본 연구의 목적은 장기요양시설에서 근무하는 요양보호사들이 돌봄 과정에서 스트레스를 어떻게 경험하고 대처하는지를 과정적으로 규명하는 데 있다. 이를 통해 요양보호사의 돌봄 경험을 이해하고 그에 기반한 실질적인 개입 방안을 제시하고자 한다. 이러한 목적을 바탕으로 본 연구는 “장기요양시설 요양보호사는 직무환경의 스트레서(stressor)를 어떠한 조건과 맥락 속에서 경험하며, 그 과정에서 어떠한 방식으로 직무스트레스에 대처하는가”를 연구 문제로 설정하였다.
연구 참여자는 장기요양시설에서 3년 이상 근무하며, 노인 돌봄 업무를 수행하고 있는 요양보호사 14명이다. 참여자 선정에는 이론적 표본추출을 적용하였으며, 자료 수집은 2024년 10월 말부터 2025년 1월 초까지 1:1 심층면접과 유선 전화 면담을 병행하여 이루어졌다. 분석 방법은 Strauss와 Corbin(1998)의 코딩 절차기본 틀과 Corbin과 Strauss(2015)가 제시한 조건–결과 매트릭스를 통합하여 수행하였다. 개방코딩에서는 원자료를 줄 단위로 분석하여 458개의 개념, 56개의 하위범주, 22개의 범주를 도출하였다. 축 코딩에서는 개방코딩을 통해 도출된 범주들을 패러다임 모델에 따라 구조화하였다.
개방코딩에서 원자료를 줄 단위로 분석하여, 458개의 개념, 56개의 하위범주, 22개의 범주를 도출하였다. 개방코딩에서 도출된 범주는 ‘신체적·정신적 한계에 부딪히는 고달픈 돌봄’, ‘임종 돌봄에 대한 두려움’, ‘무방비 속에 노출된 폭력 트라우마’, 헌신과 희생의 딜레마‘, 삶과 업무의 경계 실종’, ‘돌봄 부담의 늪’,‘하대받는 국가자격증의 도우미’, ‘낮은 처우와 부정적 사회 인식’, ‘최선의 돌봄과 감정노동의 딜레마’, ‘홀로 감당하는 돌봄 스트레스’, ‘돌봄 스트레스로부터의 탈주’, ‘돌봄의 진정성’, ‘유연한 사회적 인정투쟁’, ‘고난 속의 희망’, ‘협력적 돌봄의 잡 크래프팅’, ‘돌봄 주체로서 전문성과 주체성의 향상’, ‘돌봄 레질리언스’, ‘신체적·정신적 소진’,‘ 돌봄효능감 향상’, ‘죽음에 대한 인식변화’, ‘강건해진 돌봄 소명감’, ‘미약해진 돌봄소명감’으로 나타났다.
과정분석 결과, 요양보호사가 돌봄 과정에서 스트레스를 경험하는 과정은 “저항기”, “조정기”, “협력적 돌봄연대 구축기”, “돌봄 정체성의 성숙기”의 4단계로 나타났다. 선택 코딩에서는 연구 참여자들의 모든 경험을 포함하는 핵심범주를 도출하였다. 핵심범주는“진흙 속 헌신과 희생의 씨앗이 협력적 돌봄연대를 자양분 삼아 소명감을 내면화하며 피어난 연꽃”이다. 또한 유형분석에서는 핵심범주를 중심으로 중재적 조건과 맥락적 조건의 관계에 따라 가설적 관계를 정형화하여, ‘나 홀로 감내형’, ‘현실 순응형’, ‘자기 연민형’, ‘돌봄 자원활용형’, ‘자기 주도 성장형’의 5가지 실제 유형을 도출하였다. 마지막으로 조건-결과 매트릭스에서는 핵심범주를 중심으로 연구 참여자들의 상호작용을 개인·가족 수준, 집단 수준, 조직 수준, 사회 수준, 국가 수준으로 구분하여 기술하였다.
