Background:Burn injuries remain a significant source of morbidity globally, exhibitingconsiderable variation by age group, sex, and geographic region. The presentstudy applies the Global Burden of Disease (GBD) 2021 framework to assessburn burden stratified by total body surface area (TBSA), offering distinctestimates for minor (<20%) and major (≥20%) burns. Enhanced understandingof recent epidemiologic patterns and future trends is essential for informingprevention strategies and reinforcing trauma and rehabilitation systems. Methods:Standardized GBD 2021 methods were used to estimate prevalence,age-standardized prevalence rates (ASR), and the number of individuals livingwith burn-related morbidity from 1990 to 2021 across 204 countries andterritories. Minor and major burns were analyzed as separate strata according toTBSA thresholds. All estimates are presented with 95% uncertainty intervals.Future prevalence for 2030 and 2050 was projected using a logit-based model- 48 -incorporating the Sociodemographic Index (SDI). Changes in absolute burdenbetween 2021 and 2050 were decomposed using the Das Gupta method toidentify contributions from population growth, aging, and non-demographicfactors.Results:In 2021, the global burden of minor burns substantially exceeded that of majorburns in both absolute numbers and ASR. Although ASR for both categoriesstagnated or declined modestly in many regions, the absolute number of affectedindividuals continued to rise, primarily due to population growth and aging. Ageand sex patterns indicated that minor burns were distributed across all agegroups, while infants, young children, and older adults experienced adisproportionate share of major burn morbidity. SEV analyses demonstrated aconsistent relationship between exposure levels and minor burn prevalence;however, the pattern for major burns varied significantly depending on regionalhealthcare capacity, safety regulations, and reporting infrastructure. Long-termprojections indicate continued growth in absolute burden through 2050, largelydriven by demographic changes. Das Gupta decomposition confirmed thatpopulation growth and aging are the primary drivers of this projected increase, with non-demographic factors contributing notable regional variation. Conclusions:Despite prevention efforts and improved care resulting in stable or decliningASR, the total number of individuals living with burn-related morbidity isprojected to increase. Stratification by TBSA highlights the necessity for a dualapproach: enhancing primary prevention for minor burns in homes, schools, andworkplaces, and expanding trauma, surgical, and rehabilitation capacity for major- 49 -burns, particularly in rapidly aging societies. The combined use of SEV analysis,long-term projections, and burden decomposition provides a policy-relevantframework that identifies key leverage points, including demographic dynamics, environmental and behavioral risks, and health system factors, to inform tailorednational and regional strategies for burn prevention and care

(Das Gupta)
