    본 논문은 효과적인 종양 치료를 위한 새로운 약물전구체 (Prodrug) 기술 개발에 관한 것이다. 약물전구체는 생체 내 투여 후, 대사 과정에서 약리학적 활성을 나타내는 물질이다. 이는 종양 선택적 치료 효율이 매우 낮은 1세대 약물의 부작용을 개선할 수 있는 방법으로 부작용을 크게 감소시킬 수 있는 주목 받는 기술이다. 특히 종양미세환경에 특이적으로 과 발현되는 효소에 의해 대사되어 약리학적 활성을 나타내게 하는 방법을 통하여, 최근 연구에서도 부작용을 감소시킬 수 있는 효과적인 종양 치료 방법으로 많은 주목을 받아왔다. 본 연구에서는 효율적인 약물전구체 전달을 위한 새로운 방법을 제시해보았다.
    약물전구체는 종양 특이적 활성화가 가능하여 부작용을 감소시키는 장점이 있는 반면에, 종양 표적화 효율이 충분하지 않기 때문에, 치료 효과가 매우 낮은 단점이 있다. 이러한 문제점을 해결하기 위해 약물전구체의 종양 표적화 효율을 높이기 위한 많은 연구들이 진행되어왔다. 이는 주로 고분자 기반의 나노입자 (Nanoparticle)를 전달체로 이용하여 약물전구체의 전달 효율을 높이는 방법에 집중해 왔다. 하지만 기존의 연구들만으로는 약물전구체의 전달 효율을 높이기에 충분하지 않아서 임상적 사용이 제한되었다. 고분자 기반의 나노입자에 약물전구체를 적재하는 방법은 적재 효율이 크게 떨어지기 때문에 많은 양의 약물을 종양에 전달 할 수 없었다. 또한 대부분의 고분자 나노입자는 체내에서의 배출이 용이하지 않아 추가적인 독성이 나타날 가능성이 높기 때문에 연구가 필요하다. 따라서 본 연구에서는, 약물전구체의 종양 표적화 효율을 높이기 위해, 고분자 나노입자를 이용하지 않고, 약물전구체 자체가 나노약물전달체를 형성하도록 개발하여, 종양 동물 모델에서의 생체 내 거동과 종양 치료 효과를 확인하였다. 이러한 나노약물전달체는 종양미세환경에서 특이적으로 과 발현되는 카텝신B (Cathepsin B) 효소에 의해 특이적으로 활성화되는 것을 확인 할 수 있었다. 또한 나노약물전달체는 종양 동물 모델에서 종양 표적화 효율이 높은 반면에 나노입자를 형성하지 않는 약물전달체는 나노약물전달체에 비해 매우 낮은 종양 표적화 효율이 나타나는 것을 확인할 수 있었다. 그 결과, 위 방법을 통해 약물전달체의 종양 표적 효율을 증진시킬 수 있었고 부작용이 크게 감소하여, 치료 효율이 비약적으로 증가한 것을 확인할 수 있었다.
