Metabolic dysfunction–associated steatotic liver disease (MASLD) has rapidly emerged as a major hepatic condition among individuals with type 2 diabetes. Despite its growing clinical relevance, the impact of commonly prescribed antidiabetic medications on the development of MASLD remains insufficiently understood. Many prior studies relied on NAFLD coding systems predating the recent redefinition of MASLD or were conducted within narrowly represented national cohorts, thereby limiting the generalizability of their findings. Therefore, this study aimed to evaluate the associations between antidiabetic medication classes—including GLP-1 receptor agonists, SGLT2 inhibitors, DPP-4 inhibitors, sulfonylureas, thiazolidinediones, and metformin—and newly diagnosed MASLD.
Using electronic health records from the All of Us Research Program, we constructed a nested case–control sample of adults with type 2 diabetes. MASLD cases were identified using ICD-9 code 571.8 and ICD-10 code K76.0. Each case was matched with two controls based on demographic characteristics and year of diabetes diagnosis. Conditional logistic regression was used to estimate crude and adjusted odds ratios.
A total of 1,716 MASLD cases and 3,301 matched controls were included. No significant associations were observed across any medication class, with most effect estimates near unity. Thiazolidinediones showed a trend toward reduced risk, though the association did not reach statistical significance and the confidence intervals were wide. Stratified analyses by demographic and clinical factors did not indicate meaningful heterogeneity.
These findings suggest that, although certain antidiabetic medications may confer therapeutic benefits among patients with existing MASLD, their ability to prevent new-onset disease among individuals with type 2 diabetes appears limited. Future research incorporating medication adherence, dosage intensity, treatment patterns, and longitudinal imaging-based assessments is warranted to more precisely evaluate the potential preventive effects of antidiabetic therapy.

All of Us Research Program의 전자의무기록을 활용한 환자-대조군 연구로, 제2형 당뇨병 환자 중 MASLD 신규 진단자를 환자군으로 설정하고 연령, 성별, 당뇨병 경과 기간, 지표 연도를 기준으로 1:2 매칭된 대조군과 비교하였다.
