본 연구는 제왕절개 후 만성 통증을 겪는 24개월 이내의 산후 여성 10명을 대상으로 연부조직 가동술(Soft Tissue Mobilization, STM)과 결합한 저출력 레이저 치료(Low-Level Laser Therapy, LLLT)를 적용했을 때, 통증과 기능, 그리고 피부의 유착 정도에 미치는 영향을 연구한 예비 연구이다. 실험군 5명, 대조군 5명을 무작위 배정하였다. 실험군 5명은 연부조직 가동술과 저출력 레이저 치료를 복합 중재하였고, 대조군 5명은 동일한 조건의 저출력 레이저 치료만 적용하였다. 중재 기간은 총 8주로, 두 그룹 모두 1회 18 분, 주 2회, 4주 동안 치료하였다. 본 연구의 평가 도구는 숫자 평가 척도 (NPRS), 환자 특이 기능 척도(PSFS), 접착계(Adheremeter)를 이용하였다. 통계는 세 시점 변화 비교를 위해 프리드먼 검정(Friedman test)을 사용하였고, 두 그룹 간 효과 비교는 맨-휘트니 U 검정(Mann-Whitney U test)으로 분석하였다. 프리드먼 검정에서 통계적으로 유의한 차이(p<0.05)가 발견된 경우, 사후 분석으로 윌콕슨 부호순위 검정(Wilcoxon Signed-Rank Test)을 실시하였고, 다중 비교 보정을 위해 본페로니 보정(Bonferroni correction)을 적용하였다(유의수준=0.05/3).
본 연구의 결과는 표본수가 적어 통계적으로 유의하지 않았으나, 모든 결과 지표에서 임상적으로 의미 있는 개선이 관찰되었다. 실험군에서는 4주 간의 중재 후 NPRS는 평균 4.40점 감소하여 기존 연구에서 보고된 MCID 기준을 넘겼다. 또한 PSFS는 약 3점 상승하여 기능적 수행 수준에서 임상적으로 의미 있는 향상을 나타냈다. AS값 변화에 대한 효과 크기(Cohen’ d) 는 중간 이상의 효과 크기가 보여, 통계적 유의성과는 별개로 임상적으로 의미 있는 결과가 나왔다. 이러한 결과는 두 중재가 상호 보완적으로 작용한 결과로 해석할 수 있다. 즉, 저출력 레이저 치료가 염증을 감소시키고, 조직에 산소를 공급, 미세 순환 개선을 통해 조직 회복의 생리학적 기반을 형성하고, 연부조직 가동술이 결합조직의 구조적 변화를 유도하여 근막 이동성을 향상시키는 역할을 한 것으로 판단된다.
따라서, 본 연구는 만성 통증을 겪는 산후 여성들에게 유용한 치료 프로그램이 될 수 있음을 시사하며, 보다 큰 표본을 활용한 연구와 장기 추적 평가를 통해 이러한 효과를 추가적으로 검증할 필요가 있다.

Chronic abdominal and peri-abdominal pain is a common condition among postpartum women after cesarean section and is frequently associated with functional limitations, difficulties in daily activities, psychological distress, and reduced quality of life. However, the effect of combined soft tissue mobilization(STM) and low-level laser therapy(LLLT) on chronic abdominal and peri-abdominal pain in this population has not been sufficiently investigated. This pilot study aimed to examine the effects of a combined intervention consisting of STM and LLLT in postpartum women after cesarean section who experienced chronic pain.
Ten postpartum women within 24 months after cesarean delivery who reported chronic abdominal or peri-abdominal pain were enrolled and allocated to an experimental group(n=5) or a control group(n=5). Outcome measures included the Numeric Pain Rating Scale(NPRS), the Patient-Specific Functional Scale(PSFS), and scar adhesion measured using the Adheremeter, from which the adhesion severity score was derived. Assessments were conducted at baseline, after 4 weeks of intervention, and at a follow-up assessment 4 weeks after the end of treatment.
Although statistical significance was limited due to the small sample size, clinically meaningful improvements were observed in the experimental group. NPRS decreased by a mean of 4.40 points after 4 weeks of intervention, exceeding the minimal clinically important difference(MCID), and PSFS improved by approximately 3 points. In addition, AS score increased during the initial 4 weeks and was maintained at follow-up, demonstrating a moderate to large effect size(Cohen’s d).
These findings suggest that a combined intervention of STM and LLLT may have beneficial effects on pain reduction, functional improvement, and scar adhesion characteristics in postpartum women after cesarean section. Further studies with larger sample sizes and longer-term follow-up are warranted to confirm these findings.