    약물전달체의 종양 표적화 효율을 비약적으로 증진시킬 수 있는 방법을 개발하였으나, 이는 약물 내성 (Drug resistance)을 가지는 종양세포는 치료 효율이 크게 감소한다는 한계점을 가지고 있어, 다양한 약물 내성이 나타나는 임상적 관점에서 봤을 때 효과가 제한적이다. 약물 내성은 약물에 노출 된 종양 세포가 약물에 저항성을 가질 수 있는 인자를 과발현시켜 치료 효과가 감소하는 현상이다. 특히, 약물에 노출되었을 때, 과발현되는 세포 사멸 억제 단백질 (Inhibitor of apoptosis proteins, IAPs)는 종양 세포가 약물 내성을 가지게 하여, 치료 효과를 감소시킨다. 따라서 본 연구에서는, 약물 내성을 가지는 종양세포에도 치료 효과를 나타낼 수 있는 약물전구체를 개발하는데 집중해왔다. 이 방법은 나노약물전구체 (FRRG-DOX)에 세포 사멸 억제 단백질의 발현을 억제시킬 수 있는 단백질 (SMAC/DIABLO)을 추가적으로 결합하였고, 이는 세포 내에서 카텝신B에 의해 특이적으로 활성화된다. 그 결과, 약물 (Doxorubicin)과 약물 내성 단백질 억제제 (SMAC/DIABLO)를 동시에 활성화하여 약물 내성을 가지는 종양세포까지 사멸시킬 수 있는 방법이다. 이 약물전구체는 나노입자를 형성하여 종양 동물 모델의 체내에 주입하였을 때 종양 조직에 축적되는 EPR 효과에 의해 종양 조직에 선택적으로 많이 축적되어 부작용 없이 치료 효율을 증진시켰다. 특히, 약물 내성 단백질 억제제를 결합하지 않은 약물전구체 (FRRG-DOX)보다 종양 치료 효율이 크게 증가함을 확인할 수 있었다. 추가적으로, 종양 동물 모델의 종양을 적출하여 전기영동법과 조직 염색을 통하여 종양 조직 내의 세포 사멸 억제 단백질의 분포를 확인한 결과, 약물 내성 단백질 억제제를 추가적으로 결합한 약물전구체를 주입한 동물의 종양에서 세포 사멸 억제 단백질이 크게 감소하였음을 확인 할 수 있었다. 결과적으로 이 약물전구체는 나노입자를 형성하여 종양 표적 효율이 비약적으로 증진되어 치료 효율이 증가하였고, 추가적으로 결합한 약물 내성 단백질 억제제가 세포 사멸 억제 단백질의 발현을 크게 감소시켜, 약물 내성을 가지는 종양 세포까지도 치료가 가능한 것을 확인 할 수 있었다.
    약물 내성 단백질 억제제와 약물을 전달할 수 있는 약물전구체를 개발하여 종양의 재발시기를 비약적으로 늦추는 것은 가능하였으나, 종양세포에 내성을 나타나게 하는 기전은 매우 다양하기 때문에 동물모델의 종양을 완치하기에는 한계가 있었다. 이에 종양을 완치할 수 있는 약물전구체를 개발하기 위해 면역 체크 포인트 봉쇄가 가능한 펩타이드 기반의 PD-L1 길항제를 나노약물전구체에 추가적으로 결합하였다. 체크 포인트 봉쇄는 종양세포와 면역세포의 상호작용을 교란하여 면역세포가 종양세포를 사멸 시킬 수 있는 방법으로, 최근 면역치료 분야에서 활발히 연구되고 있다. 따라서 본 연구에서는, PD-L1 길항제를 결합한 나노약물전구체를 이용하여 약물과 면역치료제를 전달 할 수 있는 방법을 개발하였다. 이는 종양세포에서 특이적으로 활성화되어 약물에 의해 세포 사멸을 유도하고, 동시에 PD-L1 길항제에 의하여 면역세포가 종양세포를 인식할 수 있도록 하는 방법이다. 이는 생체 내에서 선택적으로 종양 조직에 축적되었으며, 카텝신 B에 의해 약물과 면역치료제를 동시에 활성화시켜 치료 효율이 크게 증가하는 것을 확인할 수 있었다. 추가적으로 종양 동물 모델의 종양을 적출하여 유세포분석기와 조직 염색을 통하여 종양 조직 내의 면역세포의 수와 T세포의 수를 확인한 결과, PD-L1 길항제를 결합한 나노약물전구체를 주입한 동물의 종양에서 크게 증가하였음을 확인할 수 있었다. 이 방법은 종양에 면역치료제와 약물을 동시에 전달하여 종양의 치료 효율을 극대화할 수 있으며 선택적인 치료가 가능하여 임상 적용을 위한 부작용을 해결할 수 있음을 확인할 수 있었다.
