This dissertation presents the development and performance evaluation of nuclear medicine imaging systems—a LYSO-GAPD–based DEXA detector and a DOI-capable breast dedicated PET scanner—to improve diagnostic accuracy for osteoporosis and breast cancer. Osteoporosis shows a high incidence in women, and its risk is further elevated following breast cancer treatments such as chemotherapy, aromatase inhibitors, and ovarian suppression, highlighting the necessity for long-term bone mineral density monitoring. Simultaneously, Breast cancer is the most frequently diagnosed malignancy among women worldwide, accounting for approximately 11.7% of all cancer cases. Early detection is essential for improving survival outcomes, however, accurate diagnosis remains challenging because many early-stage tumors measure less than 2 mm and are difficult to detect using conventional imaging modalities. Therefore, there is a growing clinical need for advanced diagnostic imaging systems that can simultaneously support early breast cancer detection and reliable bone mineral density assessment as part of integrated women's healthcare.
In Chapter 2, a LYSO-GAPD DEXA detector was designed and evaluated. The detector was constructed using 3 × 3 × 2 mm3 LYSO scintillation crystals coupled to GAPDs. Energy spectra measured with 241Am (60 keV) and 57Co (122 keV) exhibited linear energy response characteristics with energy resolutions of 23.8% and 17.2%, respectively. Dual-energy separation was successfully achieved using a 0.1-mm cerium (Ce) K-edge filter, and increasing filter thickness resulted in an exponential decrease in count rate. Phantom experiments further showed that aluminum step thicknesses from 4–12 mm were clearly distinguished, and pixel intensities increased linearly with aluminum thickness, demonstrating reliable material discrimination capability.
In Chapter 3, a dual-layer DOI-PET scanner inserting light guide between LYSO-layers was proposed to enhance both spatial resolution and sensitivity. The system was designed with an octagonal geometry consisting of 16 detector modules, each incorporating a two-layer LYSO scintillator array coupled with GAPD arrays. The 2nd layer employed fine-pitch crystals to achieve high spatial resolution, while the 1st layer used coarse-pitch crystals to improve sensitivity. A monolithic light guide was inserted to enable DOI information encoding. Experimental results demonstrated that ~97% of scintillation crystals were clearly identified in flood maps, and energy resolution was measured at ~23–25%. Line phantom studies confirmed that DOI information significantly reduced peripheral parallax error and improved sensitivity by approximately 44% at a 20 mm radial offset.
The LYSO–GAPD DEXA system developed in this work provides a practical solution for the diagnosis and monitoring of osteoporosis, while the DOI-capable breast PET scanner offers improved detection performance for early-stage breast lesions. Taken together, the two imaging technologies developed in this dissertation demonstrate the feasibility of an integrated diagnostic platform that supports both early breast cancer detection and long-term bone health management, with the potential to improve clinical outcomes and quality of life for women.

본 논문은 골다공증 및 유방암 진단 정확도 향상을 목적으로, LYSO–GAPD 기반 DEXA 검출기와 반응 깊이 판별이 가능한 유방 전용 PET스캐너를 개발하고 그 성능을 평가하였다. 골다공증은 여성에서 높은 발병률을 보이는 질환으로, 특히 유방암 항암 치료 이후 그 위험이 더욱 증가하여 장기적인 골밀도 모니터링의 중요성이 강조되고 있다. 한편, 유방암은 전 세계 여성에서 가장 빈번하게 진단되는 악성 종양으로, 전체 암 발생의 약 11.7%를 차지한다. 유방암의 조기 진단은 생존율 향상에 필수적이나, 초기 병변이 2 mm 미만의 작은 크기를 가지므로 기존 영상기기로 정확한 검출에 한계가 있다. 이러한 임상적 요구는 조기 유방암 검출과 신뢰성 있는 골밀도 평가를 동시에 지원할 수 있는 통합적 여성 건강 영상 기술의 필요성을 시사한다.
챕터 2에서는 LYSO-GAPD DEXA 검출기를 개발하고 성능을 평가하였다. 3 × 3 × 2 mm3 크기의 LYSO 섬광결정과 GAPD를 결합하여 LYSO-GAPD DEXA 검출기를 구성하였다. 241Am(60 keV)과 57Co(122 keV) 방사선원을 이용해 에너지 스펙트럼을 측정한 결과, 선형적인 에너지 위치를 보였으며 각각 23.8%(60 keV), 17.2%(122 keV)의 에너지 분해능을 나타냈다. 또한 X선 소스 모듈에 0.1 mm 두께의 Ce 필터를 적용하였을 때, 이중에너지 X선 분리가 확인되었고, 필터 두께 증가에 따라 계수율(CPS)이 지수적으로 감소하였다. 팬텀 실험을 통해 4–12 mm 알루미늄 단차가 구분되었으며, 픽셀값이 알루미늄 두께에 따라 증가하여 재료 구분 능력이 검증되었다.
챕터 3에서는 공간 분해능과 민감도를 동시에 향상시키기 위해 LYSO 결정층 사이에 광 가이드를 삽입한 DOI-PET 스캐너를 제안하였다. 본 시스템은 16개의 검출기 모듈로 구성된 팔각형 구조를 가지며, 각 모듈은 GAPD 배열에 결합된 두층의 LYSO 섬광결정 배열로 구성되었다. 2층은 좁은 픽셀간격을 적용하여 높은 공간 분해능을 확보하였고, 1층은 상대적으로 큰 픽셀간격을 사용하여 민감도를 향상시켰다. 두 섬광결정 층 사이에 광가이드를 삽입함으로써 반응 깊이 판별이 가능하도록 설계하였다. 실험 결과, 전체 섬광결정 중 약 97%가 평면영상에서 식별되었으며, 에너지 분해능은 약 23–25% 수준으로 평가되었다. 18F 선원 팬텀 실험에서는 DOI 정보를 적용함으로써 시야 주변부에서 발생하는 오차가 효과적으로 감소하였고, 중심에서 20 mm 떨어진 위치에서 민감도가 약 44% 향상됨을 확인하였다.
본 연구에서 개발된 LYSO–GAPD 기반 DEXA 시스템은 골다공증의 진단 및 장기 추적 관리를 위한 실용적인 대안을 제시하며, DOI 기능을 갖춘 유방 전용 PET 스캐너는 초기 유방암 병변의 검출 성능을 향상시킬 수 있음을 보여준다. 종합적으로, 본 논문에서 제안한 두 영상 기술은 조기 유방암 진단과 치료 후 골 건강 관리를 동시에 지원할 수 있는 통합 진단 플랫폼으로 확장 가능성을 제시하며, 여성 환자의 임상적 예후와 삶의 질 향상에 기여할 수 있는 기술적 기반을 제공할 수 있다.
