치과위생사의 치과 감염관리 업무 역량평가 루브릭 개발 = Development of a competency assessment rubric for dental hygienists in infection control

국문 초록 (Abstract)

본 연구는 치과 감염관리 업무 수행에 필요한 핵심역량을 평가하기 위한 루브릭을 개발하고, 내용타당도와 신뢰도를 검증하는 데 목적이 있다. 치과 감염관리 업무 관련 전문가 7인으로 구성된 패널을 대상으로 1차와 2차 델파이 조사를 시행하고, 분석을 통해 개발된 루브릭의 내용타당도와 신뢰도를 검증하여 다음과 같은 결론을 도출하였다.
치과 감염관리 업무 역량평가 루브릭은 문헌 조사와 내용분석을 통해 치과 감염관리 업무 수행에 요구되는 수행준거를 분류하여 치과위생사의 역량을 도출하고, 이를 ‘지식·기술·태도’ 영역으로 구분하여 구성하였다. 루브릭 문항은 지식역량의 표준주의 10문항, 환경관리 6문항, 기구재처리 10문항, 의료폐기물 관리 3문항, 세탁물 관리 1문항, 기술역량의 표준주의 8문항, 환경관리 8문항, 기구재처리 10문항, 의료폐기물 관리 2문항, 세탁물 관리 2문항, 태도역량의 표준주의 8문항, 환경관리 5문항, 기구재처리 6문항, 의료폐기물 관리 3문항, 세탁물 관리 2문항으로 구성하였다. 지식역량 30문항, 기술역량 30문항, 태도역량 24문항으로 최종적으로 3개의 역량 영역에서 총 84개의 루브릭 문항이 도출되었다.
치과 감염관리 업무와 관련된 전문가 7인의 패널을 대상으로 델파이 조사를 실시하여 내용타당도 지수(S-CVI)를 검증하였다. 1차 조사에서 지식역량 0.89, 기술역량 0.93, 태도역량 0.89로 나타났고, 2차 조사에서 지식, 기술, 태도의 모든 역량에서 1.00으로 나타나 전문가 검증 타당성이 확인되었다. 1차와 2차 전문가 델파이 조사 후 Cronbach's α 값을 확인하여 신뢰도 분석을 실시하였다. 1차 조사에서 지식역량 0.86, 기술역량 0.85, 태도역량 0.87로 전체 0.95로 나타났고, 2차 조사에서 지식역량 0.87, 기술역량 0.77, 태도역량 0.90으로 전체 0.95로 나타나 문항 내적 일관성 신뢰도는 신뢰할 만한 수준임을 검증하였다.
본 연구를 통해 개발된 치과 감염관리 업무 역량평가 루브릭은 치과 의료기관 내 감염관리 수행 수준을 체계적이고 객관적으로 평가할 수 있는 도구로서, 감염관리 질 향상과 안전한 진료환경 조성에 기여할 수 있을 것으로 기대된다. 또한, 본 루브릭은 치과위생사의 감염관리 역량 강화를 위한 교육 및 훈련 프로그램의 설계와 개선을 위한 근거 자료로 활용될 수 있을 것으로 사료된다. 향후 치과 감염관리 업무 역량평가 루브릭을 임상 현장에 적용하여 효과를 평가하는 후속 연구가 활발히 진행되기를 제언한다.

다국어 초록 (Multilingual Abstract)

This study aimed to develop a rubric for assessing the core competencies required for dental infection control tasks and to verify its content validity and reliability. Delphi surveys were conducted in two rounds with seven experts in dental infection control. The content validity index (S-CVI) and reliability of the developed rubric were analyzed, leading to the following findings.
The dental infection control competency assessment rubric was constructed based on literature review and task analysis, classifying the required performance criteria into three domains: knowledge, skills, and attitudes. The rubric included the following items: knowledge—standard precautions (10), environmental management (6), instrument reprocessing (10), medical waste management (3), and laundry management (1); skills—standard precautions (8), environmental management (8), instrument reprocessing (10), medical waste management (2), and laundry management (2); attitudes—standard precautions (8), environmental management (5), instrument reprocessing (6), medical waste management (3), and laundry management (2). In total, 84 items were developed across the three competency domains (knowledge 30, skills 30, attitudes 24).
Content validity was verified through Delphi surveys. In the first round, the S-CVI scores were 0.89 (knowledge), 0.93 (skills), and 0.89 (attitudes), and in the second round, all domains achieved a score of 1.00, confirming expert-validated content validity. Reliability analysis using Cronbach's α showed acceptable internal consistency: in the first round, α = 0.86 (knowledge), 0.85 (skills), 0.87 (attitudes), and overall 0.95; in the second round, α = 0.87 (knowledge), 0.77 (skills), 0.90 (attitudes), and overall 0.95.
The developed rubric provides a systematic and objective tool to assess infection control performance in dental healthcare settings, potentially contributing to improved infection control quality and safer clinical environments. Additionally, it may serve as a reference for designing and improving education and training programs to enhance dental hygienists’ infection control competencies. Future studies applying this rubric in clinical practice are recommended to evaluate its effectiveness.