    임상적용이 가능한 항암제는 선택적인 치료를 통해 부작용을 최소화함과 동시에 종양이 재발하지 않도록 치료 효율을 극대화할 수 있는 물질이다. 또한, 임상적 관점에서의 종양은 표현형이 매우 다양하기 때문에 환자의 표현형에 맞는 다양한 항암제의 개발은 중요하다. 본 연구는 항암치료의 부작용을 크게 감소시킬 수 있는 약물전구체의 치료 효율을 증진시킬 수 있는 다양한 방법에 관한 것이다. 본 연구에서 개발한 다양한 약물전구체는 자체가 나노입자를 형성하기 때문에 생체 내에서 선택적인 종양 치료가 가능하였고 약물의 전달 효율이 크게 증가하였다. 이는 기존의 약물전구체의 한계점이었던 치료 효율을 증진시킬 수 있었고 독성을 최소화 하여 임상 적용이 가능한 약물이 될 것으로 기대된다.

    Prodrug is a agent that exhibits pharmacological activity in the metabolic process after
    in vivo administration. This is an effective way to improve the side effects of 1st generation drugs
    (such as doxorubicin) with low selectivity. In particular, prodrug was selectively activated by
    overexpressed enzymes in the cancer microenvironment, thus reducing the side effects of
    conventional drugs. In this study, we proposed new approaches to improve the efficiency of the
    prodrug.
    Prodrug can reduce side effects through cancer-specific activation by overexpressed
    enzymes in cancer. However, there is limitations that the therapeutic efficacy is very low because
    the cancer targeting efficiency is not sufficient. In recent study, polymer-based nanoparticles have
    been used to improve the delivery efficiency of prodrugs but, polymer nanoparticle system have
    limatiation that loading efficiency is very low resulting in therapeutic efficacy is not sufficient.
    Because most of polymer-based nanoparticles can occur toxicity because they are not easily
    removed from the body, more studies for the delivery of prodrug is essential. In Chapter 2, to
    improve the cancer targeting efficiency of the prodrug, we prepared carrier-free cathepsin B
    activatable prodrug nanoparticles without any polymer. This prodrug could be specifically
    activated by cathepsin B, which is overexpressed in the cancer microenvironment, and it revealed
    that the cancer targeting efficiency was improved in the cancer-bearing animal model. As a result,
    the therapeutic efficacy of this prodrug was dramatically increased by minimizing the toxicity and
    improving the cancer targeting efficiency.
    In chapter 2, we demonstrated that carrier-free cathepsin B activatable prodrug
    nanoparticles can improve the cancer targeting efficiency of prodrug and therapeutic efficacy for
    cancer treatment were dramatically increased. However, this has limitation from the clinical
    perspective because this prodrug can not affect to cancer cells with drug-resistance. When the
    cancer cells are continuously exposed to the drug, the therapeutic efficacy is reduced by
    overexpressing a drug-resistance factor. In particular, IAPs (Inhibitor of apoptosis proteins) were
    overexpressed when the cancer cells were continuously exposed to the drug, resulting in the
    cancer cells becoming resistant to the drug. In recent study, SMAC/DIABLO has been widely
    used to suppress the expression of IAPs for overcoming drug-resistance in cancer. In Chapter 3,
    New carrier-free cathepsin B activatable prodrug nanoparticles were not only selectively
    accumulated in the cancer when injected into the animal model but can affect cancer cells with
    drug-resistance by synergetic effect between drug and SMAC/DIABLO. In particular, the
    therapeutic efficacy was significantly improved compared with the carrier-free prodrug without
    SMAC/DIABLO, and it was confirmed by western blot analysis and histology that SMAC was
    suppressed IAPs in cancer. As a result, new prodrug have highly cancer targeting efficiency and
    affects cancer cells with drug-resistance resulting in maximizing therapeutic efficacy.
    In chapter 3, we suggested new prodrug that can delayed the recurrence of the cancer by
    delivering drug-resistance inhibitors and drugs. Because mechanisms that cause drug-resistance
    are diverse, control the cancer with this prodrug that can only suppress IAPs is difficult. In
    Chapter 4, PD-L1 peptide antagonist was conjugated to the carrier-free cathepsin B activatable
    prodrug nanoparticles for the cancer control. Recently, many studies have been widely conducted
    to control cancer using immune check-point blockade, which can interfere with the interaction
    between immune cells and tumor cells. In this study, we proposed immunogenic carrier-free
    cathepsin B activatable prodrug nanoparticles that can induce apoptosis and immune-response for
    cancer control. This prodrug is composed of drug and immune check-point blockade, which not
    only induces apoptosis but causes immune-response to cancer. Whole immune cells and T-cells
    were significantly increased in cancer of animal model injected with immunogenic prodrug. As a
    result, immunogenic prodrug can deliver drug and immune checkpoint blockade to the tumor to
    maximize therapeutic efficacy and minimize toxicity.
