본 연구는 교대근무자의 언어폭력경험이 스트레스 반응을 매개로 턱관절 장애 증상과 구강건강 삶의 질에 미치는 영향을 파악하고자 한다. 연구대상자는 부산 일부 지역 병·의원 및 종합병원, 대학병원에 근무하는 간호사, 경찰과 소방공무원 208명을 대상으로 구조화된 설문지를 사용하였다. 조사 기간은 2025년 7월 1일부터 8월 31일까지 자료를 수집하였다. 교대근무자의 일반적 특성, 언어폭력경험, 턱관절 장애 증상, 구강건강 삶의 질을 조사하였다. 수집된 자료는 IBM SPSS statistics version 29.0을 이용하였으며 유의수준은 .05에서 검증하였다. 자료 분석은 빈도분석, 기술 통계분석, 독립표본 t 검정, 일원배치 분산분석, 상관분석과 매개효과 분석으로 조사하였다. 결과는 다음과 같다. 교대근무자의 언어폭력경험(상사 및 동료)은 언어폭력경험(환자 및 민원인)(r=.437, p<.01)과 유의한 양의 상관관계가 있는 것으로 나타났다. 언어폭력경험(환자 및 민원인)은 스트레스 반응(r=.510, p<.01)과 유의한 양의 상관관계가 있는 것으로 나타났다. 스트레스 반응은 턱관절 장애 증상(r=.624, p<.01)과 유의한 양의 상관관계가 있는 것으로 나타났다. 턱관절 장애 증상은 구강건강 삶의 질(r=-.673, p<.01)과 유의한 음의 상관관계가 있는 것으로 나타났다. 교대근무자의 언어폭력경험(상사 및 동료, 환자 및 민원인)과 턱관절 장애 증상 관계에서 스트레스 반응은 완전매개효과를 가지는 것으로 나타났다(p<.001). 교대근무자의 언어폭력경험(상사 및 동료)은 구강건강 삶의 질에 미치는 직접적인 영향(p<.05)과 간접적인 영향(p<.001)으로 부분매개효과를 가진 것으로 나타났다. 교대근무자의 언어폭력경험(환자 및 민원인)과 구강건강 삶의 질 관계에서 스트레스 반응은 완전매개효과를 가지는 것으로 나타났다(p<.001).
이러한 결과, 교대근무자의 언어폭력경험을 개선하기 위해서는 교대근무자의 업무 실태를 향상시킬 수 있는 다양한 방안이 모색될 필요가 있다. 나아가 교대근무자의 정신·구강 건강 증진과 더불어 공중보건학적으로 중요한 연구를 위한 기초자료를 제공하고자 한다. 특히 직장 내 언어폭력이 심리적 피해를 넘어 생리적 스트레스 반응과 구강건강 악화로 이어질 수 있음을 확인함으로써, 언어폭력 예방 및 관리 프로그램 도입의 근거를 제시하고자 한다. 또한 교대근무자의 업무량 조절과 휴식 보장 등 직무 스트레스 완화와 근무환경 개선의 필요성을 강조하며, 직업적 요인과 구강건강의 연계성 연구를 확장하고자 한다. 나아가 향후 직장 내 건강증진, 구강보건교육, 스트레스 관리 프로그램 개발을 위한 실증적 자료를 제공하고자 한다.

The purpose of this study is to understand the degree of verbal violence experience of shift workers, the resulting stress response, and the effect of stress on the symptoms of jaw joint disorders and the quality of oral health life. The subjects of the study used a structured questionnaire for 208 members of the occupational groups of nurses, police, and firefighters working in hospitals, general hospitals, and university hospitals in some areas of Busan. Data were collected from July 1, 2025 to August 31, 2025. General characteristics, verbal violence experience, jaw joint disorders, and oral health impact index were investigated for the shift workers. IBM SPSS statistics version 29.0 was used for the collected data, and the significance level was verified at .05. Data analysis was investigated by frequency analysis, descriptive statistical analysis, independent sample t-test, one-way variance analysis, correlation analysis, and mediating effect analysis. The results are as follows. It was found that shift workers’ experiences of verbal violence (from supervisors and colleagues) had a significant positive correlation with verbal violence experiences (from patients and civil complainants) (r=.437, p<.01). Verbal violence experiences (from patients and civil complainants) showed a significant positive correlation with stress responses(r=.510, p<.01). Stress responses showed a significant positive correlation with temporomandibular joint (TMJ) disorder symptoms (r=.624, p <.01). TMJ disorder symptoms showed a significant negative correlation with oral health–related quality of life (r=–.673, p<.01).
Stress response was found to have a full mediating effect(p<.001) in the relationship between shift workers’ verbal violence experiences (from supervisors/colleagues and from patients/civil complainants) and TMJ disorder symptoms. Verbal violence experiences (from supervisors and colleagues) were found to have both a direct effect (p<.05) and an indirect effect (p<.001) on oral health–related quality of life, indicating a partial mediating effect. Stress response was found to have a full mediating effect (p<.001) in the relationship between shift workers’ verbal violence experiences (from patients and civil complainants) and oral health–related quality of life.
These results suggest that in order to improve shift workers’ experiences of verbal violence, it is necessary to explore various measures that can enhance the working conditions of shift workers. Furthermore, the study aims to provide basic data for important public health research, along with contributing to the promotion of mental and oral health among shift workers. By demonstrating that workplace verbal violence can lead not only to psychological harm but also to physiological stress responses and deterioration in oral health, this study provides evidence for the need to introduce verbal violence prevention and management programs.
In addition, the findings support the need for reducing job stress and improving working environments—such as through work-load adjustment and guaranteeing rest periods for shift workers. The results also expand research on the link between occupational factors and oral health, and provide empirical data for the development of future workplace health promotion, oral health education, and stress management programs.
