한국형 정원처방 제도화 방안 현대 사회는 급격한 인구 고령화와 1인 가구의 증가, 만성질환의 만연, 그리고 팬데믹 이후 심화된 사회적 고립과 우울증 등 복합적인 보건·사회적 위기에 직 면해 있다. 이러한 다층적 문제들은 기존의 약물 중심 생의학적(biomedical) 치 료 모델만으로는 해결하기 어려운 한계에 봉착해 있으며, 이에 따라 급증하는 사 회·경제적 비용 부담과 건강 불평등 문제를 완화하기 위한 새로운 대안이 절실 히 요구되고 있다. 본 연구는 그 해결책으로 자연의 치유력을 활용하는 '자연기반 해법(Nature-based Solutions, NbS)'의 일환인 '정원치유(Garden Therapy)'의 잠재력에 주목하였다. 국내에서는 2020년 「수목원·정원의 조성 및 진흥에 관한 법률」 개정을 통 해 정원치유의 법적 정의가 도입되었으나, 후속 사항에 대한 규정이 없는 선언적 수준에 그쳐 구체적인 실행 기준이나 전달체계가 미흡한 실정이다. 정원치유의 임상적 효과는 다수의 선행연구를 통해 입증되었으나, 우수한 치유 자원을 이를 필요로 하는 수요자에게 체계적으로 연결할 '공적 전달체계'의 부재로 인해 개별 적인 체험 프로그램 수준에 머물러 있다. 이에 본 연구는 영국 등 선진국에서 이 미 검증된 '사회적 처방(Social Prescribing)' 시스템을 핵심 전달체계로 도입하 여, 우수한 치유 자원인 '정원치유'와 효과적인 전달 시스템인 '사회적 처방'을 결 합한 '정원처방(Gardening Prescription)' 모델을 구체적인 제도적 수단으로 정립 하고자 하였다. 본 연구의 목적은 정원치유를 보건·복지 시스템 내에 편입시키 기 위한 한국형 모델을 도출하고, 이를 실현하기 위한 법적, 제도적, 재정적 실행 전략을 제시함으로써 예방적이고 지속가능한 보건복지 패러다임으로의 전환을 도 모하는 데 있다. 본 연구는 이론적 고찰을 통해 정원처방의 개념을 정립하고 기존의 자연기반 치유와 차별화하였다. 정원치유는 산림이나 농촌 등 특정 자원 중심의 접근이 공 간적 격리성을 가지는 것과 달리, 도시 내 '일상 생활권'을 기반으로 하여 물리적 접근성과 활동의 지속가능성을 담보하는 '공간 기반' 접근이라는 점에서 독자적인 정체성을 지닌다. 또한, 정원치유의 개념을 활동의 목적과 대상에 따라 회복적 차원의 '치료적 가드닝(Therapeutic Gardening)'과 예방적 차원의 '복지적 가드 닝(Welfare Gardening)'으로 이원화하여 정의함으로써, 향후 정책적 접근 경로를 치료(의료)와 예방(복지)으로 세분화할 수 있는 이론적 토대를 마련하였다. '정원 처방'은 정원치유라는 '콘텐츠'를 사회적 처방이라는 '전달체계'에 탑재한 제도적 기제로 정의된다. 사회적 처방은 의료 전문가가 환자의 비의료적 문제(사회적 고 립, 우울 등)를 해결하기 위해 지역사회의 자원과 연계하는 시스템을 의미하며, 그중 자연 기반 활동을 처방하는 것을 '녹색 사회적 처방(Green Social Prescribing, GSP)'이라 한다. 본 연구는 정원처방을 녹색 사회적 처방의 가장 접근성 높은 실천 모델로 위치시켰다. 본 연구는 국내에 아직 도입되지 않은 제도를 설계하는 탐색적 연구로서, '질적 다각화 연구 설계'를 채택하였다. 연구의 절차는 문헌 고찰, 영국의 녹색 사회적 처방(GSP) 사례연구, 국내 핵심 전문가 7인을 대상으로 한 심층인터뷰(FGI), 그 리고 결과를 종합 분석하는 단계로 수행되었다. 첫째, 사례연구는 세계 최초로 사회적 처방을 국가 정책으로 도입한 영국의 사례를 심층 분석하였다. 분석 대상 은 NHS(국가보건시스템)의 장기 계획, 녹색 사회적 처방 시범사업, 관련 법령 및 정책 보고서 등을 포괄하였다. 둘째, 전문가 FGI는 보건의료, 사회복지, 조경· 정원, 정책·행정 분야의 전문가 7인을 대상으로 진행되었다. 이를 통해 영국 모 델의 국내 적용 시 발생할 수 있는 구조적 장벽(의료 수가, 인력 등)을 진단하고, 한국의 의료 및 복지 현실에 부합하는 제도화 방안을 도출하였다. 영국 사례 분석 결과, 제도의 핵심 성공 요인은 세 가지로 요약된다. 첫째, NHS와의 유기적 통합이다. 사회적 처방은 별도의 사업이 아닌 NHS의 핵심 정책 으로 편입되어 안정적인 재정 지원을 받는다. 