    Anticancer drug should minimize side effects and maximize therapeutic efficacy to
    prevent cancer-recurrence for clinical applications. Because the cancer has heterogeneity in
    clinical, it is important to develop various anticancer drugs that are appropriate for the patient’s
    phenotype. This study is concerned with various methods that can improve the therapeutic
    efficacy of prodrugs which can reduce toxicity of conventional chemotherapy. The various
    prodrugs developed in this study were formed nanoparticles resulting in delivery efficiency was
    dramatically increased. We expect this carrier-free prodrug nanoparticle systems can overcome
    the limitation of current cancer treatment for clinical applications.

    In chapter 2, we demonstrated that carrier-free cathepsin B activatable prodrug
    nanoparticles can improve the cancer targeting efficiency of prodrug and therapeutic efficacy for
    cancer treatment were dramatically increased. However, this has limitation from the clinical
    perspective because this prodrug can not affect to cancer cells with drug-resistance. When the
    cancer cells are continuously exposed to the drug, the therapeutic efficacy is reduced by
    overexpressing a drug-resistance factor. In particular, IAPs (Inhibitor of apoptosis proteins) were
    overexpressed when the cancer cells were continuously exposed to the drug, resulting in the
    cancer cells becoming resistant to the drug. In recent study, SMAC/DIABLO has been widely
    used to suppress the expression of IAPs for overcoming drug-resistance in cancer. In Chapter 3,
    New carrier-free cathepsin B activatable prodrug nanoparticles were not only selectively
    accumulated in the cancer when injected into the animal model but can affect cancer cells with
    drug-resistance by synergetic effect between drug and SMAC/DIABLO. In particular, the
    therapeutic efficacy was significantly improved compared with the carrier-free prodrug without
    SMAC/DIABLO, and it was confirmed by western blot analysis and histology that SMAC was
    suppressed IAPs in cancer. As a result, new prodrug have highly cancer targeting efficiency and
    affects cancer cells with drug-resistance resulting in maximizing therapeutic efficacy.
    In chapter 3, we suggested new prodrug that can delayed the recurrence of the cancer by
    delivering drug-resistance inhibitors and drugs. Because mechanisms that cause drug-resistance
    are diverse, control the cancer with this prodrug that can only suppress IAPs is difficult. In
    Chapter 4, PD-L1 peptide antagonist was conjugated to the carrier-free cathepsin B activatable
    prodrug nanoparticles for the cancer control. Recently, many studies have been widely conducted
    to control cancer using immune check-point blockade, which can interfere with the interaction
    between immune cells and tumor cells. In this study, we proposed immunogenic carrier-free
    cathepsin B activatable prodrug nanoparticles that can induce apoptosis and immune-response for
    cancer control. This prodrug is composed of drug and immune check-point blockade, which not
    only induces apoptosis but causes immune-response to cancer. Whole immune cells and T-cells
    were significantly increased in cancer of animal model injected with immunogenic prodrug. As a
    result, immunogenic prodrug can deliver drug and immune checkpoint blockade to the tumor to
    maximize therapeutic efficacy and minimize toxicity.
    Anticancer drug should minimize side effects and maximize therapeutic efficacy to
    prevent cancer-recurrence for clinical applications. Because the cancer has heterogeneity in
    clinical, it is important to develop various anticancer drugs that are appropriate for the patient’s
    phenotype. This study is concerned with various methods that can improve the therapeutic
    efficacy of prodrugs which can reduce toxicity of conventional chemotherapy. The various
    prodrugs developed in this study were formed nanoparticles resulting in delivery efficiency was
    dramatically increased. We expect this carrier-free prodrug nanoparticle systems can overcome
    the limitation of current cancer treatment for clinical applications.
