Accurate prognostic assessment is essential in small-animal cardiology, yet limited evidence exists regarding how structural remodeling and disease dynamics influence outcomes in clinical populations. This dissertation investigated key prognostic determinants in canine myxomatous mitral valve disease (MMVD) and feline transient myocardial thickening (TMT), with the aim of clarifying outcome-related structural changes and improving clinical risk stratification.
The first study evaluated the prognostic interaction between ACVIM stage and structural remodeling in 274 dogs with MMVD. Both the clinical stage and left atrial enlargement (LA/Ao ≥2.2) were identified as independent predictors of mortality. Importantly, dogs with preclinical stage B2 disease and severe LA remodeling demonstrated survival comparable to dogs with stage C disease with only mild-to-moderate remodeling, indicating that advanced structural disease can transcend clinical staging and should be incorporated into risk assessment.
The second study investigated whether preclinical administration of cardiac medications, including pimobendan, influenced survival after the onset of first-episode congestive heart failure (CHF) in 143 small-breed dogs. Although pretreatment was associated with larger chamber dimensions at CHF onset, it did not confer survival advantage once CHF developed. Instead, left atrial size at the time of decompensation remained the sole independent predictor of survival, emphasizing the prognostic primacy of structural remodeling rather than treatment history.
The third study established revised diagnostic criteria for TMT and characterized its prevalence and clinical behavior among 432 cats with hypertrophic cardiomyopathy (HCM) phenotypes. Using the newly defined criterion—an initial LV wall thickness ≥6 mm followed by a ≥25% reduction within 6 months—TMT accounted for 30% of cats that underwent serial echocardiography. TMT was identified predominantly in younger cats and was frequently associated with recent surgical, stressful, or inflammatory events. Affected cats showed marked regression in myocardial thickness and left atrial size with complete clinical recovery, whereas cats with true HCM exhibited persistent hypertrophy and progressive atrial enlargement. These findings refine the diagnostic framework for feline cardiomyopathy by incorporating a revised definition of TMT and emphasizing its distinction from primary HCM.
Collectively, these studies identify structural remodeling—particularly left atrial enlargement and dynamic changes in myocardial thickness—as central determinants of disease severity and outcome across species. By integrating canine MMVD progression, treatment history, and feline myocardial reversibility, this dissertation provides novel insights that enhance risk stratification, refine diagnostic interpretation, and support evidence-based management strategies for cardiac diseases in dogs and cats.
