Introduction: Effective postoperative pain management is essential for optimizing recovery after laparoscopic gynecologic surgery. Opioid-sparing multimodal analgesia is recommended, but evidence on a single intraoperative dose of a combined acetaminophen and ibuprofen regimen in short procedures is limited. The Quality of Recovery-15 (QoR-15) questionnaire provides a validated patient-reported outcome, with an 8-point change representing the minimal clinically important difference (MCID).
Methods: We conducted a prospective, randomized, controlled, assessor- and patient- blinded trial at two tertiary hospitals in Korea. Female patients undergoing elective laparoscopic gynecologic surgery were randomized to receive a single intraoperative dose of intravenous acetaminophen–ibuprofen (acetaminophen 1000 mg plus ibuprofen 300 mg, infused over 15 minutes) or placebo. The primary endpoint was the total QoR-15K score on postoperative day (POD) 1, analyzed using analysis of covariance adjusting for baseline score, study site, and anesthesia duration. Secondary endpoints included QoR-15K on POD 2, postoperative nausea and vomiting (PONV), pain scores, opioid consumption, and adverse events.
Results: Forty-four patients were analyzed (control 24, acetaminophen–ibuprofen 20). On POD 1, the acetaminophen–ibuprofen group had higher QoR-15K scores than controls (adjusted mean difference [AMD] 15.5; 95% CI 3.6–27.3; p = 0.013), exceeding the MCID. On POD 2, scores also favored the acetaminophen–ibuprofen group (AMD 8.1; 95% CI 3.1– 13.8; p = 0.008). Improvements were most evident in pain and physical independence domains. PONV scores were lower in the acetaminophen–ibuprofen group, although opioid use did not differ significantly. No treatment-related adverse events were observed.
Conclusions: In this small, two-center randomized controlled trial, a single intraoperative dose of intravenous acetaminophen–ibuprofen improved patient-reported recovery after laparoscopic gynecologic surgery, with effects exceeding the minimal clinically important difference. These findings support the role of non-opioid multimodal analgesia in enhancing recovery after short minimally invasive procedures.
