교육수준이 초졸 이하(OR=2.51, 95% CI=1.14–5.54, p=0.022) 및 중졸 (OR=3.13, 95% CI=1.32–7.46, p=0.010)인 경우 대졸 이상 집단에 비해 혈당 미조절 위험이 유의하게 높았으며, 60-69세 도시남성은 50세 미만 도시남성에 비해 혈당조 절 위험이 낮았다(OR=0.35, 95% CI=0.14~0.86, p=0.023). 50대 (OR=27.20, 95% CI=3.53–209.71, p=0.002), 60대(OR=14.34, 95% CI=2.89–71.21, p=0.003), 70세 이상(OR=20.98, 95% CI=2.76–159.23, p=0.002)은 50세 미만에 비해 혈당 미조절 위험이 유의하게 높았으며, 배우자와 동거하는 경우에 비해 배우자와 동 거하지 않는 경우에도 혈당 미조절 위험이 높았다(OR=6.31, 95% CI=1.29–30.88, p=0.023).

Objective: This study aimed to identify factors associated with poor glycemic control according to sex and residential area among adults aged 30 years and older with diabetes, in order to provide basic evidence for establishing tailored glycemic management strategies.
Method: Data were obtained from the 9th Korea National Health and Nutrition Examination Survey (KNHANES, 2022–2023). A total of 963 adults aged ≥30 years with diabetes were included and categorized into four groups according to sex and residential area (urban or rural). Glycemic control status was defined based on hemoglobin A1c (HbA1c) levels, with HbA1c ≥6.5% classified as poor glycemic control and <6.5% as controlled glycemia. Complex sample chi-square tests and logistic regression analyses were conducted to identify factors associated with poor glycemic control.
Results: Factors associated with poor glycemic control differed by sex and residential area. Among urban men, educational attainment and duration of diabetes were significantly associated with poor glycemic control. Compared with those with a college education or higher, men with elementary school education or less(OR=2.51, 95% CI=1.14–5.54, p=0.022) and middle school education (OR=3.13, 95% CI=1.32–7.46, p=0.010) had significantly higher odds of poor glycemic control. In addition, a diabetes duration of 5 years or longer was associated with a 2.81-fold higher risk of poor glycemic control compared with a duration of less than 5 years(OR=2.81, 95% CI=1.61–4.88, p<0.001). Among rural men, age, cohabitation with a spouse, educational attainment, alcohol consumption, and duration of diabetes were significantly associated with poor glycemic control. Men aged in their 50s(OR=27.20, 95% CI=3.53– 209.71, p=0.002), 60s(OR=14.34, 95% CI=2.89–71.21, p=0.003), and ≥70 years (OR=20.98, 95% CI=2.76–159.23, p=0.002) had significantly higher odds of poor glycemic control compared with those under 50 years of age. Living without a spouse was also associated with a higher risk of poor glycemic control (OR=6.31, 95% CI=1.29–30.88, p=0.023). Conversely, men with elementary school education or less showed a lower risk of poor glycemic control compared with those with a college education or higher (OR=0.22, 95% CI=0.06–0.79, p=0.021), and alcohol drinkers had a lower risk than non-drinkers(OR=0.39, 95% CI=0.18–0.86, p=0.019). A diabetes duration of 5 years or longer was strongly associated with an increased risk of poor glycemic control(OR=8.45, 95% CI=2.67–26.76, p<0.001).
Among urban women, only duration of diabetes was significantly associated with poor glycemic control. Women with a diabetes duration of 5 years or longer had a 2.10-fold higher risk of poor glycemic control compared with those with a duration of less than 5 years(OR=2.10, 95% CI=1.26–3.50, p=0.005). Among rural women, cohabitation with a spouse and duration of diabetes were significant factors. Women living without a spouse had a higher risk of poor glycemic control (OR=4.65, 95% CI=2.09–10.36, p<0.001), and those with a diabetes duration of 5 years or longer had a 2.79-fold increased risk (OR=2.79, 95% CI=1.15–6.78, p=0.023).
Conclusion: Factors associated with poor glycemic control varied by sex and residential area, indicating that glycemic control is influenced not only by individual clinical characteristics but also by social and environmental contexts. These findings highlight the need for tailored diabetes management strategies that take sex and region specific characteristics into account.