연구 결과, 요양보호사들이 경험하는 주요 스트레스 요인은 신체적, 정신적으로 과중한 업무, 폭력과 임종 돌봄, 사회적 저평가 등으로 나타났다. 특히 강요된 헌신과 역할 갈등이 돌봄 부담을 심화시켜 신체적, 정신적 소진을 초래하였다. 그러나 일부 요양보호사들은 회복탄력성, 자기효능감, 사회적 지지를 기반으로 스트레스를 완화하고, 협력적 연대와 돌봄의 진정성 회복을 통해 돌봄 효능감과 직업적 소명의식을 강화함으로써 역경 후 성장이 형성되는 과정을 보여주었다. 반면, 열악한 처우와 사회적 인식 부족, 그리고 ‘모든 문제를 스스로 감당해야 한다’라는 인식은 감정노동과 탈주 심리를 심화시켜 정서적 소진과 소명의식 저하로 이어지는 것으로 나타났다.
이상의 결과는 요양보호사의 직무스트레스가 단순한 개인적 문제를 넘어, 개인, 조직, 사회 구조가 상호작용하는 복합적 현상임을 보여준다. 특히, 돌봄은 개인의 헌신과 희생에만 의존해서는 지속될 수 없으며, 조직적 지원과 사회적 연대를 기반으로 한 공동의 돌봄 체계가 필요함을 시사한다. 본 연구는 요양보호사의 직무스트레스 경험 과정을 규명함으로써, 직무스트레스가 단순히 정서적 소진으로 귀결되는 것이 아니라 역경 후 성장으로 전환될 수 있음을 실증적으로 제시하였다는 점에서 학문적, 실천적 의의가 있다.

The purpose of this study was to explore how care workers in long-term care facilities experience and cope with job-related stress throughout the caregiving process. By examining the conditions and contexts shaping these experiences, this study aimed to provide an empirical basis for developing practical intervention strategies to support care workers’ psychological well-being and professional sustainability. The central research question guiding this study was:“Under what conditions and contexts do care workers in long-term care facilities experience job stress, and how do they cope with such stress throughout the caregiving process?”
Participants were 14 certified care workers who had worked for at least three years in long-term care facilities and were actively engaged in elderly care. Theoretical sampling was employed, and data were collected through in-depth individual interviews and follow-up telephone interviews conducted between late October 2024 and early January 2025. Data analysis followed Strauss and Corbin’s (1998) coding procedures, integrating the conditions–consequences matrix proposed by Corbin and Strauss (2015). Open coding generated 458 concepts, 56 subcategories, and 22 categories, which were further structured through axial coding based on the paradigm model.
Axial coding integrated these categories using the paradigm model, and selective coding identified the overarching core category: “A lotus blossoming from the seeds of devotion and sacrifice, nourished by collaborative care solidarity and internalized as a vocational calling.”Categories derived from open coding reflected themes such as physically and mentally exhausting care, fear of end-of-life care, trauma from violence, dilemmas between dedication and sacrifice, blurred boundaries between work and life, burdens of caregiving, emotional labor, social undervaluation, authenticity in care, collaborative job crafting, care resilience, evolving professional identity, care efficacy, and shifts in vocational calling.
Process analysis revealed four sequential phases in the stress experience of care workers: resistance, adjustment, formation of collaborative care solidarity, and maturation of caregiving identity. Typology analysis yielded five coping types—solitary endurer, reality adjuster, self-compassion type, resource-utilizer, and self-driven grower—based on the relationships between contextual and intervening conditions. The conditions–consequences matrix further demonstrated interactional processes spanning individual/family, group, organizational, societal, and national levels.
Results indicated that major stressors included excessive physical and emotional workload, exposure to violence and end-of-life care, and persistent social undervaluation of care work. While enforced sacrifice and role conflict often led to physical and emotional exhaustion, some participants demonstrated posttraumatic growth through resilience, self-efficacy, social support, and collaborative caregiving practices. Conversely, poor working conditions, limited recognition, and a belief that one must “handle everything alone” intensified emotional labor and burnout.
These findings demonstrate that care workers’ job stress is a complex, multi-layered phenomenon shaped by interactions among individual, organizational, and sociostructural factors. The study highlights the need for collective and system-level support—rather than individual sacrifice—to sustain caregiving labor. By illuminating the dynamic process through which stress can evolve into posttraumatic growth, this study contributes meaningful theoretical and practical insights for improving the sustainability and quality of long-term care.
