최근에는 자기공명영상(MRI)을 이용한 비침습적 흉관 시각화가 시도되고 있으며, 본 연구의 목적은 개를 대상으로 다양한 T2-weighted steady-state 시퀀스인 transverse T2-weighted turbo spin echo (T2W TSE), Half-Fourier acquisition single-shot turbo spin echo (HASTE), True fast imaging with steady-state precession (True FISP), sampling perfection with application-optimized contrasts using different flip angle evolutions (SPACE)를 이용하여 흉관의 시각화 정도와 주변 해부학적 구조의 시각화 가능성, 스캔 시간을 분석함으로써, 임상 상황에 따라 적절한 시퀀스를 선택할 수 있는 근거를 마련하고자 하였다.

The thoracic duct is the largest lymphatic vessel in the body, serving as the main lymphatic outflow pathway by collecting most lymph and chyle and transporting them into the venous system. Recently, non-invasive visualization of the thoracic duct using magnetic resonance imaging (MRI) has been attempted. The purpose of this study was to compare the degree of thoracic duct visualization using various T2-weighted steady-state MRI sequences, including T2-weighted turbo spin echo (T2W TSE), Half-Fourier acquisition single-shot turbo spin echo (HASTE), True fast imaging with steady-state precession (True FISP), and Sampling perfection with application-optimized contrasts using different flip angle evolutions (SPACE), in dogs. The study also analyzed the visualization of surrounding anatomical structures and scan times to provide a basis for selecting the appropriate sequence for different clinical situations. The study results showed that the HASTE sequence reliably visualized the thoracic duct as thick and clear. While the SPACE sequence showed the duct as somewhat thin and discontinuous, it allowed for the identification of surrounding structures. The T2W TSE sequence provided clear visualization of both the thoracic duct and its surrounding structures but had limitations, including potential confusion with the intercostal vessels and the inability to perform 3D reconstruction. The True FISP sequence's evaluation was limited due to the occurrence of artifacts, and the application of respiratory gating and dual coils did not show a clear effect on improving image quality. This research is expected to provide practical guidelines for the diagnosis and surgical planning of thoracic duct disorders, including rupture, obstruction, ectasia, and anatomical anomalies that may lead to chylothorax or rarely chylopericardium, by suggesting appropriate MRI sequences depending on the patient’s condition and diagnostic purpose.
