Herpes zoster (HZ) vaccination is increasingly recognized as a public health priority in aging populations, alongside steadily rising vaccination coverage. This attention is driven by concerns regarding the burden of post-herpetic neuralgia (PHN) and ongoing discussions on the inclusion of HZ vaccines in national immunization programs. Alongside expanding use, questions persist regarding potential cardiovascular (CV) safety in elderly populations. To explore whether HZ vaccination is associated with CV safety signals, we evaluated spontaneous adverse event reports among individuals aged 65 years and older using data from the WHO VigiBase, which is the largest global repository for pharmacovigilance data.
A retrospective disproportionality analysis was performed on VigiBase reports submitted between 1968 and 2024. Adults aged 65 years and above who were administered live attenuated or recombinant HZ vaccines comprised the study cohort. Comparator drugs were defined as all other vaccines. CV outcomes were identified using Standardized MedDRA Queries (SMQs), and Disproportionality analyses were performed using adjusted reporting odds ratios (aRORs) and the lower bound of the information component (IC025).
The main analysis did not reveal any disproportionality signals for CV outcomes (aROR: 0.50(95% CI: 0.46–0.55), IC025: –1.00). Sensitivity analyses using alternative comparators yielded results consistent with the main analysis, supporting the stability of the findings. Indirect comparisons between the live attenuated and recombinant vaccines showed no meaningful differences in CV safety profiles of HZ vaccines.
This study is the first large-scale global pharmacovigilance evaluation specifically examining CV outcomes following HZ vaccination in the elderly population. The absence of disproportionality signals in this study suggests no apparent CV safety concerns related to HZ vaccines and provides supportive evidence that may be referenced in ongoing immunization policy discussions, including considerations for aging-population vaccination strategies. Nevertheless, given limitations of spontaneous reporting systems, further real-world studies and mechanistic research are needed to clarify causality and strengthen evidence-based vaccination recommendations.
