고령화와 만성질환의 증가로 전 세계적으로 일상생활과 사회참여를 위해 보조기기를 필요로 하는 사람들이 지속적으로 증가하고 있다. 우리나라는 2016년 「장애인‧노인 등을 위한 보조기기 지원 및 활용촉진법」을 제정하고, 2019년부터 보조공학 서비스를 제공하는 전문인력인 ‘보조공학사’를 국가자격 제도로 관리하고 있다. 또한 보조공학 서비스 전문기관인 보조기기센터를 전국 17개 지역에 설치하여 운영하고 있지만, 장애인 보조기기 교부사업 등 극히 일부 제도의 연계만 이루어지고 있다.
본 연구에서는 우리나라 공적급여 보조기기 제도 중 가장 보편적인 제도인 국민건강보험공단 장애인 보조기기 보험급여 제도와 노인장기요양보험 복지용구 급여 제도를 중심으로, 현행 제도의 구조적 문제점을 체계적으로 분석하고, 보조공학 서비스 전문기관과의 연계를 기반으로 한 서비스 전달체계 개선 방안을 제시하였다. 현행 제도는 보조기기 ‘공급’에 치중되어 전문적 상담·평가·적용·사후관리 등 전문적인 보조공학 서비스 요소가 충분히 반영되지 못하고 있는 실정이다. 이는 선행연구에서 공통적으로 지적되었던 부분으로 이러한 문제점을 개선하고 효과적인 보조기기 서비스 전달체계 모델을 제시하기 위해 연구가 수행되었다.
연구 목적의 달성을 위해 첫째, 관련 법령과 통계자료를 분석하여 제도적 구조와 행정 절차를 파악하고, 국내 제도와 해외 제도를 비교 분석하였다. 둘째, 보조기기 관련 선행연구와 정책자료를 분석하여 제도의 실효성과 문제점을 파악하였다. 셋째, 전국 22개 보조기기센터에서 근무하는 실무자를 대상으로 전수 조사를 실시하여 현행 급여제도의 문제점과 개선 요구사항을 파악하였다. 넷째, 전국 22개 보조기기센터에서 근무하는 센터장과 팀장을 대상으로 델파이 조사를 실시해 서비스 전달체계의 효과적인 개선 방안과 전문기관 연계 방안을 모색하였다. 마지막으로 보건복지부·국립재활원 등 정책담당자들과 심층 면담을 실시해 제도 개선안의 정책적 실현 가능성과 행정적 타당성을 검토하였다.
보조공학 실무자 대상 설문조사 결과 응답자의 72.6%(n=53)는 현행 보험급여 품목이 적정하지 않다고 응답하였다. 보험급여 품목의 문제점으로는 지체·뇌병변장애인 및 의지·보조기 품목의 편중성이 51%(n=27)로 가장 높게 나타났다. 급여 품목 확대 필요성은 76.7%(n=56)가 필요하다고 응답하였고, 보험급여 품목 확대 필요 영역으로는 걷기 및 이동, 씻기, 자세유지, 운전하기 등 이동 및 일상생활 영역의 품목 확대 필요성이 높게 나타났다. 복지용구 급여 품목의 적정성에서는 56.1%(n=41)가 적정하지 않다고 응답하였고, 문제점으로는 품목 수의 제한으로 이용자의 다양한 특성이 반영되지 못하는 점이 57.8%(n=26), 급여 품목의 다양성 및 종류 제한이 42.2%(n=19)으로 나타났다. 복지용구 급여 품목 확대 필요성으로는 60.9%(n=44)이 필요하다고 응답하였다. 이는 보험급여 제도 및 복지용구 급여 제도에서 장애 및 노인 질환의 특성에 기반한 다양한 수요를 충족하기 위해서는 수요에 입각한 기능 및 환경 평가 중심의 포괄적인 품목 개편이 필요함을 시사한다.
