    본 연구는 집중치료실에 입원한 노르에피네프린 투여환자를 대상으로 장갑과 양말을 이용한 사지보온중재가 말초혈액순환에 미치는 효과를 확인하기 위함이다. 본 연구는 비동등성 대조군 전후 설계 유사실험연구로, 2025년 3월 1일부터 10월 1일까지 B광역시 P대학교병원 집중치료실에 입원한 환자 39명 대상을 임의로 배정하였으며 실험군과 대조군에 각각 15명을 최종 분석하였다. 실험군에게는 장갑과 양말을 적용하여 사지보온을 제공하였고, 총 7일간 10회 걸쳐 반복관찰, 조사하였다. 중재효과는 사지의 말초피부 표면온도(℃), 맥박 산소포화도(%), 모세혈관 충전시간(초)으로 확인하였다. 수집된 자료는 빈도, 백분율, 평균, 표준편차, Chi-square test, Fisher`s exact test, Mann-Whitney U test, Generalized Estimating Equations(GEE)로 분석하였다. 비무작위 배정연구로서, GEE분석 시 일부 사전측정값과 노르에피네프린 투여용량을 공변량으로 보정하였다.
    GEE 분석결과 시간 경과에 따른 두 군의 변화양상은 사지(손가락, 전완, 발가락, 발바닥)의 말초피부 표면온도(χ²=29.73, p<.001), 사지(손가락, 발가락)의 맥박 산소포화도(χ²=18.67, p=.031)와 사지(손가락, 발가락) 모세혈관 충전시간(χ²=17.94, p=.036)은 모두 통계적으로 유의한 차이가 있었다.
    본 연구결과를 바탕으로 임상적으로 노르에피네프린 투여환자에게 사지보온을 적용하면 말초혈액순환을 개선에 도움을 줄 수 있으며 혈관수축제로 인한 부작용을 예방하는데 기여 할 수 있을 것이다.

    The purpose of this study was to evaluate the effects of limb warming on peripheral circulation among patients receiving norepinephrine infusion. Peripheral circulation was assessed using skin surface temperature, oxygen saturation, and capillary refill time, and the effects of wearing sleep gloves and socks were examined.
    This controlled pretest–posttest quasi-experimental study was conducted fromMarch 1 to October 1, 2025, in the intensive care unit of P University Hospital in B City. The participants were adult patients who were admitted to the intensive care unit and were receiving norepinephrine infusion. A total of 30 participants were enrolled, with 15 in the experimental group and 15 in the control group. The experimental group received a thermal intervention by wearing sleep gloves and socks, whereas the control group received standard care without warming and was observed over time. Peripheral circulation was evaluated over seven days following written informed consent and initiation of the intervention. Collected data were analyzed using frequency, percentage, mean, standard deviation, Fisher’s exact test, chi-square test,Mann–Whitney U test and Generalized Estimating Equations(GEE). GEE analysis showed that the patterns of change over time differed significantly between the two groups for peripheral skin surface temperature of the extremities (fingers, forearms, toes, and soles surfaces; χ²=29.73, p<.001), oxygen saturation of the extremities (fingers and toes; χ²=18.67, p=.031), and capillary refill time of the extremities (fingers and toes; χ²=17.94, p=.036).
    Based on these findings, limb-warming interventions such as wearing gloves and socks in patients receiving continuous intravenous norepinephrine infusion in the intensive care unit may help improve peripheral circulation by increasing skin temperature, maintaining oxygen saturation, and shortening capillary refill time.
    These interventions may contribute to the prevention of peripheral complications, including pallor, cyanosis, and necrosis associated with vasoconstrictor therapy.