둘째, '링크워커(Link Worker)'의 확립이다. 의료진(GP)과 지역사회 자원을 연결하는 전문 중개 인력인 링크워커를 배치하여 의료진의 업무 부담을 줄이고 환자 맞춤형 연계를 가능하게 했다. 셋 째, 범부처 협력 거버넌스이다. 보건부와 환경부가 '공동 성과 기금'을 조성하여 부처 간 칸막이를 제거하고 예산을 공동으로 집행함으로써 정책의 실행력을 높였 다. 전문가 FGI 결과, 영국 모델을 국내에 그대로 이식하는 데에는 장벽이 존재 함이 밝혀졌다. 가장 큰 장애 요인은 한국의 '민간 주도형·행위별 수가제 (fee-for-Service, FFS)' 의료 환경이다. 공공재원(인두제) 기반의 영국 GP는 환 자의 건강이 증진되면 의사의 업무가 줄어들고 인센티브를 받는 구조인 반면, 한 국의 민간 의사는 환자를 외부(정원)로 의뢰하는 행위가 곧 진료 수익 감소로 이 어지므로 구조적 저항이 발생할 수밖에 없다. 또한, 건강보험 재정 투입에 대한 근거 부족과 사회적 합의 미비도 주요한 장벽으로 지적되었다. 이러한 분석 결과를 종합하여, 본 연구는 초기 단계에서 민간 의료기관과의 직 접 연계를 시도하기보다는 공공 전달체계를 활용하는 '공중보건 우선 접근 모델 (Public Health-First Access Model)'을 최적의 대안으로 도출하였다. 이는 FFS 갈등에서 자유로운 보건소, 정신건강복지센터, 치매안심센터 등 공공기관을 1차 의뢰 거점으로 설정하여 제도를 안착시킨 후, 축적된 데이터를 바탕으로 건강보 험 영역으로 확장하는 우회 전략이다. 또한, 대상자의 상태에 따라 1단계(예방· 증진), 2단계(관리·중재), 3단계(치료·재활)로 구분하는 '3단계 정원처방 스펙트 럼 모델'을 제안하여 서비스의 강도와 전문성을 차별화하였다. 본 연구는 도출된 '공중보건 우선 접근 모델'을 기반으로 한국형 정원처방의 성 공적 제도화를 위한 5대 실행 전략과 10대 핵심 정책 과제를 다음과 같이 제언한 다. 첫째, 법적·제도적 기반의 '양면 전략'이다. 제도의 실효성을 확보하기 위해 공급과 수요 측면의 법적 근거를 동시에 마련해야 한다. 공급 측면에서는 「수목 원·정원의 조성 및 진흥에 관한 법률」을 개정하여 정원치유와 정원처방의 정의 를 구체화하고, 치유정원 인증 기준 및 전문인력 자격 근거를 마련해야 한다. 수 요 측면에서는 「국민건강증진법」이나 「보건의료기본법」 등에 사회적 처방을 '비의료적 건강관리 서비스'의 하나로 명시하여, 보건소 등 공공기관이 지역 주민 에게 정원처방을 공식적으로 의뢰할 수 있는 법적 근거를 확보해야 한다. 둘째, 운영 체계의 공공성 강화 및 연계이다. 한국의 민간 의료 중심 환경을 고려하여, 제도 도입 초기에는 보건소, 치매안심센터 등을 핵심 거점으로 삼아야 한다. 이를 기존의 '지역사회 통합돌봄' 사업 및 읍면동의 '찾아가는 보건복지 서 비스'와 연계하여 의료 사각지대를 해소하고, 고위험군을 조기에 발굴하여 정원치 유 프로그램으로 연결하는 공공 전달체계를 구축해야 한다. 셋째, 전문 인력의 이원화 및 양성이다. 제도의 핵심 엔진인 인적 인프라를 구 축하기 위해 전문 인력의 역할을 '연결자'와 '실행자'로 이원화해야 한다. 대상자 의 욕구를 파악하고 적절한 자원을 연결하는 중개자인 '건강복지 연결가(가칭, 한 국형 링크워커)'와 실제 현장에서 전문적인 치유 프로그램을 운영하는 '치유정원 사'로 역할을 명확히 분담하여 양성해야 한다. 초기에는 보건소 방문간호사나 사 회복지사 등 기존 인력에게 직무 교육을 통해 자격을 부여하는 방식을 활용하고, 점진적으로 국가 공인 자격화를 추진해야 한다. 넷째, 복지 재원 우선 활용 및 단계적 확장 전략이다. 재정 확보의 난항을 타 개하기 위해 단계적 접근이 필요하다. 초기에는 국민건강증진기금, 고향사랑기부 제, 지자체 사회복지 예산 등을 활용한 시범사업을 통해 재원을 확보하고, 사회 서비스 전자바우처 제도와 연계하여 접근성을 높인다. 중장기적으로는 시범사업 을 통해 축적된 의료비 절감 효과 데이터를 근거로 사회성과연계채권(SIB)을 도 입하거나, 건강보험의 예방 급여 및 선별 급여 항목으로 진입을 추진하는 로드맵 을 수립해야 한다. 다섯째, 범부처 협력 연계형 거버넌스 구축이다. 정원처방은 보건(수요)과 산 림·정원(공급)의 결합이므로 부처 간 협력이 필수적이다. 