This study empirically analyzed healthcare utilization between self-reliant and dependent beneficiaries within South Korea's National Basic Livelihood Security System, focusing on heterogeneity among recipients under the same public assistance program. Using data from the 2017–2024 waves of the Korea Welfare Panel Study, self-reliant beneficiaries were defined based on repeated employment continuity in accordance with the Enforcement Decree of the Employment Insurance Act and prior research standards. Outpatient visits, inpatient admissions, and medical expenditures were used as dependent variables and analyzed using mixed-effects models, with additional propensity score matching (PSM) conducted to address potential selection bias. The results show that dependent beneficiaries exhibit consistently higher levels of healthcare utilization than their self-reliant counterparts, and these differences persist even among individuals with the same level of subjective health status. These findings suggest that self-reliance status plays a meaningful role in promoting health equity through a more equitable distribution of medical resources, underscoring the need for tailored support policies that reflect beneficiaries’ employment characteristics and health conditions.
