Hepatic encephalopathy (HE) is a neuropsychiatric disorder caused by impaired hepatic detoxification, leading to the accumulation of neurotoxic metabolites such as ammonia. Hyperammonemia plays a central role in the pathogenesis of HE by disrupting the structural integrity and functional homeostasis of the blood–brain barrier (BBB), which triggers neuroinflammatory cascades. These BBB disruptions and subsequent neuroinflammation result in synaptic dysfunction, neural circuit imbalance, and a spectrum of neurological abnormalities including cognitive deficits and altered consciousness. Therefore, strategies that protect the BBB and attenuate neuroinflammation have emerged as important therapeutic approaches for HE. The present study aimed to determine whether exendin-4, a glucagon-like peptide-1 (GLP-1) receptor agonist, could ameliorate BBB dysfunction and associated neuroinflammatory responses under hyperammonemic conditions. In vivo, a bile duct ligation (BDL) model was established in 13-week-old male C57BL/6J mice. Two weeks post-surgery, liver injury was confirmed by significant elevations in serum AST and ALT levels. In the cerebral cortex of BDL mice, expression of tight junction proteins was reduced, indicative of BBB disruption, and there was a concomitant increase in oxidative stress– and apoptosis-related proteins, suggesting molecular injury to BBB components. Based on these observations, an in vitro model mimicking hyperammonemia was developed. Ammonia exposure in bEnd.3 brain microvascular endothelial cells led to disruption of intercellular tight junctions, increased monolayer permeability, and downregulation of tight junction proteins. In C8-D1a astrocytes, ammonia induced a robust increase in proinflammatory cytokines (IP-10, M-CSF, KC), highlighting the contribution of astrocyte-mediated neuroinflammation to the pathogenesis of HE. Treatment with exendin-4 mitigated these pathological alterations in both cell models, preserving BBB structural integrity and suppressing inflammatory responses. Furthermore, RNA sequencing of bEnd.3 cells revealed that exendin-4 modulates gene expression across multiple biological pathways involved in oxidative stress regulation, inflammatory signaling, membrane stabilization, and barrier maintenance, indicating that its protective effects are mediated by multifaceted signaling mechanisms. In conclusion, this study demonstrates that exendin-4 attenuates hyperammonemia-induced BBB disruption and mitigates subsequent neurological injury. These findings suggest that GLP-1 receptor agonists may represent a promising therapeutic strategy to prevent or reverse neuropathological changes and functional deficits of the central nervous system in HE.
