Sodium-glucose cotransporter 2 (SGLT2) inhibitors have been reported to provide various metabolic benefits beyond glucose lowering, including increased uric acid excretion, suggesting a possible association with reduced risk of gout. However, evidence on the association between SGLT2 inhibitor use and gout risk among Korean patients remains limited. This study aimed to evaluate whether SGLT2 inhibitor use, compared with DPP-4 inhibitor use, is associated with lower gout incidence in patients with type 2 diabetes using real-world data from the Korean Health Insurance Review and Assessment Service (HIRA). We additionally assessed subgroup differences by sex and age, as well as the robustness of findings across alternative analytic approaches. A retrospective cohort study was conducted using a nationwide health insurance claims database (HIRA) in South Korea from 2017 to 2021. Patients with type 2 diabetes who newly initiated SGLT2 or DPP-4 inhibitors were identified, and major exclusion criteria were applied to construct the final cohort. The primary outcome was incident gout after the index date. Confounding was addressed using propensity score-based trimming and inverse probability of treatment weighting (IPTW), followed by Cox proportional hazards models to estimate hazard ratios (HRs). Subgroup analyses by sex and age and several sensitivity analyses were performed. A total of 125,704 patients were included, with 15,865 in the SGLT2 inhibitor group and 109,839 in the DPP-4 inhibitor group. In the primary IPTW analysis, SGLT2 inhibitor users showed a significantly lower risk of gout compared with DPP-4 inhibitor users (HR 0.92, 95% CI 0.86-0.99). This pattern was broadly consistent across additional adjustments and sensitivity analyses. No significant difference was observed for gout requiring hospitalization, while women and patients younger than 65 years exhibited more pronounced reductions in risk. This large-scale retrospective cohort study is the first to evaluate the association between SGLT2 inhibitor use and gout risk among Korean patients with type 2 diabetes. SGLT2 inhibitor use was consistently associated with a lower risk of gout compared with DPP-4 inhibitor use. These findings suggest potential metabolic benefits of SGLT2 inhibitors beyond glycemic control and provide real-world evidence that may support treatment decisions in patients with type 2 diabetes who are at increased risk for gout.
