Inherited retinal dystrophies (IRDs) exhibit significant genetic heterogeneity across ethnic populations, necessitating population-specific precision gene therapies. Leber congenital amaurosis (LCA), a severe early-onset IRD, accounts for approximately 20% of childhood blindness among children with visual impairment. While voretigene neparvovec (LUXTURNA) remains the only approved gene therapy for LCA, its target—RPE65 mutations—is rare in East Asian populations. Instead, the NMNAT1 c.709C>T (p.Arg237Cys) variant represents a major pathogenic hotspot mutation in East Asia, constituting a substantial unmet clinical need. Here we report a permanent, one-shot therapeutic strategy for NMNAT1-associated LCA using an optimized prime editing system. By performing high-throughput screening of a 19,195-member pegRNA library, we identified lead candidates enabling highly efficient correction of the pathogenic variant. Subsequent validation and optimization of pegRNA and prime editor protein further enhanced prime editing performance, achieving up to 80% genome correction in human cell lines. Furthermore, we established a dual-AAV intein-split delivery system to enable efficient in vivo reconstitution of the optimized prime editor for downstream translational models. Collectively, this study establishes a generalizable framework for population-tailored genome editing therapies, providing a clinically translatable path for treating inherited disorders.
