    세계적인 고령화 추세가 가속화됨에 따라 노년층의 건강관리가 사회와 학계 모두에서 중요한 관심사로 대두되고 있다. 그중에서도 정기적인 복약을 필요로하는 계층의 건겅관리는 만성질환자가 치료 효과와 삶의 질을 유지하기 위한 핵심 단계로, 기억력 감퇴, 인지 기능 저하 및 감각 능력 쇠퇴 등 다양한 요인으로 인해 복용 누락, 오복용, 중복 복용 등의 문제가 쉽게 발생하여 치료 순응도와 약물 안전성에 심각한 영향을 미친다. 최근 스마트 메디케이션 박스 제품이 보조적 해결책으로 점차 시장에 진입하고 있으나, 기존의 설계는 기술 논리와 기능 구현 중심으로 이루어져 있어 노년 사용자의 정서적 경험, 심리적 공감 및 사회적 적응 측면에서의 특수한 요구를 간과하는 경우가 많다. 이로 인해 사용 지속성이 부족하고 정서적 단절이 심화되는 문제가 나타나고 있다. 따라서 기능적 요구를 충족시키는 동시에 감성적 디자인을 통해 노년 사용자의 즐거움, 신뢰감 및 소속감을 향상시키는 방안이 시급한 연구 과제로 제기되고 있다.
    이러한 사회적 적응 측면에서의 건강관리를 효과적으로 하기위하여 본 연구는 감성 디자인 기반의 노년층 스마트 메디케이션 박스 개발을 연구 주제로 하여 감성 디자인 이론(Emotional Design), 사용자 중심 디자인(UCD), 오감 디자인(Five-sense Design)을 종합적으로 도입하고, Kano 모델과 QFD 방법을 결합하여 노년 사용자의 다층적 요구를 체계적으로 식별하고 전환하였다. 연구는 문헌 고찰과 시장 조사를 통해 기존 스마트 메디케이션 박스의 기능 논리, 인간공학적 적합성 및 인터페이스 최적화 측면에서의 발전과 한계를 명확히 하였으며, 기존 제품이 노년층의 정서적 요구와 사회적 수용성에 충분히 대응하지 못하고 있음을 지적하였다. 이후 반구조화 인터뷰와 설문조사를 기반으로 열 가지 후보 요구 요소를 도출하고, Kano 모델을 활용하여 이를 분류함으로써 기본형, 기대형 및 매력형 요구의 분포 특성을 식별하였다. 그 결과, 대형 글자와 대비적 표시, 조작이 간편한 물리적 버튼 및 다중 모드 알림 기능은 기본형 요구로, 자동 분약, 보충 알림, 가족 연동 및 비상 알림은 기대형 요구로, 생활화된 외관 디자인, 음성 격려 피드백 및 개성화 선택은 매력형 요구로 분류되었다. 이러한 분류는 각 요구가 만족도와 불만족도에 미치는 비대칭적 영향을 규명하였을 뿐만 아니라, 이후 디자인 요소의 우선순위 설정에 과학적 근거를 제공하였다.

    종합하면 본 연구의 혁신점은 세 가지 측면에서 나타난다.

    With the rapid acceleration of global population aging, health management for older adults has emerged as a major concern in both society and academia. Among these concerns, the health management of individuals who require regular medication is a critical step for patients with chronic diseases to maintain therapeutic effectiveness and quality of life. However, due to factors such as memory decline, reduced cognitive function, and deteriorated sensory abilities, issues such as missed doses, incorrect intake, and duplicate medication occur frequently, which significantly affect treatment adherence and medication safety. In recent years, smart medication boxes have begun to enter the market as auxiliary solutions. Nevertheless, existing designs are largely driven by technological logic and functional implementation, often overlooking the unique emotional experience, psychological empathy, and social adaptation needs of older users. As a result, problems such as insufficient long-term use and emotional disengagement have become increasingly evident. Therefore, it has become an urgent research task to develop solutions that not only meet functional requirements but also enhance older users’ enjoyment, sense of trust, and sense of belonging through emotional design.
