    배경 및 목적: 인구집단은 여러 유해인자에 동시에 노출되며, 이러한 복 합노출은 단일 물질의 노출수준이 건강영향을 유발하지 않을 정도로 낮더라 도 건강영향과 유의한 연관성을 보이기도 한다. 신장 손상 위험이 증가하는 단일 중금속의 기준용량은 여러 연구에서 보고되었으나, 복합노출 및 중금속 간 상호작용의 효과를 고려할 수 있는 새로운 방법론의 개발이 필요하다. 본 연구의 목적은 중금속의 복합노출수준을 나타내는 환경위험점수를 이용하여 신장 손상 위험이 증가하는 기준점수를 추정하는 것이다.
    방법 및 도구: 「오믹스 분석을 통한 환경보건취약지역 주민의 환경성질 환 원인물질 추적 기술개발」(Forensic Research via Omics Markers in environmental health vulnerable areas, FROM) 연구에 참여한 환경보건 취약 지역 주민 중 광산 또는 제련소 주변지역에 거주하여 금속 노출이 우려되는 성인 360명을 연구 대상자로 선정하였다. 소변 중 중금속 9종(수은, 바나듐, 크롬, 망간, 니켈, 몰리브덴, 카드뮴, 안티모니, 납)를 노출변수로, 신장 손상 지표(beta-2-microglobulin [β2-MG], N-acetyl-β-D-glucosaminidase [NAG])를 결 과변수로 사용하였다. 제한적 3차 스플라인 분석, 선형 회귀분석과 로지스틱 회귀분석을 이용하여 단일 중금속과 신장 손상 간의 용량-반응 관계를 확인 하였다. 상호작용 항을 포함한 Adaptive Elastic-Net (AENET-I)을 적용하여 중 금속의 복합노출수준을 나타내는 환경위험점수를 구축하고, 환경위험점수와 신장 손상 간의 용량-반응 관계를 확인한 뒤 이를 이용해 환경위험점수가 50 백분위수, 75백분위수일 때에 비해 신장 손상 위험이 5%, 10% 증가하기 시작 하는 기준점수를 추정하였다.
    결과: 단일 중금속 중 바나듐과 크롬은 β2-MG, 수은은 NAG와 함께 증가 하는 경향을 보였으나 통계분석 방법에 따라 서로 다른 결과를 보였으므로 일관적인 용량-반응 관계를 확인할 수 없었다. 반면 AENET-I을 이용해 구축 한 환경위험점수는 단일 중금속에 비해 신장 손상과 더욱 강하고 일관적인 용량-반응 관계를 보였다. 신장 손상 위험이 증가하는 기준점수는 β2-MG에 대해 5.316-5.653, NAG에 대해 1.789-2.086이었으며, 대상자 중 기준점수 초과 자는 각각 15.8-40.0%, 12.2-35.0%였다. 초과자 중에는 신장손상지표가 정상 범위인 대상자들도 존재했다.
    결론: 본 연구에서 복합노출수준을 요약한 환경위험점수는 단일 중금속에 비해 신장 손상과 더욱 강하고 일관적인 용량-반응 관계를 보였다. 환경위험 점수를 활용한 기준점수는 예방적 모니터링이 필요한 고위험군을 선별하기 위한 복합노출수준의 참고치로 활용될 수 있으며, 환경역학 분야에서 복합노 출의 건강영향을 평가하기 위한 새로운 방법을 제시한다.

    Background and Objectives: Populations are exposed to multiple pollutants simultaneously. Even when the exposure level of a single pollutant is not enough to cause health effects, combined exposure can show significant associations with health outcomes. Although benchmark doses (BMD) of individual heavy metals for renal tubular damage have been reported in several studies, new methodologies that consider the effects of combined exposure and interactions among heavy metals are needed. This study aims to estimate a benchmark score (BMS) for renal tubular damage using environmental risk score (ERS) that represents the combined exposure level of heavy metals.
    Methods and Materials: Among participants in the Forensic Research via Omics Markers in environmental health vulnerable areas (FROM) study, 360 adults residing in areas near abandoned mines or smelters with potential heavy metal exposure were selected as study subjects. Nine urinary metals (mercury, vanadium, chromium, manganese, nickel, molybdenum, cadmium, antimony, and lead) were used as exposure variables, and renal tubular damage markers (beta-2-microglobulin [β2-MG] and N-acetyl-β-D-glucosaminidase [NAG]) were used as outcome variables. Dose-response relationships between individual heavy metals and renal tubular damage were examined using restricted cubic spline regression, linear regression, and logistic regression. ERS using adaptive elastic-net including interaction terms (AENET-I) was constructed. The dose-response relationships between ERS and renal tubular damage were examined, and BMS at which renal tubular damage increases by 5% and 10% compared to the 50th and 75th percentiles of ERS were estimated.
    Results: Vanadium and chromium showed increasing trends with β2-MG, and mercury showed increasing trends with NAG; however, inconsistent results were observed depending on the statistical analysis method. In contrast, the ERS constructed using AENET-I showed stronger and more consistent dose-response relationships with renal tubular damage compared to individual heavy metals. The BMS for renal tubular damage were 5.316-5.653 for β2-MG and 1.789-2.086 for NAG, with 15.8-40.0% and 12.2-35.0% of subjects exceeding the BMS for β2-MG and NAG, respectively. Among those exceeding the BMS, some subjects had renal tubular damage markers within the normal range.
    Conclusions: The ERS showed stronger and more consistent dose-response relationships with renal tubular damage than individual heavy metals. The BMS based on ERS can be used as a reference value for combined exposure, which can be applied to identify high-risk groups requiring preventive monitoring and to present a new approach for evaluating the health impacts of combined exposure in environmental epidemiology.
