Background: Romosozumab is a monoclonal antibody targeting sclerostin, approved for the treatment of osteoporosis in patients at high risk of fracture. Although romosozumab carries an FDA boxed warning for potential increased risk of myocardial infarction and stroke based on findings from pivotal trials, subsequent evidence from randomized trials and real-world studies has been inconsistent, leaving its cardiovascular safety profile unresolved. Accordingly, we aimed to investigate potential cardiovascular safety signals related to romosozumab using data from a global pharmacovigilance database in this study.

Method: We conducted an disproportionality analysis using the World Health Organization(WHO) global Individual Case Safety Report (ICSR) database, Vigibase, up to . ICSRs reporting romosozumab were extracted and compared with those of other osteoporosis therapies. Cardiovascular-related events were identified using Standardized MedDRA Queries (SMQs) including Cardiac failure, Ischaemic heart disease, Cardiac arrhythmias, and Embolic and thrombotic events, etc. The analysis were performed by calculating the Reporting Odds Ratio (ROR) with 95% confidence intervals (CI) and the information component (IC).

Results: A total of 29,045 ICSRs involving romosozumab were included, of which 89.6% were female and 51.5% were older than 65 years. Most reports originated from North America and Europe. Cardiovascular AEs most frequently involved ischaemic events, followed by arrhythmias and hear failure-related symptoms. In disproportionality analyses, romosozumab demonstrated elevated signals for several cardiovascular events compared with all other drugs, and showed even stronger signals when compared with denosumab. Notably, signals were identified for: Ischaemic heart disease (ROR: 3.23(2.86 – 3.65), Embolic and thrombotic events (ROR: 3.20(2.97 – 3.45). Sensitivity analyses restricted to major drug classes and SMQs further strengthened the signal for these 2 cardiovascular events.

Conclusions: This study identified a reporting of major cardiovascular events, including Ischaemic heart disease and Embolic and thrombotic events, among romosozumab users. These findings complement existing concerns raised in pivotal trials and highlight the need for continued, systematic surveillance of cardiovascular risk in patients receiving romosozumab, especially those with underlying cardiovascular risk.
