This study examined the effects of an Extended Reality (XR) simulation–based cardiopulmonary resuscitation (CPR) education program on high school students’ CPR performance, cognitive load, and post-training CPR self-efficacy. The study also investigated whether these effects differed depending on students’ pre-training CPR self-efficacy levels (low vs. high). XR-based CPR education provides an immersive learning environment that integrates scenario-based practice, real-time visual and auditory feedback, and iterative performance correction. However, empirical evidence on its effects on both performance and psychological
outcomes remains limited. Accordingly, this study aimed to empirically verify the pedagogical effects of XR CPR training and explore interaction patterns between instructional method and learners’ initial self-efficacy. Participants were 52 male students (grades 10–12) who voluntarily agreed to participate from a boys’ high school in Y-city, Jeollanam-do, South Korea. After excluding two dropouts during the intervention, data from 50 students were included in the final analysis. Students were randomly assigned to either the XR simulation group or the traditional CPR training group. The study employed a 2×2 mixed experimental design with instructional method (traditional vs. XR) and pre-training CPR self-efficacy level (low vs. high) as between-group factors, and time (pre, mid-practice, and post) as a within-group factor for CPR performance. Measures included a 14-item CPR self-efficacy scale, a 16-item cognitive load scale with four sub-factors (task burden, mental effort, task difficulty, and self-evaluation), and CPR performance scores collected using Laerdal’s QCPR manikin system. Quantitative data were analyzed using repeated-measures ANOVA, MANOVA, and two-way ANOVA. Additionally, semi-structured interviews were conducted with 10 students from the XR group six weeks after training to obtain qualitative data. The results were as follows. First, a statistically significant interaction between time and instructional method was found for CPR performance (p<.001), indicating that the XR group showed continuous improvement over time, whereas the traditional group showed a declining trend.
Second, for cognitive load, neither the main effect of instructional method nor its interaction with prior self-efficacy was significant, while the main effect of prior self-efficacy was significant. Third, for post-CPR self-efficacy, although the main effect of instructional method was not significant, both the main effect of prior self-efficacy and its interaction with instructional method were significant (p=.050). Fourth, qualitative findings revealed that XR environments enhanced students’ psychological readiness and willingness to perform CPR in real-life situations by providing realistic contextual experiences and immediate performance feedback. In conclusion, XR simulation–based CPR education demonstrated a clear advantage in improving objective CPR performance. Meanwhile, cognitive and psychological learning outcomes were influenced more strongly by learners’ pre-training CPR self-efficacy than by instructional method alone. These results suggest that XR CPR education functions not only as a tool for strengthening CPR skills but also as a mechanism for aligning learners’ subjective confidence with their actual capabilities, thereby fostering “appropriate confidence commensurate with actual performance.” Furthermore, XR experiences showed potential to reduce the common fear of performing CPR in real emergencies. Therefore, XR CPR instruction should be designed with structured debriefing and tailored
feedback that consider learners’ initial self-efficacy levels to maximize educational effects. This study contributes practical and theoretical implications for the purposeful integration of XR into school-based CPR education.
