    본 연구의 목적은 노년층 신경언어·인지장애를 대상으로 한 비대면 인지·의사소통 중재의 효과를 체계적으로 고찰하여, 효과가 반복적으로 관찰되는 의사소통 기능 영역과 중재 운영 특성을 종합적으로 제시하고 향후 임상 및 연구 방향을 도출하는 데 있다. 고령화에 따른 신경계 질환 유병률 증가와 더불어 정보통신기술 발달 및 팬데믹 이후 비대면 서비스 수요가 확대되는 상황에서, 비대면 중재가 노년층의 의사소통 수행에 어떠한 영향을 미치는지 근거 기반으로 정리하고자 하였다.
    이를 위해 PRISMA 가이드라인에 따라 국내(RISS, KISS, DBpia) 및 국외(PubMed, CINAHL, SCOPUS, Web of Science) 데이터베이스에서 키워드 검색을 수행하였다. 최종 포함 문헌은 11편이었으며, 동일 연구 내 서로 다른 중재 프로토콜을 분리하여 분석한 결과 중재 단위는 12건이었다. 연구 정보(저자, 출판연도, 국가), 연구 설계, 대상자 특성(진단군, 표본 수, 연령), 중재 목표·내용·용량, 제공 방식(동기/비동기/혼합), 플랫폼·도구, 치료자 관여 수준 및 의사소통 파트너 참여 여부, 결과지표와 결과를 추출하였다. 비뚤림 위험 평가는 무작위배정 연구에 RoB 2.0, 비무작위/단일군/사례 연구에 RoBANS 2.0을 적용하였다. 연구 설계와 중재·결과지표의 이질성이 커 통계적 메타분석은 수행하지 않고 서술적 통합을 실시하였다. 효과는 도구 간 차이를 고려하여 어휘–의미, 형태–구문, 화용(담화·스크립트 산출 및 기능적 의사소통·참여·삶의 질), 읽기, 인지와 의사소통(전반 인지)의 5개 상위 범주로 재구성하여, 유의한 향상(Y), 비유의 또는 혼재(NS), 미제시(공란)로 코딩하였다.
    연구 결과, 포함 연구는 국외 연구 비중이 높았고 대상 진단군은 신경언어장애(실어증, PPA)가 다수를 차지했으며, 신경인지장애(MCI, AD) 연구는 제한적이었다. 비뚤림 위험은 전반적으로 ‘일부 우려’가 가장 큰 비중을 차지하였고, 비무작위 연구에서는 교란변수 통제와 평가자 눈가림 관련 취약성이 반복적으로 확인되었다. 효과 분석에서 어휘–의미 도메인은 표적 과제 중심의 어휘 인출·명명 수행에서 훈련효과가 비교적 일관되게 보고되었으나, 일반화·유지효과는 연구 간 변동성이 크거나 보고가 제한적이었다. 화용 도메인에서는 담화·스크립트 산출의 정확도 및 일부 질적 지표에서 개선이 확인되었으며, 기능적 의사소통 성과와 의사소통 관련 삶의 질 지표는 연구 간 결과가 제한적이거나 혼재하였다. 형태–구문 도메인은 문장이해 및 문장 산출 관련 지표에서 일부 유의한 개선이 보고되었으나, 보고 연구 수가 적어 일반화에 제약이 있었다. 읽기/문어 수행과 전반 인지 선별점수는 유의한 개선으로 분류된 결과가 확인되지 않았다.
    종합하면, 노년층 신경언어·인지장애 대상 비대면 중재의 효과는 전반 인지의 향상보다는 표적 언어 과제 수행과 담화·스크립트 산출과 같은 의사소통 수행 지표에서 더 반복적으로 관찰되었다. 다만 전이·지속 효과 및 기능적 의사소통/삶의 질과 같은 임상적으로 직접적인 성과는 근거가 제한적이거나 결과가 혼재하였으며, 연구 설계 및 보고의 이질성과 비뚤림 위험을 고려한 보수적 해석이 요구된다.

    The purpose of this study was to systematically review the effects of telepractice-based cognitive–communication interventions for older adults with neurogenic language and cognitive disorders, to identify communication domains in which effects are repeatedly observed, to synthesize key implementation characteristics, and to propose implications for clinical practice and future research. Given population aging, an increasing prevalence of neurological conditions, and the growing demand for remote services following advances in information and communication technologies and the COVID-19 pandemic, this review aimed to summarize the evidence on how telepractice interventions influence older adults’ cognitive–communication performance.
    Following the PRISMA guidelines, a systematic search was conducted in domestic (RISS, KISS, DBpia) and international (PubMed, CINAHL, SCOPUS, Web of Science) databases. A total of 11 studies met the inclusion criteria. Because one study reported two distinct intervention protocols, outcomes were extracted and analyzed separately, yielding 12 intervention units. Data were extracted on study information (authors, publication year, country), study design, participant characteristics (diagnostic group, sample size, age), intervention targets, content and dosage, delivery mode (synchronous, asynchronous, or hybrid), platforms/tools, level of clinician involvement, involvement of communication partners/caregivers, and outcome measures and results. Risk of bias was assessed using RoB 2.0 for randomized controlled trials and RoBANS 2.0 for non-randomized, pre–post, and single-case designs. Due to substantial heterogeneity in study design, interventions, and outcome measures, meta-analysis was not conducted; instead, findings were synthesized narratively. To enhance comparability across measures, outcomes were reorganized into five higher-level domains—lexical–semantic (word retrieval/naming), morpho-syntactic (sentence-level processing), pragmatic (discourse/script production and functional communication/participation/quality of life), reading, and cognition and communication (global cognition)—and coded as significant improvement (Y), non-significant or mixed findings (NS), or not reported/indeterminable (blank).
    Most included studies were conducted outside Korea, and neurogenic language disorders (aphasia, primary progressive aphasia [PPA]) comprised the majority of diagnostic groups, whereas studies targeting neurocognitive disorders (mild cognitive impairment [MCI], Alzheimer’s disease [AD]) were limited. Overall risk of bias was most frequently rated as “some concerns,” and non-randomized studies repeatedly showed weaknesses related to confounding control and assessor blinding. In the effects analysis, the lexical–semantic domain showed relatively consistent training effects on targeted word retrieval/naming tasks; however, generalization and maintenance effects were variable across studies or infrequently reported. In the pragmatic domain, improvements were observed in discourse/script production accuracy and some qualitative indices, whereas functional communication outcomes and communication-related quality of life measures were limited or mixed. For the morpho-syntactic domain, some studies reported significant improvements in sentence comprehension and sentence production, but the small number of studies constrained generalization. No outcomes were classified as significant improvements for reading/written-language performance or global cognitive screening scores.
    In summary, telepractice interventions for older adults with neurogenic language and cognitive disorders more consistently demonstrated effects on communication performance—particularly targeted language-task outcomes and discourse/script production—than on global cognition. Nevertheless, evidence for transfer and durability of treatment effects, as well as clinically proximal outcomes such as functional communication and quality of life, remains limited or mixed, warranting cautious interpretation in light of heterogeneity and risk-of-bias concerns.
