This study compared and analyzed the results of FilmArrayⓇ GI panel assay and stool culture test performed at a single institution over a two-year period following the end of the international public health emergency for COVID-19, thereby confirming the clinical utility of the FilmArrayⓇ GI panel assay. The FilmArrayⓇ GI panel assay demonstrated a significantly higher positive rate and broader pathogen spectrum compared to stool culture test. The positive rates for the FilmArrayⓇ GI panel assay and stool culture test were 48.7% (1,061/2,179) and 3.0% (43/1,436), respectively, showing a significant difference (P < 0.001). The FilmArrayⓇ GI panel assay detected a total of 1,514 pathogens: 1,164 bacterial, 343 viral, and 7 parasitic. In stool culture test, bacteria were detected in 43 cases. The FilmArrayⓇ GI panel assay detected Clostridioides difficile in 496 cases and diarrheagenic E. coli/Shigella in 281 cases, whereas stool culture detected none in either bacterium. The FilmArrayⓇ GI panel assay detected Salmonella spp. in 114 cases and Campylobacter spp. in 98 cases, whereas stool culture detected Salmonella spp. in 40 cases and Campylobacter spp. in 2 cases. In the age- and gender-specific analysis, the positive rate of the FilmArrayⓇ GI panel assay was significantly higher than that of stool culture (P < 0.001). The positive rates for bacteria, viruses, and parasites in the FilmArrayⓇ GI panel assay were 39.8%, 14.7%, and 0.3%, respectively. Among the 664 cases where both tests were ordered simultaneously, FilmArrayⓇ GI panel assay alone was positive in 351 cases (52.9%), while stool culture alone was positive in 0 cases. The TAT for the FilmArrayⓇ GI panel assay was significantly shorter at an average of 82 minutes compared to the average of 3,838 minutes for stool culture test (P < 0.001). In conclusion, this study demonstrated that the FilmArrayⓇ GI panel assay offers advantages over traditional stool culture test in terms of rapidity, comprehensiveness, and high sensitivity, proving effective in accurately and rapidly diagnosing pathogens including bacteria, viruses, and parasites that are not detectable by culture. These results suggest that the FilmArrayⓇ GI panel assay plays a crucial role in clinical treatment decisions and public health crisis management, necessitating the establishment of a complementary system alongside traditional stool culture test. However, as this study was conducted at a single institution in Daegu, its generalizability is limited, and more detailed analysis through future multi-institutional and national-level follow-up studies is required.
