중간엽 줄기세포(Mesenchymal stem cell, MSC)는 손상된 조직의 재생을 촉진하고 면역 환경을 조절할 수 있는 다분화능 성체줄기세포로서, 다양한 질환에 대한 치료 가능성이 제시되고 있다. 특히 낮은 면역원성과 다양한 조직으로의 분화능을 지니고 있어 세포치료제 개발에 있어 핵심적인 세포원으로 평가된다. 최근 퇴행성 질환, 면역질환, 난치성 질환을 대상으로 한 임상시험이 증가함에 따라, 치료 시점까지 세포의 품질을 안정적으로 유지할 수 있는 저장 기술의 중요성이 점차 강조되고 있다. 이러한 배경에서 동결보존 기술은 세포치료제 제조와 공급에 있어 필수적인 핵심 기반으로 인식되고 있다. 현재 MSC의 동결보존에는 Dimethyl sulfoxide (DMSO)가 가장 널리 사용되고 있으나, 세포 기능에 영향을 미치는 독성 및 환자 주입 과정에서 발생할 수 있는 부작용 우려가 지속적으로 보고되고 있어 임상적 측면에서의 한계가 제기되고 있다. 이에 따라 본 연구에서는 탯줄 유래 중 간엽 줄기세포(Umbilical cord-derived MSC, UC-MSC)를 대상으로, 침투형 및 비침투형 동결보호제의 상보적 효과에 기반한 DMSO 무함유 동결보호제조성을 확립하고자 하였다. 이를 위해 침투형 동결보호제인 Ethylene glycol (EG), Propylene glycol (PG), Glycerol과 비침투형 동결보호제인 Polyethylene glycol (PEG), Mannitol, Trehalose를 조합하여 해동 후 세포 생존율에 대한 1차 선별을 수행하였으며 이후 EG 및 PEG 조성이 가장 유망한 후보로 확인됨에 따라, 두 성분의 농도를 조절하여 최적 조건을 탐색하는 2차 선별을 진행하였다. 그 결과, 7.5% EG + 7.5% PEG 조성이 가장 안정적인 동결보호 효과를 나타내어 후속 평가에 적용하였다. 최종 선별된 동 결보호제에서 보관된 세포의 성능을 평가하기 위해 일정 기간 동안 동결보존 후 단계적으로 해동하여 분석을 수행하였다. 해동 직후 생존율은 모든 시점에서 DMSO 기반 동결보호제와 통계적인 차이가 없었으며 세포 증식율 분석(Cumulative population doubling level assay)에서도 두 조건간 통계적인 차이가 확인되지 않았다. 또한 면역표현형 분석 결과, CD105+/CD73+ 발현이 유지되었으며 CD31 발현은 음성으로 확인되어 MSC의 정체성이 보존 됨을 확인하였다. 골 및 지방세포로의 분화능 또한 DMSO 기반 조건과 유사한 수준으로 유지되었으며, 콜로니 형성능 분석에서는 EG + PEG 조건에 서 더 많은 개수의 콜로니 형성이 확인되어 자가재생능 측면에서 우수한 결과를 보였다. 이상의 결과를 통해 7.5% EG + 7.5% PEG 조성은 UC-MSC 의 생존, 증식능, 정체성, 다분화능뿐만 아니라 자가재생능까지 안정적으로 유지할 수 있는 효과적인 DMSO 대체 동결보존 조성임을 확인하였다. 본 연구는 DMSO 사용에 따른 독성 및 부작용 우려를 줄일 수 있는 대안을 제시하며, 향후 세포치료제의 임상적 안전성 향상에 기여할 수 있을 것으로 기대된다.

Mesenchymal stem cell (MSC) are multipotent adult stem cells capable of promoting tissue regeneration and modulating the immune microenvironment, offering therapeutic potential for various diseases. Due to their low immunogenicity and multilineage differentiation capacities, they are considered a key cell source for the development of cell-based therapies. With the increasing number of clinical trials for degenerative, immune-related, and refractory diseases, the importance of cell preservation technologies that can maintain cell quality until the point of administration has been emphasized. Cryopreservation has therefore become a critical technological basis for the manufacturing and distribution of cell therapeutics. Dimethyl sulfoxide (DMSO) is widely used as a cryoprotectant for MSC preservation; however, its cytotoxicity and potential adverse events during clinical administration present substantial limitations. Thus, this study aimed to establish a DMSO-free cryoprotective formulation based on complementary effects of penetrating and non-penetrating cryoprotectants for the cryopreservation of umbilical cord-derived MSCs (UC-MSCs). Penetrating cryoprotectants including ethylene glycol (EG), propylene glycol (PG), and glycerol were combined with non-penetrating cryoprotectants polyethylene glycol (PEG), mannitol, and trehalose for primary screening, which evaluated post-thaw cell viability. Among the tested combinations, formulations containing EG and PEG demonstrated superior cryoprotective efficiency. Subsequently, secondary screening was performed by optimizing the concentrations of EG and PEG, and the 7.5% EG + 7.5% PEG combination showed the most stable cryoprotective effect and was selected for further evaluation. Post-thaw functional assessment after cryostorage revealed no significant differences in cell viability compared to the DMSO-based condition at any evaluated time point. Cumulative population doubling level (CPDL) analysis also confirmed no notable difference in proliferative capacity between the two conditions. Immunophenotyping demonstrated the maintenance of MSC identity, with stable CD105+/CD73+ expression and negative CD31 expression. Differentiation toward osteogenic and adipogenic lineages was comparable to the DMSO group, and colony-forming unit-fibroblast (CFU-F) assays revealed a higher number of colonies in the EG + PEG group, indicating superior self-renewal potential. Collectively, these findings demonstrate that the 7.5% EG + 7.5% PEG formulation is an effective DMSO-free cryopreservation strategy that preserves the viability, proliferative capacity, identity, multipotency, and self-renewal ability of UC-MSCs. This study suggests a promising alternative that may alleviate safety concerns associated with DMSO use, and further contribute to improving the clinical safety of cell-based therapies.