보건복지부와 산림청을 중심으로 농림축산식품부, 환경부 등이 참여하는 '국가 사회적 처방 위원회(가칭)' 를 신설하여 정책을 조율하고 공동 예산을 운용해야 한다. 또한, 정부는 '연결'과 '재정 지원'에 집중하되, 실제 서비스 제공은 지역의 사회적 기업, 비영리단체 등 민간(VCSEs)이 담당하는 지속가능한 민관협력 생태계를 구축해야 한다. 본 연구는 정원치유를 단순한 여가 활동이나 개별 프로그램이 아닌 '녹색 사회 적 처방'이라는 새로운 정책 프레임워크 안에서 체계적으로 분석하고, 한국의 독 특한 의료 및 제도적 환경을 반영한 '한국형 정원처방 제도화 로드맵'을 국내 최 초로 제시했다는 점에서 중요한 학술적·정책적 의의를 지닌다. 본 연구에서 제 안된 방안들은 향후 국민의 정신건강 증진, 의료비 부담 완화, 사회적 통합 강화, 그리고 정원 관련 산업의 육성 등 다양한 분야에서 긍정적인 파급 효과를 가져올 것으로 기대된다. 다만, 본 연구는 대규모 국내 실증 데이터의 부재, 실제 수혜자 의 의견 수렴 미흡 등의 한계를 지니며, 향후 제도의 안착을 위해 엄격한 효과 검증을 위한 임상 실증 연구 및 경제성 평가 연구 등의 후속 연구가 지속적으로 수행되어야 할 것이다.

Modern society faces complex health and social crises, including rapid population ageing, the rise
of single-person households, the prevalence of chronic diseases, and deepened social isolation and
depression following the pandemic. These multi-layered problems have reached limits that are difficult
to resolve solely through existing drug-centred biomedical treatment models; thus, new alternatives are
urgently required to alleviate the burden of socio-economic costs and mitigate health inequalities. This
study focuses on the potential of 'Garden Therapy' as part of 'Nature-based Solutions(NbS)' as a solution.
In South Korea, although the legal definition of garden therapy was introduced through the amendment
of the Act on the Creation and Promotion of Arboretums and Gardens in 2020, it remains at a declarative
level without specific implementation standards or delivery systems. Accordingly, this study aimed to
establish a 'Gardening Prescription' model as a concrete institutional means by introducing the 'Social
Prescribing' system, which has already been verified in advanced countries such as the UK, as a core
delivery system. This combines the excellent healing resource of 'garden therapy' with the effective
delivery system of 'social prescribing', aiming to present a practical policy alternative that promotes
a transition to a preventative and sustainable healthcare paradigm by proactively intervening in the 'Social
Determinants of Health'.
