본 연구는 아토피피부염 환자의 자가간호 수행 측정도구를 개발하여 타당도와 신뢰도를 검증하는 방법론적 연구이다. Riegel의 만성질환 자가간호 중범위 이론(A middle-range theory of self-care of chronic illness)을 바탕으로 개념적 기틀을 구성하였으며, DeVellis의 8단계 도구개발 절차에 따라 진행하였다.
본 연구의 자료수집 기간은 2024년 9월부터 10월까지였다. 연구 대상은 연구 참여에 동의한 아토피피부염 환자 402명이었으며, 이 중 응답이 불충분한 설문지 9부는 제외하였다. 393명의 대상자를 탐색적 요인분석에 197명, 확인적 요인분석에 196명으로 무작위 배정하였다. 자료분석은 SPSS version 28.0과 AMOS version 28.0을 이용하였다.
도구개발 1단계는 문헌고찰 및 심층면담을 통하여 도구의 구성요소를 확인하였다. 2단계는 문항구성으로 7개 요인 53개의 예비문항이 결정되었다. 3단계에서는 5점 Likert 척도를 결정하였다. 4단계 도구검정은 전문가 타당도를 통해 내용타당도가 확보되었으며, 5단계 도구검토에서는 아토피피부염 환자 20명을 대상으로 예비조사를 실시하였다. 6단계 도구 적용에서는 요인분석을 위해 49개 문항으로 본조사를 실시하였다. 7단계 도구평가에서는 구성타당도, 집단비교타당도 및 수렴 타당도를 확보하였다. 내적 일관성 신뢰도와 검사-재검사 신뢰도를 통해 신뢰도를 확보하였다. 최종 문항으로 5요인 17문항을 선정하였다. 마지막 8단계 도구 최적화에서는 문법과 가독성을 고려하여 최종 문항을 확정하였다. 각각의 요인은 ‘생활습관유지’, ‘환경유지’, ‘피부유지’, ‘모니터링’, ‘자가관리’로 명명하였다.
확인적 요인분석을 통해 최종 문항의 모형적합도, 수렴타당도, 판별타당도를 검정하였다. 만성질환자의 자기효능감 도구(SECD-K-6)와 상관관계를 통해 수렴타당도를 확인하였다. 개발된 도구의 내적 일관성 신뢰도는 Cronbach’s α=.88이었고, 검사-재검사 신뢰도가 확보되었다. 점수가 높을수록 아토피피부염 환자의 자가간호 수준이 높음을 의미한다.
본 연구를 통해 개발된 ADSCS(Atopic Dermatitis Patients Self-Care Scale)는 아토피피부염 환자의 자가간호 수준을 객관적으로 평가할 수 있는 도구로, 체계적인 관리와 합병증 예방을 지원하여 궁극적으로 삶의 질 향상에 기여할 것으로 기대된다.

This study aimed to develop a self-care performance measurement tool for patients with atopic dermatitis and verify its validity and reliability. The conceptual framework was based on Riegel's middle-range theory of self-care of chronic illness and DeVellis' eight-step tool development process.
Data collection for this study took place from September to October 2024. The study subjects included 402 patients with atopic dermatitis who agreed to participate in the study. Nine incomplete questionnaires were excluded. Of the 393 subjects, 197 were randomly assigned to exploratory factor analysis and 196 to confirmatory factor analysis. Data analysis was performed using SPSS version 28.0 and AMOS version 28.0
In the first stage of tool development, the components of the instrument were identified through a literature review and in-depth interviews. In the second stage, item composition was determined, resulting in a preliminary set of 53 items across seven factors. In the third stage, a 5-point Likert scale was adopted. In the fourth stage, tool validation, content validity was ensured through expert validation. In the fifth stage, tool review, a pilot study was conducted with 20 atopic dermatitis patients. In the sixth stage, tool application, a main survey was conducted with 49 items for factor analysis. In the seventh stage, tool evaluation, construct validity, group comparative validity, and convergent validity were ensured. Reliability was ensured through internal consistency and test-retest reliability. The final set of items consisted of 17 items across five factors. Finally, in the eighth stage, tool optimization, the final items were finalized after considering grammar and readability. The factors were named "Lifestyle Maintenance," "Environmental Maintenance," "Skin Maintenance," "Monitoring," and "Self-Management."
Confirmatory factor analysis was used to test the model fit, convergent validity, and discriminant validity of the final items. Convergent validity was confirmed through correlations with the Self-Efficacy Tool for Patients with Chronic Diseases (SECD-K-6). The internal consistency reliability of the developed instrument was Cronbach's α=.88, and test-retest reliability was confirmed. Higher scores indicate higher levels of self-care in patients with atopic dermatitis.
The Atopic Dermatitis Patients Self-Care Scale (ADSCS) developed in this study is a tool that can objectively assess the level of self-care in patients with atopic dermatitis. It is expected to support systematic management and the prevention of complications, ultimately contributing to an improved quality of life.
