본 연구는 2024년 9월 FDA에서 발행한 분산형 임상시험에 대한 가이드라인, 즉, ‘Conducting clinical trials with decentralized elements - Guidance for Industry, Investigators, and Other Interested Parties ‘의 발행 전후로 분산형 임상시험 요소의 활용을 변화를 분석하는 것을 목표로 한다.
연구는 1) efficacy, safety 부분, 2) 환자 방문 단계(consent, screening, routine visit, follow-up), 3) data management(data capture(CRF), Monitoring) 부분으로 나누어 분석을 진행하였다.

The concept of decentralized clinical trials (DCT) has been discussed since the early 2000s, and global interest in this approach has surged following the COVID-19 pandemic, leading to more active research efforts. This study aims to analyze changes in the utilization of DCT elements before and after the publication of the FDA Guidance for Industry, Investigators, and Other Interested Parties: Conducting Clinical Trials with Decentralized Elements in September 2024.
Using the FDA guidance issuance date as a base point, DCT element utilization was compared between two periods, from 01Sep2023 to 31Aug2024, and from 01Sep2024 to 31Aug2025. The analysis was conducted across three sections as follows - 1) efficacy and safety, 2) patient visit stages (consent, screening, routine visit, and follow-up), and 3) data management (data capture (CRF) and monitoring).
A significantly statistical increase in the use of DCT elements was observed only in the safety part, while no significant differences were detected in efficacy, screening, routine visits, follow-up procedures, and data management (including data capture and monitoring). No DCT elements were identified in the informed consent process in both periods. Comparison with the study which was conducted earlier, 2019–2020 Phase 2–4 protocols suggest that, in spite of the limitation of overall DCT utilization, we could find modest progress was found in specific parts such as safety.
Nonetheless, there was still less utilization in the other parts such as recruitment and investigational product management, which was caused by regulatory barriers, delayed system development, and financial difficulties.
Despite the value of DCTs, Adopting DCT is still conservative and limited in few components such as remote data acquisition, remote/tele visits, and e-consent. Future expansion of DCT implementation will be accompanied by regulatory, operational systems, and greater transparency in clinical research information. Expansion of DCT utilization can require more time than we expected, these approaches may provide benefits in efficiency and convenience not only in clinical trial industry but also to patients.
