    본 연구의 목적은 지역사회에 거주하는 지체·뇌병변 고령장애인을 대상으로 지역사회 기반 재활프로그램을 적용하였을 때, 건강수준과 삶의 질에 미치는 효과를 검증하는 것이다. 연구 대상자는 지역사회 거주 만 65세 이상 고령자 중 선정기준에 부합하고 연구 참여에 동의한 20명(실험군 10명, 대조군 10명)이었으며, 통제집단 사전·사후검사 설계(pretest-posttest control group design)를 적용하였다.
    실험군에는 1회 50분, 주 2회, 총 12회기로 구성된 지역사회 기반 재활프로그램을 집단형 중재로 제공하였다. 한편, 대조군은 대기자 통제집단(wait-list control group)으로 설정하여, 윤리적 고려에 따라 연구 종료 후 동일한 프로그램을 제공하였다. 건강수준과 삶의 질을 평가하기 위해 세계보건기구장애평가목록 2.0(Measuring Health and Disability: Manual for WHO Disability Assessment Schedule, WHODAS 2.0), 한국형 도구적 일상생활활동 측정도구(Korean Instrumental Activity of Daily Living, K-IADL), 건강관련 삶의 질 척도(Short Form-36 Health Survey, SF-36)을 사용하여 사전·사후 평가를 실시하였다.
    연구 결과, 실험군 내 사전·사후 비교에서는 WHODAS 2.0에서 이동 영역(p = .021), 사회참여 영역(p = .021)에서 유의미한 기능 향상을 보였고, K-IADL에서는 식사준비 영역(p = .010), 물건사기 영역(p = .012)에서 의존도가 유의하게 감소하였다. 또한 SF-36에서는 일반건강 범주(p = .010), 활력 범주(p = .010), 정신건강 범주(p = .014) 및 정신적 요소 요약(p = .027)에서 유의미한 향상이 나타났다(p < .05). 대조군 내 사전·사후 비교에서는 WHODAS 2.0, K-IADL, SF-36의 모든 하위영역 및 총점에서 유의한 차이가 없었다(p > .05). 두 집단 간 사전·사후 변화량 비교에서는 WHODAS 2.0의 이동 영역(p = .035)에서 실험군이 대조군보다 유의미한 향상을 보였으며(p < .05), K-IADL의 식사준비 영역(p = .009), 물건사기 영역(p = .002) 및 총점(p = .020)에서 실험군의 의존도 감소가 대조군보다 유의미한 향상이 나타났다(p < .05). SF-36에서는 일반건강 범주(p = .047), 감정적 역할제한 범주(p = .021), 정신건강 범주(p = .015) 및 정신적 요소 요약(p = .012)에서 실험군이 대조군보다 유의미한 향상을 보였다(p < .05). 건강수준과 삶의 질 간의 상관관계 분석에서는 WHODAS 2.0과 SF-36 정신적 요소 요약 간에 강한 음의 상관이 관찰되었으나, 통계적으로 유의미한 수준에는 도달하지 않았다(p = .073, ρ = -0.59). WHODAS 2.0과 신체적 요소 요약 간 상관관계는 중등도의 양의 상관이 관찰되었으나, 통계적으로 유의하지 않았다(p = .291, ρ = .37), (p > .05).
    이러한 결과는 지역사회 기반 재활프로그램이 지체·뇌병변 고령장애인의 건강수준과 삶의 질을 향상시키는 데 효과적인 중재임을 보여준다. 본 연구는 재활의 의미를 신체기능 회복에만 한정하지 않고, 정신 및 사회적 영역까지 포괄하는 생활 기반 재활로 확장한다는 점에 의의가 있다. 향후 여러 지역사회 기관에서 노화와 장애를 동시에 고려한 재활프로그램 개발에 실질적인 근거 마련과 다양한 지역사회 서비스와 통합적으로 적용될 수 있는 가능성을 기대한다.

    This study aimed to verify the effects of applying a community-based rehabilitation program on the health and quality of life of elderly people with physical and brain lesion disorder.
    The study employed a pretest–posttest control group design involving 20 participants aged 65 years or older who met the selection criteria and agreed to participate in the study (10 in the experimental group and 10 in the control group).
    The experimental group received a group-based community-based rehabilitation program consisting of 12 sessions (50 minutes per session, twice a week). The control group was assigned as a wait-list control group and did not receive the intervention during the study period; however, for ethical considerations, the same program was provided after study completion. Health status and quality of life were assessed using the World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0), the Korean Instrumental Activities of Daily Living (K-IADL), and the Short Form-36 Health Survey (SF-36), administered before and after the intervention.
    Within-group comparisons revealed that the experimental group demonstrated significant improvements in mobility (p = .021) and participation in society (p = .021) on the WHODAS 2.0. On the K-IADL, significant reductions in dependency were observed in meal preparation (p = .010) and shopping (p = .012). Additionally, significant improvements were found in general health (p = .010), vitality (p = .010), mental health (p = .014), and the mental component summary (p = .027) of the SF-36 (p < .05). In contrast, no significant pre–post changes were observed in any domains of the WHODAS 2.0, K-IADL, or SF-36 in the control group (p > .05).
    Between-group comparisons of change scores indicated that the experimental group showed significantly greater improvement than the control group in mobility on the WHODAS 2.0 (p = .035). On the K-IADL, significantly greater reductions in dependency were observed in meal preparation (p = .009), shopping (p = .002), and total scores (p = .020) in the experimental group. Furthermore, the experimental group demonstrated significantly greater improvements in general health (p = .047), role limitations due to emotional problems (p = .021), mental health (p = .015), and the mental component summary (p = .012) of the SF-36 (p < .05).
    Correlation analysis between health status and quality of life revealed a moderate positive correlation between changes in WHODAS 2.0 scores and changes in the physical component summary of the SF-36, although this relationship was not statistically significant (ρ = .37, p = .291). A strong negative correlation was observed between changes in WHODAS 2.0 scores and the mental component summary, but this association also did not reach statistical significance (ρ = −0.59, p = .073).
    These findings suggest that community-based rehabilitation programs are effective interventions for improving health status and quality of life among older adults with physical and brain lesion disorder. This study is significant in that it expands the concept of rehabilitation beyond physical function recovery to a life-based approach encompassing mental and social domains. The results provide practical evidence supporting the development of rehabilitation programs that concurrently address aging and disability and their integrated application within community-based service systems.
