As populations continue to age, there is an increasing focus on physical health due to its close relationship with quality of life. Healthy diet play a critical role in preventing metabolic syndrome and chronic diseases, and may contribute to the prevention of osteoarthritis and the maintenance of physical function in older adults. However, the associations between diet quality and these outcomes remain unclear. Moreover, since diet quality is influenced by both individual and environmental factors, it is necessary to identify the determinants of diet quality in order to address nutritional disparities.
This cross-sectional study analyzed large-scale population-based datasets. First, data from the 2003–2016 U.S. National Health and Nutrition Examination Survey (NHANES) were used to examine the association between vitamin K(VK) intake and physical function (PF), hand grip strength (HGS), and mortality among adults aged ≥50 years (n = 4,950). VK intake was assessed using two 24-hour dietary recalls and supplement use questionnaires. PF was evaluated using 19 survey items categorized into five functional domains (activities of daily living; ADL, instrumental activities of daily living; IADL), lower extremity mobility; LEM, general physical activities; GPA, and leisure and social activities; LSA). HGS was measured
using a dynamometer, and Mortality data were obtained from the linked dataset between the National Center for Health Statistics and the NHANES. Second, the association between diet quality and knee osteoarthritis (KOA) was evaluated in stratified regional groups among Korean adults aged ≥50 years (n = 2,222), using data from the KNHANES. Diet quality was measured using the Korean Healthy Eating Index (KHEI), and KOA was assessed via radiographic diagnosis. Third, to assess the interaction between individual characteristics and regional factors affecting diet quality, data on regional factors were obtained from Korean Statistical Information Service and the Community Health Survey, and individual characteristics
were assessed using data from KNHANES 2013–2018, including participants aged ≥20 years and older (n = 26,853). Regional factors included education level, annual per capita income, food security, welfare-related facilities, time to the nearest large retailer, and internet usage. Individual factors included age, education level, household income level, and single-person household status. All analyses were conducted using complex sample design methods. Group differences were assessed using chi-square and t-tests. Associations between VK intake and PF, HGS, and mortality were examined using odds ratios (ORs), correlation analysis, and Cox proportional hazards models. Associations between KHEI and KOA were analyzed using ORs, and interactions between individual and regional factors were assessed using ANOVA.
The results showed that adequate VK intake was significantly associated with lower odds of in IADL difficulty (OR = 0.60) and LEM difficulty (OR = 0.64), particularly among men. No significant associations were found with HGS or mortality. In Korean adults, higher KHEI scores were associated with lower odds of KOA (P for trend = 0.03), especially among metropolitan residents Individuals residing in regions with a higher average education level tended to have higher diet quality (P = 0.013). In metropolitan areas, individuals with higher education and income levels had better diet quality when living in regions with greater market accessibility and higher internet usage (P = 0.03, P = 0.042, respectively). Conversely, among low-income individuals, these regional advantages were negatively associated with diet quality. In rural areas, individuals with low education or income levels had higher diet quality when more physical activity facilities were available in the region (P = 0.024, P = 0.038, respectively). In conclusion, Adequate vitamin K intake and higher diet quality were associated with improved physical function and reduced OA risk among older adults. Furthermore, diet quality varied significantly depending on both individual socioeconomic status and regional environmental factors. Dietary management plays a vital role in maintaining health among older adults. To effectively address nutritional disparities and enhance population health, a tailored approach that simultaneously considers both individual-level and regional factors is essential.
