본 연구는 회복자본 모델을 이론적 기반으로 트라우마 경험이 있는 한국 성인을 대상으로 트라우마 요인과 문제성 음주행동 간의 구조적 관계 및 종단적 변화 양상을 규명하고자 하였다. 이를 위해 국내 성인을 대상으로 3차례에 걸쳐 수집한 종단자료를 활용하여 네트워크 분석과 잠재계층성장분석을 순차적으로 적용한 두 개의 연구를 수행하였다.
본 연구는 성인의 트라우마 경험이 문제적 음주행동에 미치는 영향력을 다차원적으로 규명하고자 수행되었다. 연구 1에서는 네트워크 분석을 통해 트라우마와 음주행동 간의 구조적 관계를 탐색하였으며, 연구 2에서는 잠재계층성장분석을 통해 문제적 음주행동의 발달 궤적을 식별하고 영향 요인을 검증하였다.
본 연구의 주요 결과를 요약하면 다음과 같다. 첫째, 네트워크 분석 결과, 침습적 반추와 트라우마 분노반응이 가장 높은 중심성 지표를 보이며 네트워크 내 핵심 노드로 확인되었다. 둘째, 하위 네트워크 분석을 통해 '경험/결과 군집'과 '정서·인지 반응 군집' 2개의 하위 네트워크가 식별되었다. 셋째, 문제성 음주행동에 직접적인 영향이 강한 트라우마 요인은 아동기 학대, 누적된 트라우마, 트라우마 분노반응이었다. 넷째, 음주행동의 발달 궤적은 '저위험 유지형', '경계선 문제행동 집단', '감소형', '만성 고위험형'의 4개 계층으로 분류되었다. 마지막으로, 성별, 연령, 아동기 학대, 트라우마 분노반응이 문제성 음주행동 변화에 유의한 영향요인으로 나타났다.
이러한 결과를 바탕으로 도출된 본 연구 1의 결론은 다음과 같다. 첫째, 침습적 반추와 트라우마 분노반응은 트라우마와 문제성 음주행동을 잇는 가장 중요한 핵심 연결고리이다. 특히 분노반응은 침습적 반추보다 문제적 음주행동에 더 직접적이고 핵심적인 기제로 기능한다. 둘째, 트라우마와 음주행동의 구조는 외상 사건 자체인 '경험의 축'과 그 이후의 '반응의 축'이라는 이원적 구조를 형성한다. 여기서 음주가 경험 군집에 포함된 것은 음주가 단순한 반응을 넘어 누적된 트라우마와 결합된 생활 양식임을 시사한다. 셋째, 아동기 학대, 트라우마 분노반응, 누적된 트라우마는 문제적 음주행동을 예측하는 가장 중요한 요인이다. 반면 반추 양식은 직접적인 예측보다 트라우마와 정서, 행동을 잇는 매개적 연결 고리로 이해될 필요가 있다.
본 연구 2의 결론은 다음과 같다. 첫째, 한국 성인의 문제적 음주행동은 발달적으로 이질적인 4가지 궤적으로 분화된다. 특히 전체의 과반을 차지하는 '경계선 문제행동 집단'은 만성화된 중위험 상태로 고착될 위험이 높으며, '감소형 집단'의 존재는 음주 문제가 변화 가능한 발달적 과정임을 입증한다. 둘째, 성별, 연령, 아동기 학대, 분노반응은 음주 궤적을 결정짓는 핵심 변인이다. 남성과 높은 연령은 위험 궤적에 속할 가능성을 높이며, 아동기 학대는 회복을 저해하는 부정적 자본으로 작용하는 한편, 분노반응은 만성 고위험군으로 진입시키는 결정적 정서 메커니즘으로 기능한다.
