA)–based minimal residual disease (MRD) assays have emerged as powerful prognostic tools in resected colorectal cancer (CRC). Tumor-informed assays currently provide the highest sensitivity but require tumor tissue and individualized assay design. Tumor-naïve (plasma-only) approaches offer practical advantages but remain less robustly validated and have historically shown modest sensitivity. In this study, I aimed to validate the clinical performance of a tumor-naïve, whole-genome methylation- and fragmentomics-based MRD assay in resected stage II–III CRC, to compare it directly with a commercial tumor-informed assay, and to explore their potential complementarity.
    Methods:
    This retrospective validation cohort included patients with stage II–III CRC who underwent curative-intent surgery and serial plasma sampling at preoperative (P0), early postoperative (P1), and postoperative 1-year (P2) timepoints. An enzymatic methylation sequencing (EM-seq)–based assay was used to derive a tumor methylation score (TMS) from colorectal cancer–specific differentially methylated regions and a fragment length score (FLS) from cfDNA fragmentomics. Prespecified cutoffs were used to classify patients as TMS-high/low and FLS-high/low. Disease-free survival (DFS) was analyzed using Kaplan–Meier estimates, landmark Cox models, multivariable Cox regression adjusted for clinicopathologic factors, and time-dependent Cox regression incorporating longitudinal TMS and FLS measurements. In a subset of patients, a commercial tumor-informed assay was performed in parallel, allowing head-to-head comparison of MRD status, and DFS association.
    Results:
    Of 131 enrolled patients, TMS and FLS were successfully generated for 102 (78.6%) and were available at one or more timepoints. With a median follow-up of approximately 43 months, 48 patients (36.6%) experienced recurrence. At the early postoperative landmark (P1), TMS-high was associated with markedly inferior DFS compared with TMS-low (median DFS, 18.4 months vs. not reached; univariate HR 2.60; 95% CI, 1.40–4.83), and remained independently prognostic in multivariable analysis (HR 4.19; 95% CI, 1.92–9.14). The corresponding landmark sensitivity and specificity for predicting recurrence were 28.3% and 92.8%, respectively. FLS showed weaker associations with DFS, but the combination of TMS and FLS at P2 identified a composite low-risk group (TMS-low/FLS-low) with excellent outcomes compared with patients who were TMS-high and/or FLS-high. Time-dependent Cox models demonstrated that dynamic increases in TMS over time were significantly associated with subsequent recurrence, underscoring the added value of longitudinal monitoring.
    In the 47-patient subset with paired tumor-naïve and tumor-informed data, the tumor-informed assay showed superior prognostic performance, with a univariate DFS HR of 5.84, a sensitivity of 60.0%, and a specificity of 85.2% at P1, and remained the only significant factor in multivariable models. Nevertheless, the tumor-naïve assay identified a subset of recurrences that were missed by the tumor-informed assay, particularly extrahepatic relapses, suggesting biological complementarity between methylation/fragmentomic and mutation-based signals.
    Conclusions:
    The tumor-naïve, whole-genome methylation- and fragmentomics-based MRD assay showed clinically meaningful prognostic value in resected stage II–III CRC but lower standalone sensitivity than a tumor-informed assay. Longitudinal changes in TMS improved risk stratification, and cross-platform analysis suggested that tumor-naïve and tumor-informed assays capture complementary aspects of residual disease. These findings underscore both the current limitations and the future potential of optimized, multi-modal tumor-naïve MRD assays for MRD-guided treatment in CRC.
