기존의 화학사고 기인 유해화학물질에 노출된 인근 주민들의 급성위 해성평가는 민감한 건강 영향평가에는 적합하지 않은 비상대응 유형의 참고치에 주로 의존해 왔다는 한계가 있다. 이에 본 연구는 화학사고로 인한 인근 주민들의 유해화학물질 노출에 따른 급성위해성평가에 활용할 수 있는 Acute Reference Concentration (ARfC)을 개발하고 사례연구를 통해 그 활용가능성을 검토하였다.
ARfC 개발을 위하여 급성참고치의 비상대응 유형과 공중보건 보호 유형 대표로 Acute Exposure Guideline Level (AEGL)과 Acute Reference Exposure Level (Acute REL)을 선정한 후, 이를 Reference Concentration (RfC)을 기반으로 이론적 정의 항목 3가지와 수치 도출 항목 6가지로 나누어 비교하였다. 비교 결과를 토대로, AEGL 및 Acute REL 각각의 장· 단점을 상호보완하여 RfC와 유사한 수준의 ARfC를 개발하였으며, HEC 보정을 제외한 8가지 항목으로 구성하였다.
개발된 ARfC의 활용가능성을 평가하기 위하여 사고대비물질이며, 반감기가 짧고 국내 사고사례가 많은 질산을 대상으로 ARfC를 산출하였 다. 질산의 ARfC 산출 결과, AEGL-1보다 최소 12배 낮은 수치로 나타났다. 이는 RfC와 유사한 개념의 급성참고치인 Acute Reference Exposure (ARE)의 황화수소 사례에서 AEGL-1 보다 낮은 값으로 도출된 결과와 유사한 경향으로, ARfC 역시 공중보건 보호를 위한 합리적인 급성참고치로 판단된다. 한편, Acute REL과 비교하였을 때, ARfC는 5.94배 낮게 도출되었다. 이러한 차이는 연구자에 따른 무유해관찰수준(No-Observed-Adverse-Effect-Level, NOAEL) 해석의 차이에 기인한 것으로, 참고치 도출 과정에서 불가피하게 수반되는 불확실성을 보여준다. 이외에도 본 연구는 자료 수집의 제약, 생리학 기반 약물동태(Physiologically Based Pharmacokinetic, PBPK) 모델링 및 Benchmark Concentration (BC) 접근법의 제한적 반영, 단일 물질에 국한된 사례 연구 등 여러 한계점을 확인하였다. 그럼에도 불구하고, 본 연구는 화학사고로 인한 주민들의 유해화학물질 급성 노출 피해를 정량적으로 평가하기 위한 급성위해성평가 참고치를 기존 급성참고치의 기본 틀을 기반으로 개발하였다는 점에서 의미가 있다.

Existing acute health risk assessments for residents exposed to hazardous chemicals resulting from chemical accidents have relied primarily on acute reference values for emergency response, which are not suitable for evaluating sensitive health effects. In response to this limitation, this study developed an Acute Reference Concentration (ARfC) applicable to acute health risk assessment of hazardous chemical exposure among residents affected by chemical accidents, and examined its applicability through a case study.
For the development of the ARfC, Acute Exposure Guideline Level (AEGL) and Acute Reference Exposure Level (Acute REL) were selected as representative acute reference values for emergency response and acute reference values for public health protection, respectively. These values were systematically compared based on the Reference Concentration (RfC) framework, using three theoretical definition criteria and six quantitative derivation criteria. Based on the comparative analysis, the limitations of AEGL and Acute REL were compensated for and their strengths were combined, resulting in the development of an ARfC comparable to the RfC and composed of eight derivation elements, excluding human equivalent concentration (HEC) adjustment.
To evaluate the applicability of the developed ARfC, nitric acid—classified as an accident preparedness substance with a short half-life and a high frequency of accident occurrence in Korea—was selected as the target chemical, and an ARfC was derived. The resulting ARfC for nitric acid was at least 12-fold lower than the corresponding AEGL-1 value. This result is consistent with the trend observed in the hydrogen sulfide case for Acute Reference Exposure (ARE), an acute reference value conceptually similar to the RfC, in which values lower than AEGL-1 were derived. Accordingly, the ARfC is considered a reasonable acute reference value for public health protection. In comparison with the Acute REL, the derived ARfC was 5.94-fold lower. This difference is attributable to variations in the interpretation of the no-observed-adverse-effect level (NOAEL) among researchers, highlighting the inherent uncertainty inevitably associated with the derivation of reference values.
In addition, this study identified several limitations, including constraints in data availability, limited incorporation of physiologically based pharmacokinetic (PBPK) modeling and the benchmark concentration (BC) approach, and the restriction of the case study to a single substance. Despite these limitations, this study is meaningful in that it developed an acute health risk assessment reference value for quantitatively evaluating acute exposure to hazardous chemicals among residents affected by chemical accidents, based on the fundamental framework of existing acute reference values.
