    Objectives: To investigate the proportion in HC-BPPV (horizontal canal benign paroxysmal positional vertigo) patients who exhibited the same and the opposite direction in head-roll test and lying-down/head-bending nystagmus (LDN/HBN), and to assess the treatment outcomes of HC-BPPV with the opposite direction in head-roll vs (LDN/HBN). Methods: Total 283 HC-BPPV patients were enrolled with 139 geotropic (geo-) and 144 apo-geotropic (apo-) HC-BPPV in the head-roll test. LDN/HBN was evaluated to classify patients into three groups; 1) the same direction as head-roll, 2) the opposite direction as head-roll, and 3) no nystagmus in LDN/HBN. Barbecue roll maneuver was performed according to the direction of localization in head-roll test. Results: Among 139 geo-cases, 40 (28.8%) showed the same direction in head-roll test and LDN/HBN; 29 (20.9%) with the opposite localization; 70 (50.3%) with no LDN/HBN. Among 144 apo-cases, 47 (32.6%) showed the same; 18 (12.5%) with the opposite; 79 (54.9%) without LDN/HBN. The overall resolution rate was 74.8% in geo-cases and 57.6% in apo-cases in a 2- week follow-up. There was no significant difference in resolution rate among three groups in geo-group. In apo-group, there was no difference in resolution rate between the same and the opposite direction, whereas the resolution rate of no LDN/HBN group was higher than that of other two groups. Conclusions: HC-BPPV with the opposite direction between head-roll test and LDN/HBN has a similar treatment outcome to that with the same direction. Care should be taken in the interpretation of LDN/HBN for the localization in HC-BPPV.
