This study aimed to examine the effects of age and chronotype (predisposing factors), job stress (precipitating factor), and sleep hygiene as well as dysfunctional beliefs and attitudes about sleep (perpetuating factors) on insomnia among shift-working nurses, based on Spielman’s 3P model. By exploring the relationships among these variables, this study sought to provide foundational evidence for the development of effective intervention strategies and evidence-based policies to improve sleep problems in shift-working nurses. Participants were 298 shift-working nurses with at least six months of clinical experience at a tertiary hospital located in G city. Data were collected from August 1 to August 8, 2025, using structured self-administered questionnaires. The data were analyzed using IBM SPSS Statistics version 29.0. Descriptive statistics (frequencies, percentages, means, and standard deviations) were calculated, and independent t-tests, chi-square tests, Fisher’s exact tests, and hierarchical logistic regression analysis were conducted.
The results of this study are as follows:
    1. A total of 298 participants were included; the majority were female (89.3%), and the mean age was 29.27 ± 4.05 years. Most participants were unmarried (72.8%) and had a bachelor’s degree or lower (85.9%). The mean length of clinical experience was 6.41 ± 5.33 years, and 41.3% worked in general wards, while 58.7% worked in intensive care units or emergency departments.
    2. The mean chronotype score among shift-working nurses was 43.92 ± 7.62. The majority of participants were classified as intermediate type (60.1%), followed by evening type (36.9%) and morning type (3.0%). The mean job stress score was 123.83 ± 27.30, the mean sleep hygiene score was 79.45 ± 16.60, and the mean score for dysfunctional beliefs and attitudes about sleep was 59.78 ± 14.62. The mean Insomnia Severity Index score was 11.66 ± 5.74. Of the participants, 208 (69.8%) were classified as the non-insomnia group with a mean score of 8.65 ± 3.62, whereas 90 (30.2%) were classified as the insomnia group with a mean score of 18.60 ± 3.13.
    3. Univariate analyses examining the associations between insomnia and participants’ general and work-related characteristics showed no statistically significant differences according to sex (χ² = 1.18, p = .315), age (t = 0.41, p = .682), age group (χ² = 0.31, p = .862), marital status (χ² = 2.40, p = .156), educational level (χ² = 0.06, p = .858), years of nursing experience (t = 0.86, p = .388), or work unit (χ² = 0.05, p = .898).
    4. Univariate analyses found statistically significant differences in chronotype (t = 2.00, p = .047), job stress (t = −2.56, p = .011), sleep hygiene (t = −6.18, p < .001), and dysfunctional beliefs and attitudes about sleep (t = −7.42, p < .001) according to insomnia.
    5. Hierarchical logistic regression analysis identified sleep hygiene (B = 0.04, OR = 1.04, 95% CI [1.02–1.06], p < .001) and dysfunctional beliefs and attitudes about sleep (B = 0.06, OR = 1.06, 95% CI [1.03–1.08], p < .001) as factors significantly associated with insomnia among shift-working nurses. The final model explained 28.7% of the variance (Nagelkerke R² = .287), and the model fit was acceptable (Hosmer–Lemeshow χ² = 4.25, p = .834).
In conclusion, this study demonstrated that sleep hygiene and dysfunctional beliefs and attitudes about sleep were significantly associated with insomnia among shift-working nurses. These findings suggest that, to alleviate insomnia and improve sleep patterns among shift-working nurses, it is essential to implement sleep interventions that not only promote proper sleep hygiene but also address maladaptive sleep-related cognitions and attitudes through cognitive behavioral therapy for insomnia (CBT-I)–based programs.

(Hosmer & Lemeshow χ² = 4.25, p = .834).
