Background
In the drug development process, clinical trials consume the most time and resources, and their success directly impacts the efficiency of the overall development process. Crucially, the completion of a clinical trial according to plan is considered a vital indicator directly related to the feasibility of drug development. Seamless Phase II/III clinical trials offer the advantage of shortening the transition time between phases and reducing the total development period by designing Phase II and Phase III consecutively. However, existing research has primarily focused on the effect of seamless design on reducing the development timeline, while empirical analysis of the clinical trial's 'completion rate' itself remains relatively scarce.
Objective
Unlike previous studies that concentrated on the impact of seamless design on shortening the development period, this study aims to empirically analyze the completion rate to elucidate the influence of the clinical trial design method on the likelihood of completion.
Results
This study extracted 10,459 records from ClinicalTrials.gov spanning from 2009 to 2019. The completion rates for Seamless design clinical trials and Non-seamless design (Phase II, Phase III) clinical trials were measured, compared, and analyzed. To mitigate the sample size imbalance and selection bias between the two groups, the Propensity Score Matching (PSM) technique was utilized. Factors previously known to affect the completion rate—specifically, therapeutic area (oncology vs. non-oncology), number of subjects, subject age, and funder type—were used as covariates.
Conclusion
It was confirmed that there is no statistically significant difference in the likelihood of 'completion' between the seamless and non-seamless groups. Although the findings did not reveal a statistically significant difference, this conclusion itself carries practical significance, providing important evidence and realistic expectations for research institutions considering the adoption of seamless designs.
