본 연구는 요양병원 간호사의 환자안전문화 인식에 영향을 미치는 요인을 파악하고자 수행된 서술적 조사연구이다.
연구대상자는 전라남도 지역에 소재한 요양병원 병동에서 근무하고 있는 간호사 중, 연구 목적을 이해하고 연구 참여에 서면 동의한 자로, 요양병원 근무경력이 6개월 이상인 간호사를 편의 표집 하였다. 자료 수집은 2025년 4월 9일부터 2025년 4월 25일까지 구조화된 설문지를 통해 수집하였으며, 최종 154부의 설문지를 분석에 사용하였다. 연구 도구는 강보라와 오희영(2018)의 윤리적 간호역량 도구, 김미영 등(2016)의 간호 위임 태도 및 간호 위임 준비성 도구, Lee와 Dahinten (2021)의 환자안전문화 측정 도구인 K-HSOPSC 2.0(Korean-language version of the revised hospital survey on patient safety culture) 을 사용하였다. 수집된 자료는 IBM SPSS statistics WIN version 26.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficient, multiple regression으로 분석하였다.
본 연구의 결과를 요약하면 다음과 같다.
1. 환자안전문화 인식 정도는 5점 척도에서 전체 평균은 3.52±0.52점, 윤리적 간호역량은 4점 척도에서 전체 평균은 3.07±0.30점, 간호 위임 태도는 5점 척도에서 전체 평균은 3.00±0.41점, 간호 위임 준비성은 5점 척도에서 전체 평균은 3.74±0.55점이었다.
2. 대상자의 일반적 특성 중 연령(F=6.558, p<.001), 직급(F=9.657, p<.001), 총 요양병원 근무경력(F=4.186, p=.007), 현재 부서 근무경력(F=3.533, p=.016), 근무 병동의 간호사 1인당 환자 수(t=2.381, p=.019), 근무 병원의 병상 수(F=10.709, p<.001), 근무 병원의 환자안전 지침 유무에 대한 대상자의 인식(t=2.074, p=.040), 환자안전과 관련한 교육 유무(t=5.357, p<.001)에서 환자안전문화 인식과 통계적으로 유의한 차이를 보였다.
3. 상관분석 결과, 윤리적 간호역량(r=.503, p<.001), 간호 위임 준비성(r=.499, p<.001)은 환자안전문화 인식과 유의한 양의 상관관계를 보였다.
4. 회귀분석 결과, 윤리적 간호역량(β=.350, p<.001), 환자안전과 관련한 교육 받음(β=.261, p<.001), 간호 위임 준비성(β=.205, p=.008), 직급(수간호사)(β=.163, p=.023) 순으로 요양병원 간호사의 환자안전문화 인식에 유의한 영향 요인이었으며, 모형의 설명력은 43.8%였다.
이상의 연구 결과, 요양병원 간호사의 환자안전문화 인식에 영향을 미치는 요인으로 윤리적 간호역량, 환자안전 관련 교육, 간호 위임 준비성, 직급으로 확인되었다. 이에 따라 윤리적 환경 조성과 함께 요양병원 특성을 반영한 윤리 및 위임 교육, 위임 가이드라인 제공 등 간호사 개인의 역량 강화를 위한 조직 차원의 지원이 필요함을 시사한다.

Purpose: This descriptive survey study aimed to identify the factors influencing the perception of Patient Safety Culture among nurses working in long-term care hospitals.
Methods: Participants were nurses with at least six months of work experience at long-term care hospital wards located in Jeollanam-do, South Korea, who voluntarily agreed to participate after understanding the purpose of the study. Data were collected from April 9 to April 25, 2025, using structured questionnaires, and a total of 154 responses were included in the final analysis.
The instruments used in this study included the Ethical Nursing Competence Tool developed by Kang and Oh (2018), the Nurses’ Attitudes toward Delegation and Nurses’ Preparedness for Delegation tools developed by Kim et al. (2016), and the K-HSOPSC 2.0 (Korean-language version of the revised Hospital Survey on Patient Safety Culture) developed by Lee and Dahinten (2021).
Data were analyzed using descriptive statistics, t-tests, ANOVA, Scheffé tests, Pearson’s correlation coefficients, and multiple regression analysis with IBM SPSS Statistics version 26.0.
Results:
1. The mean score for Patient Safety Culture perception was 3.52±0.52 on a 5-point scale. The mean scores were 3.07±0.30 for Ethical Nursing Competence (4-point scale), 3.00±0.41 for Nurses’ Attitudes toward Delegation (5-point scale), and 3.74±0.55 for Nurses’ Preparedness for Delegation (5-point scale).
2. Significant differences in Patient Safety Culture perception were found according to general characteristics including age (F=6.558, p<.001), position (F=9.657, p<.001), total experience in long-term care hospitals (F=4.186, p=.007), experience in current department (F=3.533, p=.016), nurse-to-patient ratio in current ward (t=2.381, p=.019), number of hospital beds (F=10.709, p<.001), awareness of hospital patient safety guidelines (t=2.074, p=.040), and experience of patient safety education (t=5.357, p<.001).
3. Ethical Nursing Competence (r=.503, p<.001) and Nurses’ Preparedness for Delegation (r=.499, p<.001) showed significant positive correlations with Patient Safety Culture perception.
4. Multiple regression analysis identified Ethical Nursing Competence (β=.350, p<.001), experience of patient safety education (β=.261, p<.001), Nurses’ Preparedness for Delegation (β=.205, p=.008), and charge nurse position (β=.163, p=.023) as significant predictors of Patient Safety Culture perception. The explanatory power of the model was 43.8%.
Conclusion: Based on the findings, Ethical Nursing Competence, experience with patient safety education, Nurses’ Preparedness for Delegation, and professional position were identified as significant factors influencing the perception of Patient Safety Culture among nurses in long-term care hospitals. These results suggest the need for organizational-level support to strengthen individual nursing competencies. Specifically, efforts should focus on fostering an ethical practice environment and providing ethics and delegation education tailored to the characteristics of long-term care settings, along with clear delegation guidelines.
