    옴(Scabies)은 전염성이 매우 강해 조기 진단이 필수적이나, 습진이나 건선 등 타 피부 질환과 임상적 양상이 유사하여 전문 장비 없이는 육안 구분이 매우 어렵다. 이를 보조하기 위한 딥러닝 기반 진단 연구가 시도되고 있으나, 의료 데이터의 희소성 및 실제 임상 환경 간의 도메인 불일치(Domain Shift)로 인한 일반화 성능 저하가 심각한 문제로 지적된다. 본 연구는 이러한 문제를 해결하기 위해, 모델이 도메인에 따라 변하는 배경 정보 대신 피부 병변의 본질적인 특징에 집중하도록 유도하는 새로운 방법론을 제안한다. 이는 중요도 기반 분류 프레임워크인 OFA (One For All)에 웨이블릿 변환(Wavelet Transform) 기반의 도메인 지식을 통합하는 방식이다. 제안 모델은 웨이블릿 변환을 통해 배경 정보를 배제시키고 병변의 중요성을 강조한 물리적 가이드 맵을 생성하고, 이를 모델이 산출하는 중요도 맵과 정렬시키는 새로운 가이던스 손실 함수를 도입하여 최적화되었다. 이 방식은 모델이 피부 톤, 조명 변화, 촬영 기기 특성 등 도메인에 따라 달라지는 비본질적 정보 대신 피부 질환의 본질적인 특징에 집중하도록 유도한다. 부산대학교 양산병원에서 수집된 고화질 데이터를 소스 도메인으로, 부산대학교 간호대학에서 수집된 저화질 데이터를 타겟 도메인으로 설정하여 성능을 검증한 결과, 제안 모델은 소스 도메인에서 86.27%의 최고 정확도를 달성하였다. 특히 타겟 도메인에 대한 퓨샷 러닝(Few-Shot Learning) 환경에서는 베이스라인 모델 대비 최대 약 2배 이상의 일반화 성능을 입증하였다. 본 연구는 복잡한 임상 환경에서도 옴을 정확히 선별할 수 있는 높은 분류 정확도와 도메인 적응력을 동시에 확보한 방법론적 기여를 제공한다.

The proposed method introduces a novel Guidance Loss function. This loss generates a physical guide map that excludes background information and emphasizes lesion importance via WT, and aligns it with the model's computed importance map. This forces the model to focus on the intrinsic pathological features rather than domain-specific variables like lighting or equipment. Performance was validated using high-quality data from Pusan National University Yangsan Hospital as the Source Domain and low-quality data from Pusan National University College of Nursing as the Target Domain. The model achieved 86.27% accuracy in the Source Domain. Crucially, in the Few-Shot Learning environment for the Target Domain, it demonstrated up to a two-fold improvement in generalization performance compared to baseline models. This study offers a robust methodology for accurate Scabies screening by securing both high classification accuracy and domain adaptability in complex clinical environments.
