Sensitive skin (SS) is associated with poor skin barrier function, increased sensory reactivity, and neuroimmune dysregulation. Reduced ceramides, cholesterol, and fatty acid levels cause increased transepidermal water loss and dehydration, while increased sensory responses contribute to burning, stinging, and itching. Despite its high global prevalence, standardized diagnostic criteria for SS remain lacking.
To improve diagnostic reliability, we assessed SS in 975 women from Beijing and Shanghai using a newly developed questionnaire combined with physiological measurements, patch tests, and the lactic acid sting test (LAST). Among them, 307 participants underwent detailed biophysical and biochemical analyses, including NMF and lipid profiling. The combination of the questionnaire and LAST enhanced detection accuracy and reduced false negatives. SS individuals showed characteristic differences such as altered hydration and pH, reduced ceramides and fatty acids, and lower amino acid levels in the stratum corneum, supporting the value of integrated assessment approach. A genome-wide association study (GWAS) in 1,690 Korean women further explored SS susceptibility. Although no SNPs reached genome-wide significance, several variants including rs11689992, rs7614738 (USP4/GPX1), RASSF8, and NRXN3 showed suggestive associations, implicating oxidative stress and neuro-immune pathways in SS vulnerability. To enhance objectivity in clinical evaluation, an AI-based irritation assessment model using YOLOv5x was developed. Achieving an accuracy of 0.983 and F1 score of 0.982, the model demonstrated strong potential for standardized patch test scoring.
Overall, integrated clinical, biochemical, genetic, and AI-driven approaches provide a more comprehensive understanding of SS and support the development of objective diagnostic tools and personalized management strategies.
