한국은 초고령사회로 빠르게 진입하고 있으며, 돌봄 인력 부족과 만성질환 증가에 대응하기 위해 인공지능(AI)을 활용한 디지털 돌봄기술이 국가 및 지자체 단위에서 확대되고 있다. 특히 AI 기반 비대면 돌봄서비스는 고립 위험 노인의 안전 확인과 정서적 지지 제공에 중요한 역할을 수행하고 있으나, 서비스가 효과적으로 작동하기 위해서는 이용자의 디지털 헬스 리터러시 수준 및 서비스 사용의 수용적 태도에 대한 이해가 필수적이다. 디지털 취약계층이 다수 포함된 노인맞춤돌봄서비스 이용 노인의 경우, 디지털 역량의 격차는 서비스 수용성과 돌봄 성과의 격차로 이어질 수 있어 실증적 분석이 요구된다.
이에 본 연구는 실제 AI케어콜 서비스를 이용하는 노인을 대상으로 디지털 헬스 리터러시 수준과 그 관련 요인을 분석하고, 나아가 디지털 헬스 리터러시와 AI케어콜 서비스에 대한 수용적 태도 간의 관계 및 수용적 태도의 관련 요인을 규명하고자 하였다.
본 연구의 자료는 2024년 4월 기준, 한국의 대표적 공공돌봄사업인 노인맞춤돌봄서비스에서 새롭게 도입된 ‘AI케어콜’ 서비스를 초기 적용한 수행기관에서 수집되었다. 해당 서비스는 기존의 AI 기반 단순 안부 확인 기능을 넘어 수행기관 특성에 맞춘 대화 구성, 기후·날씨 기반 안전 알림, 일상생활 모니터링, 건강관리 조언 등 보다 적극적인 돌봄 기능을 포함하도록 고도화된 형태이다. 신규 사업에 참여한 초기 6개 수행기관 가운데 지역(도시·농촌)과 기관 규모를 고려하여 선정된 4개 기관에서 참여 노인을 대상으로 전수조사가 실시되었으며, 총 1,067명의 자료가 수집되었다. 본 연구는 이 자료를 이용한 이차 분석 연구이다.
디지털 헬스 리터러시는 한국어판 eHEALS로 측정하였으며, 점수 분포 특성을 고려하여 기준에 따라 집단을 구분한 후 다변량 로지스틱 회귀분석을 수행하였다. AI케어콜 서비스에 대한 수용적 태도 역시 이분화하여 관련 요인을 분석하였다.
분석 결과, 디지털 헬스 리터러시는 교육수준, 거주지역, 인지기능, 노쇠, 타 디지털서비스 이용 경험 개수와 유의한 관련성을 보였다. 교육수준이 높고 도시에 거주하는 노인은 디지털 헬스 리터러시 수준이 높았으며, 인지기능 문제가 있거나 노쇠한 노인은 리터러시 수준이 낮은 경향을 보였다. 또한 타 디지털서비스 이용 경험이 있는 노인은 경험이 없는 노인에 비해 디지털 헬스 리터러시가 유의하게 높았다. 추가적으로 디지털 헬스 리터러시 평균점수가 4.0–5.0점에 해당하는 상위 집단을 대상으로 한 분석에서는 여성이 유의미한 관련 요인으로 나타났다.
AI케어콜 서비스에 대한 수용적 태도 분석 결과, 디지털 헬스 리터러시 수준이 높을수록 수용적 태도가 긍정적인 경향을 보였으며, 독거가구 노인이 비독거가구 노인에 비해 수용적 태도가 높은 경향을 보였다.
본 연구는 실제 AI 기반 공공돌봄서비스 이용 노인을 대상으로 디지털 헬스 리터러시와 수용적 태도의 관련 요인을 구분하여 분석하였다는 점에서 의의가 있다. 디지털 취약계층이 집중된 공공돌봄서비스 이용 노인 집단을 중심으로 도출된 본 연구의 결과는 향후 AI 돌봄서비스 설계, 이용자 맞춤형 지원 전략 수립, 서비스 수용성 제고 및 돌봄 격차 완화를 위한 정책적 논의에 기초자료를 제공할 것이다.

As Korea rapidly transitions into a super-aged society, digital care technologies utilizing artificial intelligence (AI) have been increasingly adopted at the national and local government levels to address workforce shortages and the growing burden of chronic diseases. In particular, AI-based non-face-to-face care services play an important role in monitoring safety and providing emotional support for socially isolated older adults. However, for such services to function effectively, an understanding of users’ digital health literacy and their acceptance attitudes toward AI-based care services is essential. Among older adults receiving public care services, disparities in digital capabilities may lead to gaps in service acceptance and care outcomes, underscoring the need for empirical investigation.
This study aimed to examine the level of digital health literacy and its associated factors among older adults using an AI care-call service and to identify factors associated with acceptance attitudes toward the service. In addition, the relationship between digital health literacy and acceptance attitudes was explored.
Data were obtained from older adults participating in the AI Care-Call service, newly introduced within Korea’s national community-based elderly care program. The service extends beyond simple AI-based safety checks by incorporating institution-specific conversation designs, weather- and climate-related safety alerts, daily activity monitoring, and health management guidance. Among six pilot service providers, four institutions were selected based on region (urban vs. rural) and organizational size, and a census survey was conducted. A total of 1,067 participants were included, and secondary data analysis was performed.
Digital health literacy was measured using the Korean version of the eHealth Literacy Scale (eHEALS). Given the distributional characteristics of eHEALS scores, multivariate logistic regression analyses were conducted using different cutoff criteria. Acceptance attitude toward the AI care-call service was dichotomized and analyzed separately. Independent variables included sociodemographic characteristics, health-related factors (cognitive function and frailty), and digital access factors (experience with other digital services).
The results indicated that digital health literacy was significantly associated with educational attainment, residential area, cognitive function, frailty, and the number of other digital services used. Higher digital health literacy was observed among older adults with higher education levels, those residing in urban areas, and those with prior digital service experience, whereas lower literacy was associated with cognitive impairment and frailty. In an additional analysis focusing on the highest literacy group (eHEALS scores of 4.0–5.0), female sex emerged as a significant associated factor. Regarding acceptance attitudes toward the AI care-call service, higher digital health literacy was associated with more positive attitudes, and older adults living alone showed a greater tendency toward positive acceptance compared to those living with others.
This study provides empirical evidence on digital health literacy and acceptance attitudes among older adults using a public AI-based care service. By focusing on a digitally vulnerable population receiving community-based care, the findings offer important implications for the design of AI-driven care services, user-centered education strategies, and policy efforts aimed at enhancing service acceptance and reducing disparities in digital care.