이러한 결론은 다음과 같은 시사점을 갖는다. 문제성 음주행동에 개입 시 단순한 증상 완화를 넘어, 트라우마 사건의 경험과 인지·정서적 반응 축을 동시에 표적화하는 통합적 접근이 필요하다. 특히 분노와 반추와 같은 교량 증상에 대한 직접적 개입은 트라우마로 인한 음주 문제의 악순환을 차단하는 데 핵심적인 역할을 할 것이다. 또한 고위험군뿐만 아니라 중위험 유지 집단에 대한 예방적 개입과 성별 및 생애주기별 특성을 고려한 차별화된 치료 전략 수립이 요구된다.

This study aims to elucidate the structural relationship and longitudinal patterns of change between trauma factors and problematic drinking behavior in Korean adults with trauma experiences, based on the recovery capital model as a theoretical foundation. To this end, two sequential studies were conducted using longitudinal data collected in three waves from adults in South Korea, applying network analysis and latent class growth analysis.
This study was conducted to multidimensionally examine the influence of adult trauma experiences on problematic drinking behavior. Study 1 explored the structural relationship between trauma and drinking behavior through network analysis, while Study 2 identified the developmental trajectories of problematic drinking behavior and verified influencing factors through latent class growth analysis.
The main findings of this study are summarized as follows. First, network analysis revealed that intrusive rumination and trauma-related anger reactions showed the highest centrality indices, confirming them as core nodes within the network. Second, subnetwork analysis identified two subnetworks: 'Experience/Outcome Cluster' and 'Emotional/Cognitive Response Cluster'. Third, the trauma factors with the strongest direct influence on problematic drinking behavior were childhood abuse, cumulative trauma, and trauma-related anger reactions. Fourth, the developmental trajectories of drinking behavior were classified into four latent classes: 'Low-Risk Maintenance Group', 'Borderline Problem Behavior Group', 'Decreasing Group', and 'Chronic High-Risk Group'. Finally, gender, age, childhood abuse, and trauma-related anger reactions emerged as significant factors influencing the probability of belonging to each trajectory.
The conclusions drawn from these findings for Study 1 are as follows. First, intrusive rumination and trauma-related anger reactions are the most crucial core links connecting trauma and drinking behavior. In particular, anger reactions function as a more direct and central mechanism for problematic drinking behavior than intrusive rumination. Second, the structure of trauma and drinking behavior forms a dual structure consisting of the 'axis of experience', which is the traumatic event itself, and the 'axis of response' that follows. The inclusion of drinking within the experience cluster suggests that drinking is not merely a reaction but a lifestyle intertwined with cumulative trauma. Third, childhood abuse, trauma-related anger reactions, and cumulative trauma are the most significant predictors of problematic drinking behavior. In contrast, the ruminative style should be understood not as a direct predictor but as a mediating link connecting trauma, emotion, and behavior.
The conclusions of Study 2 are as follows. First, problematic drinking behavior among Korean adults diverges developmentally into four distinct trajectories. Notably, the 'Borderline Problem Behavior Group,' which accounts for over half of the total sample, is at high risk of solidifying into a chronic, moderate-risk state, while the existence of the 'Decreasing Group' demonstrates that drinking problems are a developmental process capable of change. Second, gender, age, childhood abuse, and anger response are key variables determining drinking trajectories. Being male and of higher age increases the likelihood of belonging to a high-risk trajectory, while childhood abuse functions as negative capital that impedes recovery, and anger response serves as a decisive emotional mechanism driving entry into a chronic high-risk group.
These conclusions carry the following implications. When intervening in problematic drinking behavior, an integrated approach is needed that goes beyond simple symptom alleviation to simultaneously target both the experience of traumatic events and the cognitive-emotional response axis. In particular, direct intervention on bridge symptoms such as anger and rumination will play a crucial role in breaking the vicious cycle of trauma-induced drinking problems. Furthermore, preventive interventions are required not only for high-risk groups but also for those maintaining a moderate-risk state, along with the establishment of differentiated treatment strategies that consider gender and life-cycle specific characteristics.
