중재는 P-CAB(보노프라잔, 테고프라잔, 펙수프라잔), 비교는 PPI(란소프라졸, 에소메프라졸, 라베프라졸, 덱스란소프라졸)였으며, 주요 평가변수는 8주 시점 내시경상 미란성 식도염 치유율이었다.

Gastroesophageal reflux disease (GERD) is a chronic disorder in which gastric contents reflux into the esophagus, causing heartburn, chest pain, and related symptoms. Because of its high prevalence and tendency to recur, GERD is associated with substantial healthcare resource use and impaired quality of life. Proton pump inhibitors (PPIs), the standard pharmacologic treatment, have several limitations, including a short half-life, the need for administration before meals, and variability in drug response related to CYP2C19 genetic polymorphisms. Potassium-competitive acid blockers (P-CABs) provide faster and more potent acid suppression; however, their superiority over PPIs has not been consistently demonstrated in individual randomized controlled trials. Therefore, this study aimed to comprehensively evaluate the comparative efficacy and safety of P-CABs versus PPIs in patients with endoscopically confirmed erosive GERD.
A systematic review and meta-analysis of randomized controlled trials (RCTs) that directly compared P-CABs with PPIs was conducted.. RCTs enrolling adult patients with erosive GERD were identified through searches of Cochrane CENTRAL, MEDLINE, EMBASE, and ClinicalTrials.gov using predefined strategies. Interventions were P-CABs (vonoprazan, tegoprazan, and fexuprazan), and comparators were PPIs (lansoprazole, esomeprazole, rabeprazole, and dexlansoprazole). The primary outcome was endoscopic healing of erosive esophagitis at week 8. Secondary outcomes included healing rates at weeks 2 and 4, and the incidence of adverse events, treatment-related adverse events, and serious adverse events. Risk of bias was assessed using standardized tools focusing on randomization, allocation concealment, blinding, and completeness and appropriateness of outcome reporting.
A total of 10 RCTs including 3,316 patients were eligible for the meta-analysis; most were double-blind, parallel-group clinical trials. In pooled analyses, the relative risk for endoscopic healing at week 8 was close to 1.0, indicating similar final healing rates between P-CAB and PPI groups. In contrast, healing rates at weeks 2 and 4 were significantly higher in the P-CAB group than in the PPI group. The advantage of P-CABs was particularly evident in studies using vonoprazan and in the subgroup of patients with severe erosive esophagitis (Los Angeles classification C/D). These findings suggest that P-CABs may facilitate more rapid endoscopic healing through faster and stronger acid suppression.
With respect to safety, there were no clinically meaningful differences between P-CABs and PPIs in the incidence of overall adverse events, treatment-related adverse events, or serious adverse events, and the overall safety profiles were comparable. However, some outcomes showed heterogeneity across trials, and quantitative synthesis of symptom-related endpoints was limited because of variation in assessment tools and reporting formats. Evidence remains insufficient regarding long-term treatment and maintenance therapy, as well as differential effects in patients with incomplete response to PPIs or specific genetic backgrounds. In conclusion, P-CABs appear to be comparable to PPIs in terms of 8-week healing rates and short-term safety, while showing a tendency toward superiority in early healing. P-CABs may therefore be considered a preferred treatment option in patients for whom early healing and rapid symptom relief are particularly important. Further studies are needed to clarify their long-term safety, real-world effectiveness, and benefits in specific patient subgroups.
