Background: While the overall incidence of tuberculosis (TB) in South Korea is declining, the proportion of TB cases among foreign-born individuals and their treatment interruption rates remain significantly higher than those of native Koreans, making them a primary vulnerable group in national TB control. This study systematically analyzes the development and limitations of Korea’s TB management policies for foreigners and derives effective improvement measures aligned with WHO and IOM recommendations through a comparative analysis with the United States, the United Kingdom, and Singapore.
Methods: Using a Process Tracing Design, Korea’s policies were analyzed across three stages: the Formation Period (2010–2016), the Institutionalization Period (2017–2019), and the Supplementation/Exposure of Limitations Period (2020–present). Through a Qualitative Comparative Case Study, the US, UK, and Singapore were selected for comparison across four domains: legal basis and systems, screening and diagnosis, treatment and support, and patient registration and follow-up. Evaluation was based on 26 international criteria for migrant TB management proposed by WHO/IOM, classified as implemented (○), partially implemented (△), or not implemented (×).
Results: Despite institutional progress, such as mandatory pre-entry screening, exemption of out-of-pocket costs, and free treatment for uninsured or undocumented migrants, the treatment interruption rate among foreigners was approximately ten times higher than that of Koreans (21.5% vs. 2.3%). Comparison with international standards showed that while Korea achieved an overall implementation rate of 80.8%, its "full implementation" rate was only 42.3%, revealing a significant gap between policy and practice. Four critical gaps were identified: (1) lack of real-time linkage between pre- and post-entry screening information, (2) insufficient legal mandates for latent TB infection (LTBI) management, (3) barriers to healthcare access for undocumented migrants, and (4) absence of cross-border information-sharing systems.
Conclusions: Improvement is urgent in four key areas for Korea. First, an electronic linkage system integrating the National TB Surveillance System with immigration databases must be established. Second, LTBI screening should be mandated for immigrants from high-burden countries. Third, legal provisions prohibiting identity verification and mandatory reporting during TB treatment should be explicitly stated to ensure practical access for undocumented migrants. Fourth, the Infectious Disease Control and Prevention Act should be amended to provide a legal basis for cross-border patient information sharing. This study proposes adopting the universal coverage model of the UK and the information-linkage model of the US as alternatives for the Korean model.
