This study aimed to evaluate the safety profile of ocrelizumab with a specific focus on depression by conducting a disproportionality analysis using Individual Case Safety Reports (ICSRs) recorded in the WHO VigiBase. Ocrelizumab is a high-efficacy anti-CD20 monoclonal antibody widely used for the treatment of multiple sclerosis (MS), yet concerns about potential neuropsychiatric adverse events, particularly depressive symptoms, remain insufficiently understood in international post-marketing surveillance. Given the inherently elevated baseline risk of depression among MS patients and the limited comparative evidence across different disease-modifying therapies (DMTs), evaluating reporting disproportionality across agents is essential to inform clinical and regulatory decision-making.
The study was conducted using data submitted between September 2022 and August 2025. Reports related to ocrelizumab were compared with those for three major MS therapies—interferon beta-1a, fingolimod, and cladribine. Depression was identified at the Preferred Term (PT) level (Depression, v28.1, 10012378). Disproportionality analysis, including Reporting Odds Ratio (ROR) and Bayesian Confidence Propagation Neural Network (BCPNN)-based Information Component (IC), were calculated using SAS 9.4, with signals defined as ROR lower 95% CI > 1 or IC025 > 0. Subgroup analyses were performed by sex, age group, and WHO region to explore the consistency of reporting patterns across demographics and geographic distribution.
A total of 422,201 ICSRs were included, of which 174,448 were associated with ocrelizumab. In the main analysis, ocrelizumab showed no elevated reporting signal for depression compared with the combined comparator group (ROR 0.817, 95% CI: 0.745–0.895; IC025 = –0.178). Subgroup results demonstrated heterogeneous patterns: males showed a mildly increased ROR, whereas females consistently exhibited a lower likelihood of depression reporting with ocrelizumab. Age-stratified analyses indicated relatively lower reporting among younger patients, and regional analyses reflected the known epidemiological distribution of MS, with very limited reporting from Asian countries likely due to the low prevalence of MS and the recent approval of ocrelizumab in some regions.
These findings suggest that, in global pharmacovigilance data, ocrelizumab does not demonstrate a disproportionate increase in depression reporting compared with other MS therapies. Nevertheless, sex-specific variations, age-related differences, and regional disparities underscore the need for continued monitoring and context-sensitive interpretation. The study provides internationally relevant evidence that may support physicians, regulators, and researchers in understanding neuropsychiatric safety considerations associated with high-efficacy MS treatments.
