본 연구는 구강보건의료정책에 대한 치과위생사의 인식과 교육 요구도를 분석하여, 정책 참여 활성화와 교육체계 개선에 기초자료를 제시하고자 하였다. 전국의 일부 치과의원, 치과병원, 대학-종합병원에 근무하는 치과위생사 130명을 편의표본추출하여 2024년 7월 23일부터 2024년 9월 14일까지 구조화된 온라인 설문조사를 실시하였다. 수집한 자료를 SPSS Statistics version 29.0 프로그램으로 분석하였으며, 유의수준은 0.05에서 검정하였다. 자료 분석 방법은 빈도분석, 교차분석, t-검정, 일원배치 분산분석, 상관분석, Borich 요구도 및 우선순위 분석, IPA 분석을 시행하였다. 본 연구의 결과는 다음과 같다.
구강보건의료정책 교육 경험은 ‘근무기관’(p<.01), ‘직위’(p<.01), ‘근무지역’(p<.05)에서 유의하였고, 외부교육 경험은 ‘학력’(p<.05)과 ‘근무기관’(p<.05)에서 차이가 있었다. 참여도는 ‘근무기관’(p<.05), ‘직위’(p<.01)에서 유의하였으며, 대형기관과 관리직, 수도권 근무자의 교육 경험이 높았다. 교육 경험 여부에 따른 구강보건의료정책 인지도는 교육 경험이 없는 집단이 높았다(p<.01). 구강보건의료정책 교육 필요도는 ‘노인맞춤 돌봄서비스 사업’(p<.01), ‘통합건강증진사업’(p<.05)에서 유의한 차이가 나타나, 교육 경험이 없는 집단에서 교육 요구가 더 높았다. 상관분석에서는 정책 인식이 높을수록 수행도(p<.01), 중요도(p<.05)가 높게 나타났고, 중요도-교육 필요도(p<.01), 교육 필요도-참여도(p<.05) 간 정적 상관이 나타났다. 반면 수행도-참여도(p<.05) 간에는 유의한 부적 상관이 나타났다. Borich 요구도 및 우선순위 분석에서는 ‘아동 치과주치의사업’(8.35)이 가장 높은 순위로 나타났고, IPA 분석에서는 ‘학교/보건소 구강보건센터 운영사업’과 ‘수돗물 불소농도 조정사업’이 1사분면에 위치하였고, ‘아동 치과주치의사업’은 2사분면에 위치하여 최우선으로 강화해야하는 정책으로 분류되었다.
구강보건의료정책 이해와 참여를 강화하기 위해서는 체계적인 정책 교육 기반 구축이 필요하다. 정책의 중요성에 대한 인식은 높았으나 실제 참여와 교육 기회는 제한적이므로, 직무 현장에서 쉽게 접근할 수 있는 온라인 교육과정과 직무별 맞춤형 프로그램 개발이 요구된다. 또한 구강보건의료정책의 실무 적용성을 높이기 위해 사례 중심 교육과 지속적인 역량 강화 교육이 병행되어야 한다. 특히 아동·학교구강보건, 장애인사업 등 핵심 정책 영역에 대한 심화 교육을 통해 치과위생사의 전문성을 높이고, 정책 실행 과정에서의 실질적 역할을 확대할 필요하다는 것을 제안한다. 나아가 본 연구 결과는 정책 교육과 참여를 연계한 지원체계를 마련하고, 치과위생사가 구강보건의료정책의 주체로서 정책 발전에 기여할 수 있는 여건을 조성해야하는 필요성에 대한 기초자료로 제공하고자 한다.

This study aimed to analyze dental hygienists' awareness of oral health care policies and their educational needs, thereby providing foundational data for enhancing policy participation and improving the educational system. A convenience sample of 130 dental hygienists working in selected dental clinics, dental hospitals, and university-affiliated general hospitals nationwide participated in a structured online survey conducted from July 23 to September 14, 2024. The collected data were analyzed using SPSS Statistics version 29.0, with a significance level set at 0.05. Data analysis methods included frequency analysis, chi-square tests, t-tests, one-way analysis of variance (ANOVA), correlation analysis, Borich needs and priority analysis, and IPA analysis. The results of this study are as follows.
Experience with oral health policy education was significant for ‘employing institution’ (p<.01), ‘position’ (p<.01), and ‘work location’ (p<.05). Experience with external education differed based on ‘educational background’ (p<.05) and ‘employing institution’ (p<.05). Participation rates were significantly associated with ‘workplace’ (p<.05) and ‘position’ (p<.01), with higher education experience observed among large institutions, managerial positions, and those working in the metropolitan area. Awareness of oral health care policies was higher among those without training experience (p<.01). The perceived need for oral health care policy training showed significant differences in the ‘Senior-Tailored Care Service Program’ (p<.01) and ‘Integrated Health Promotion Program’ (p<.05), with higher training demand among those without training experience. Correlation analysis showed that higher policy awareness correlated with higher implementation (p<.01) and importance (p<.05). Positive correlations were found between importance and perceived need for education (p<.01) and between perceived need for education and participation (p<.05). Conversely, a significant negative correlation was found between implementation and participation (p<.05). In the Borich needs and priority analysis, the ‘Child Dental Home Program’ (8.35) ranked highest. In the IPA analysis, the ‘School/Public Health Center Oral Health Center Operation Program’ and the ‘Tap Water Fluoride Concentration Adjustment Program’ were positioned in the first quadrant, while the ‘Child Dental Home Program’ was positioned in the second quadrant, classifying it as the policy requiring the highest priority for strengthening.
To enhance understanding and participation in oral health care policies, establishing a systematic policy education foundation is essential. While awareness of policy importance was high, actual participation and educational opportunities were limited. Therefore, developing easily accessible online courses and job-specific customized programs for the workplace is required. Additionally, to increase the practical applicability of oral health care policies, case-based education and continuous competency enhancement training should be conducted concurrently. Specifically, it is proposed that in-depth education on core policy areas, such as child/school oral health and disability programs, should be provided to enhance the professionalism of dental hygienists and expand their practical roles in the policy implementation process. Furthermore, the results of this study aim to provide foundational data on the necessity of establishing a support system that links policy education and participation, and creating conditions for dental hygienists to contribute to policy development as key actors in oral health care policy.