This study adopted a 'Qualitative Triangulation Design' and was conducted through the stages of
a literature review, a case study of the UK's Green Social Prescribing(GSP), in-depth Focus Group
Interviews(FGI) with seven key domestic experts, and a comprehensive analysis of the results. The
main findings and conclusions derived are as follows:
First, through a theoretical review, this study differentiated the concept of gardening prescription fromexisting forest healing or agro-healing. Unlike these approaches, which have spatial isolation centred
on specific resources(forests, rural areas), gardening prescription possesses a unique identity as a
'Space-based' approach that ensures physical accessibility and the sustainability of activities based on
the urban 'Daily Life Sphere'. Furthermore, the concept of garden therapy was dichotomised into
'Therapeutic Gardening' for restorative purposes and 'Welfare Gardening' for preventative purposes,
providing a theoretical foundation to subdivide policy approach pathways into treatment(medical) and
prevention(welfare).
Second, the analysis of the UK GSP case identified the key success factors as the organic integration
with the National Health Service(NHS), the establishment of 'Link Workers'—professional intermediaries
connecting medical staff(GPs) and community resources—and cross-departmental cooperative governance
where the Department of Health and the Department for Environment created a 'Shared Outcomes Fund'
to remove silos between departments.
Third, the expert FGI results diagnosed the structural barriers to introducing the UK model to Korea.
The most significant obstacle identified was Korea's 'private-led Fee-for-Service(FFS)' medical
environment. Unlike UK GPs based on public funding(capitation), for Korean private doctors, referring
patients externally(to gardens) leads to a reduction in medical revenue(opportunity cost), inevitably
causing structural resistance. Synthesising these analysis results, this study derived the 'Public Health-First
Access Model' as the optimal alternative, utilising public delivery systems rather than attempting to
link with private medical institutions in the initial stage. This represents a bypass strategy to establish
the system by designating public institutions free from FFS conflicts, such as Public Health Centres,
Mental Health Welfare Centres, and Dementia Centres, as primary referral hubs, and subsequently
expanding into the health insurance realm based on accumulated data.
Based on the 'Public Health-First Access Model', this study proposed five implementation strategies
and ten key policy tasks for the successful institutionalisation of Korean-style Gardening Prescription:
1. 'Two-Track Strategy' for Legal Basis: To ensure effectiveness, legal grounds must be established
on both the supply and demand sides. On the supply side, the Arboretum and Garden Act should be
amended to clarify the definition of garden therapy and establish certification standards and professional
qualifications. On the demand side, social prescribing should be specified as a 'non-medical health
management service' in the National Health Promotion Act or the Framework Act on Health and Medical
Services to secure a legal basis for public institutions to officially refer gardening prescriptions.
2. Strengthening Public Operational Systems: Considering Korea's private-centred medical
environment, public health centres should serve as hubs in the initial stage. These should be linked
with existing 'Community Care' projects and visiting health and welfare services to eliminate medical
blind spots and build a public delivery system that identifies high-risk groups early and connects them
to garden therapy programmes.
3. Dualisation and Training of Workforce: Roles should be clearly divided between 'Health-Welfare
Link Workers(tentative)', who identify needs and connect resources, and 'Garden Therapists', who operate
healing programmes on-site. Initially, qualifications should be granted through job training for existing
personnel like visiting nurses, followed by the gradual promotion of national certification.
4. Prioritising Welfare Funds and Phased Strategy: To overcome financial hurdles, a phased approach
is needed. Initial funding should be secured through the National Health Promotion Fund and local
budgets via 'Test & Learn' pilot projects, linked with social service vouchers. In the mid-to-long term,
based on accumulated cost-effectiveness(SROI) data, a roadmap should be established to introduce Social
Impact Bonds(SIB) or enter National Health Insurance preventative/selective benefits.
5. Establishment of Inter-ministerial Governance: As gardening prescription combines health(demand)
and forestry/landscaping(supply), inter-ministerial cooperation is essential. A 'National Social Prescribing
Committee(tentative)' centred on the Ministry of Health and Welfare and the Korea Forest Service should
be created. The government should focus on connection and funding, while the private sector(VCSEs)
handles service provision, building a sustainable public-private partnership ecosystem.
This study holds significant academic and policy implications in that it systematically analysed garden
therapy within the new policy framework of 'social prescribing' rather than as a simple activity
programme, and presented a 'Korean-style Gardening Prescription Institutionalisation Roadmap' reflecting
Korea's unique medical and institutional environment for the first time domestically. The measures
proposed in this study are expected to bring positive ripple effects in various fields, including the
improvement of national mental health, alleviation of medical cost burdens, strengthening of social
integration, and fostering of garden-related industries. However, this study has limitations such as a
lack of large-scale domestic empirical data. Therefore, follow-up studies, such as rigorous empirical
research for effectiveness verification and economic evaluation research, must be continuously conducted
