Objective: This study aimed to classify out-of-hospital cardiac arrest (OHCA) patients who received bystander cardiopulmonary resuscitation (CPR) into neurological non-deterioration and deterioration groups based on changes in Cerebral Performance Category(CPC) scores before and after cardiac arrest, and to identify factors associated with neurological outcome changes. Methods: A total of 1,400 adult patients aged 18 years or older were included from the 2023 Sudden Cardiac Arrest Survey who had received bystander CPR and had documented CPC scores both before cardiac arrest and at emergency department(ED) discharge. General and clinical characteristics were presented as frequencies and percentages. Participants were categorized into non-deterioration and deterioration groups according to changes in CPC scores before and after cardiac arrest. Differences between the two groups were analyzed using the chi-square test and independent t-test. To identify factors associated with neurological outcome changes, univariate logistic regression analysis was performed, followed by multivariate logistic regression analysis including statistically significant variables and clinically relevant factors. Results: In the multivariate logistic regression analysis, the risk of CPC score deterioration was 1.596 times higher among patients residing in non-metropolitan areas(OR=1.596, 95% CI=1.11–2.31, p=0.013), 3.449 times higher when an automated external defibrillator (AED) was not used by emergency medical services(OR=3.449, 95% CI=2.23–5.34, p<0.001), and 8.474 times higher in patients without pre–emergency department return of spontaneous circulation(ROSC)(OR=8.47, 95% CI= 2.42–29.66, p=0.001). In contrast, the presence of a shockable rhythm on electrocardiogram at ED arrival was associated with a lower risk of CPC score deterioration(OR=0.145, 95% CI=0.035–0.597, p=0.008). Conclusion: This study identified residence in non-metropolitan areas, non-use of AEDs by EMS, absence of pre-ED ROSC, and initial electrocardiographic rhythm at ED arrival as significant factors influencing neurological outcome changes in patients with OHCA. These findings highlight the importance of prehospital interventions and suggest that bystander CPR may contribute to improved neurological outcomes through favorable changes in CPC scores.
