Metabolic dysfunction-associated steatotic liver disease (MASLD) frequently coexists with hypertension. However, evidence regarding antihypertensive medication effects on hepatic complications remains limited. Therefore, this study aimed to investigate the association between antihypertensive drug use and major adverse liver outcomes (MALO) in patients with hypertension and MASLD using the All of Us Research Program.
A case-control study was conducted, with MALO cases and 1:2-matched controls based on age, sex, race/ethnicity, and diagnosis year. MALO was defined using ICD-9/10 codes. Antihypertensive drugs included ACE inhibitors, ARBs, beta-blockers, calcium channel blockers, and thiazide diuretics. Medication exposure was identified using RxNorm during the 12-month prior to the index date. Multivariable conditional logistic regression was performed with adjustment for age, sex, and metabolic factors. Stratified analyses examined sex, age, diabetes, and dyslipidemia status. A total of 342 MALO cases and 623 controls were included in the analysis. ACE inhibitors and ARBs demonstrated significant protective associations with MALO (adjusted OR = 0.70, 95% CI: 0.53–0.91, p = 0.008), representing 30% risk reduction. Other drugs showed no significant associations. Sex-stratified analysis revealed stronger protection in males (OR = 0.58, 95% CI: 0.39–0.87, p = 0.009) versus females (OR = 0.81, 95% CI: 0.56–1.16, p = 0.242). Among the patients with diabetes, the risk reduction reached 40% (OR = 0.60, 95% CI: 0.42–0.87, p = 0.007).
In conclusion, ACE inhibitors and ARBs confer hepatoprotective benefits in hypertensive MASLD patients, particularly males with concurrent diabetes. These findings support considering hepatic effects alongside blood pressure control in antihypertensive selection for MASLD patients