    To address the social adaptation aspects of health management effectively, this study adopts the development of a smart medication box for older adults based on emotional design as its research topic. Emotional Design theory, User-Centered Design (UCD), and Five-Sense Design were comprehensively integrated, and the Kano model was combined with the QFD method to systematically identify and translate the multilayered needs of older users. Through literature reviews and market analysis, the study clarified the developmental trends and limitations of existing smart medication boxes in terms of functional logic, ergonomic suitability, and interface optimization, pointing out that current products insufficiently respond to the emotional and social acceptance needs of older adults.
    Subsequently, ten candidate user requirements were derived through semi-structured interviews and questionnaires. Using the Kano model, these requirements were classified, revealing their distribution across basic, performance, and attractive categories. Large typography with high visual contrast, easily operated physical buttons, and multi-modal reminder functions were classified as basic needs. Automatic dispensing, refill alerts, family connectivity, and emergency notifications were classified as performance needs. Lifestyle-oriented appearance design, voice-based encouraging feedback, and personalization options were classified as attractive needs. This classification not only clarified the asymmetric effects of each requirement on satisfaction and dissatisfaction but also provided scientific evidence for prioritizing subsequent design elements.
    In the requirement translation stage, the QFD method was applied to construct a systematic WHATs–HOWs matrix that mapped user needs to specific design elements, ensuring that the sensory, instinctive, and reflective needs of older adults were effectively incorporated into the design solutions. The design concept proposed a “UCD + Five-Sense Collaboration” approach: visually employing large typography, high-contrast colors, and unified icons; auditorily and haptically integrating soft alert tones and vibration feedback; and, at the interaction level, incorporating voice-based encouragement and emotional stabilization mechanisms. To support long-term medication adherence, dosage detection and family-link modules were also integrated. Based on these design foundations, three differentiated conceptual solutions were developed.
    First, Plan A is a portable medication device emphasizing carryability and low learning burden.
    Second, Plan B is a home-use device inspired by an hourglass form, focusing on emotional expression and functional closure.
    Third, Plan C is an upgraded lightweight model emphasizing storage efficiency and mobility, with simplified modules to reduce cost and enhance scalability.
    The comparative evaluation demonstrated that each of the three design concepts had distinct advantages in size, weight, usage context, learning load, and cost distribution, making them suitable for different groups of older users.
    In the evaluation stage for design verification, both design-development experts and potential users were invited to conduct multidimensional assessments of the three design concepts. The evaluation criteria included functionality, aesthetics, operability, and emotional friendliness. Results indicated that Plan B achieved the highest score in emotional empathy and family interaction suitability, making it ideal for home-based users. Plan A excelled in portability and ease of operation, making it suitable for older adults with active lifestyles. Plan C, with its cost-efficiency and universality, was highly promising as an entry-level product. The comparative analysis further confirmed the varying importance of different design elements across usage contexts and empirically validated the effectiveness of applying emotional design to smart medication box development.
    Overall, the innovations of this study can be summarized in three dimensions.
    First, theoretical innovation lies in constructing an interdisciplinary framework for analyzing smart products for older adults by integrating Emotional Design with UCD and Five-Sense Design, thereby expanding the application of emotional design theory in the healthcare product domain.
    Second, methodological innovation is demonstrated by combining the Kano model with QFD to establish a closed-loop process from requirement identification to design translation, ensuring both scientific rigor and practical feasibility in the design pathway.
    Third, practical innovation lies in proposing and validating three differentiated conceptual designs, thereby exploring strategies for balancing emotional needs with functional implementation and providing substantial reference materials for future elderly-friendly product design.
    The findings indicate that emotional design not only alleviates older adults’ feelings of rejection and anxiety toward smart devices but also enhances user enjoyment, social empathy, and long-term medication adherence. Ultimately, it contributes positively to the human-centered, socially inclusive, and sustainable development of smart healthcare products, demonstrating the practical effectiveness of the design approach.
