주 분석은 성향점수 기반 IPTW(Inverse Probability of Treatment Weighting)를 적용해 수행하였다.

Background: Hypertension is the most common cardiovascular disease (CVD) risk factor in Korea, and its prevalence rises sharply with advancing age. Despite improvements in blood pressure control, cardiovascular mortality has not declined accordingly, partly due to the increasing burden of hypercholesterolemia. This trend highlights the growing importance of effective LDL cholesterol management. Although major clinical trials conducted mainly in Western populations have demonstrated that statins reduce cardiovascular events in patients with controlled hypertension, long-term evidence specific to Korean patients is limited. To address this gap, the present study compared the 5-year preventive effectiveness and safety of moderate-intensity atorvastatin (20 mg) and rosuvastatin (5–10 mg) in Korean adults with controlled hypertension and no prior CVD.
Methods: A retrospective cohort study was conducted using claims data from the Korean National Health Insurance Service (NHIS). The study population included adults aged 60–69 years with controlled hypertension at baseline who initiated moderate-intensity statin therapy (atorvastatin 20 mg or rosuvastatin 5–10 mg) during the enrollment period. Propensity score–based inverse probability of treatment weighting (IPTW) was used to balance baseline characteristics between treatment groups. The primary outcome was the incidence of three-point major adverse cardiovascular events (3P-MACE). Secondary outcomes included each component of 3P-MACE as well as key safety outcomes.
Results: Among 3,461 patients included in the study, 876 received atorvastatin and 2,585 received rosuvastatin. The incidence of major cardiovascular events showed a 34% lower risk in the atorvastatin group compared with the rosuvastatin group, although this difference was not statistically significant (p=0.11). Among individual cardiovascular outcomes, the risk of nonfatal stroke was significantly lower with rosuvastatin, showing a 47% risk reduction (p=0.04). Muscle-related adverse events were also 26% lower in the rosuvastatin group. No statistically significant differences were observed between the two groups for other cardiovascular or safety outcomes.
Conclusion: This study is the first large-scale retrospective cohort analysis to directly compare the effectiveness and safety of moderate-intensity atorvastatin and rosuvastatin in Korean patients with controlled hypertension using real-world data. The results offer clinically meaningful insights, particularly for outcomes showing clearer between-drug differences—such as nonfatal stroke and muscle-related adverse events—and may support more informed statin selection in routine clinical practice.
