최하 위 소득군에 비해 중·하위군 (OR=1.18, 95% CI: 1.01–1.38), 중·상위군(OR=1.21, 95% CI: 1.04-1.41), 최고소득군(OR=1.22, 95% CI: 1.05-1.43)에서 위험도가 상 승하였고, 초등학교 미만 대비 중학교 졸업군(OR=1.44, 95% CI: 1.21-1.70), 고 등학교 졸업군(OR=1.21, 95% CI: 1.04-1.41), 대학 이상 졸업군(OR=1.20, 95% CI: 1.02-1.41)에서도 증가하였다.

Background: Hepatitis B virus (HBV) and hepatitis C virus (HCV) infections are major causes of liver disease in Korea. Understanding the relationship between socioeconomic status (SES) and the prevalence of HBV and HCV is essential for developing effective prevention and management strategies. This study aimed to examine the associations of SES, defined by education, household income, and occupation, with the prevalence of HBV and HCV in Korean adults.
Methods: Data were obtained from 61,811 adults aged ≥19 years who participated in the Korea National Health and Nutrition Examination Survey (2012–2023). The mean age of the study population was 51.21 ± 16.6 years. The age distribution was as follows: ≤29 years, 12.4%; 30–39 years, 15.1%; 40–49 years, 18.1%; 50–59 years, 19.3%; 60–69 years, 18.8%; 70–79 years, 13.0%; and ≥80 years, 3.3%. SES was defined by educational attainment, household income, and occupation. HBV infection was defined as hepatitis B surface antigen (HBsAg) positivity, and HCV infection was defined as anti-HCV antibody positivity. Logistic regression analyses were performed to estimate adjusted odds ratios (ORs) for the associations between SES indicators and HBV or HCV prevalence, adjusting for age, sex, marital status, smoking, alcohol consumption, physical activity, and body mass index category.
Results: The prevalence of HBV and HCV was 3.1% and 0.8%, respectively. In multivariable analyses, a higher SES was significantly associated with an increased risk of HBV infection. Compared with the lowest income quartile, the adjusted ORs were 1.18 (95% CI: 1.01–1.38), 1.21 (95% CI: 1.04–1.41), and 1.22 (95% CI: 1.05–1.43) for the lower-middle, upper-middle, and highest income groups, respectively. Compared with less than primary education, individuals with middle school (OR=1.44, 95% CI: 1.21–1.70), high school (OR=1.21, 95% CI: 1.04–1.41), and college or higher education (OR=1.20, 95% CI: 1.02–1.41) showed a higher risk of HBV infection. By occupation, Service/sales (OR=1.22, 95% CI: 1.05–1.41) and manual/agricultural workers (OR=1.17, 95% CI: 1.03-1.32) also showed higher risks than the economically inactive group. These effects were more pronounced among men (p < 0.05). In contrast, HCV risk was higher in lower SES groups: compared with less than primary education, those with high school (OR=0.73, 95% CI: 0.56–0.95) and college or higher education (OR=0.56, 95% CI: 0.41–0.76) had lower risks. Household income tended to have a lower risk as income levels increased, but this association was not statistically significant. No significant differences were observed across occupational groups, and no interaction by sex was identified.
Conclusions: This study identified distinct socioeconomic patterns in the prevalence of HBV and HCV among Korean adults, with HBV being more common in higher socioeconomic groups and HCV in lower socioeconomic groups. These findings suggest that socioeconomic factors should be considered in hepatitis prevention policies and high-risk population management strategies