보험급여 품목의 기준금액 및 내구연한 적정성 결과에서는 수동휠체어, 전동휠체어, 전동스쿠터 품목에서 전반적으로 시장가격에 비해 기준금액이 낮게 측정되어 있고, 내구연한이 상대적으로 길다고 평가하였다. 복지용구 급여 품목 중에서는 이동변기, 성인용 보행기, 목욕의자, 전동침대, 수동침대 등의 품목에서 사용 가능 햇수가 길다고 응답하였다. 이러한 비현실적인 급여기준은 급여 이용자의 본인부담을 증가시키고, 적정 보조기기의 사용을 저해하는 주요 요인이 되므로 품목별 적정 급여기준을 산정하기 위한 체계적인 연구를 바탕으로 급여체계의 개선이 필요함이 나타났다.
보험급여 제도 및 품목 정보 제공 관련 문항에서는 응답자의 98.6%(n=72)가 중요하다고 응답했지만, 현행 보험급여 제도에서 이용자에게 정보를 효과적으로 제공한다는 응답은 15%(n=11)로 나타났다. 복지용구 급여 제도 문항에서는 89.1%(n=65)가 정보 제공이 중요하다고 응답하였으나, 현행 복지용구 급여 제도에서 이용자에게 정보를 효과적으로 제공한다는 응답은 65.8%(n=48)에 머물러 보험급여 및 복지용구 급여 이용자에게 효과적인 정보 제공을 위한 제도 개선이 시급한 것으로 나타났다.
보험급여 제도의 사후관리 문항에서는 응답자의 97.2%(n=71)가 사후관리 서비스가 필요하다고 응답했다. 하지만, 사후관리 서비스가 체계적으로 이루어지고 있다는 응답은 5.5%(n=4), 그렇지 않다는 응답은 74%(n=54)로 나타나 보험급여 제도에서 체계적인 관리 체계 마련 37%(n=20), 수리비 급여 신설 33.3%(n=18) 등 전반적인 사후관리 제도의 개선이 필요한 것으로 나타났다. 복지용구 급여 제도의 사후관리 문항에서는 72.6%(n=53)가 사후관리가 적절히 이루어지고 있지 않다고 응답하였고, 사후관리 서비스를 개선하기 위해 ‘적정 사용을 위한 교육 및 훈련(37%)’, ‘체계적 사후관리 체계 마련(31.5%)’이 필요하다고 응답하였다. 응답자들은 보험급여 제도와 복지용구 급여 제도 모두에서 사후관리 서비스 수준이 미흡하다고 인식하고 있었으며, 특히 복지용구 급여 제도에서는 안전사고에 대한 관리 적절성이 낮게 나타나 안전 사용을 위한 전문적인 교육·훈련 및 안전관리 체계 마련 등이 필요한 것으로 나타났다.
보험급여 제도의 전달체계 문제점으로는 ‘체계적인 사례관리시스템 부재(35.6%)’, ‘일원화된 서비스 전달체계 확립되지 않음(27.4%)’, ‘보조기기 공급자 위주의 전달체계(19.2%)’가 지적되었고, 전문인력을 통한 서비스 제공 필요성은 87.7%(n=64)로 나타났다. 전문인력을 통한 중재 서비스가 필요한 이유로는 76.6%(n=49)가 개인의 특성 및 욕구가 반영된 전문적인 서비스 제공이 필요하다고 응답하였다. 복지용구 급여 제도 문항에서는 85%(n=62)가 전문인력을 통한 복지용구 급여 서비스 제공이 필요하다고 응답하였고, 그 이유로는 ‘복지용구 추천·교육·훈련 등 전문적인 서비스 필요(83.1%)’, ‘지역별 거점 관리체계 역할 필요(10.2%)’로 나타났다. 이처럼 보험급여 제도 및 복지용구 급여 제도에서 전문인력의 중재를 통한 서비스 제공이 필요하며, 보조기기센터를 중심으로 한 지역 네트워크 구축과 전달체계 개편의 필요성을 시사하였다.
보조기기센터 전문가 대상 델파이 조사 결과에서는 장애인 보조기기 보험급여 제도 및 복지용구 급여의 정보 제공, 상담 및 평가, 교육 및 훈련, 사후관리까지 포괄하는 체계적인 서비스 과정으로 개선해야 한다는 의견이 도출되었다. 특히 ‘공단 등록업소 종사자 의무교육(94.1%)’과 ‘표준화된 상담 및 평가 매뉴얼 개발(88.2%)’이 가장 높은 전문가 의견 일치율을 보였고, ‘온라인 자원 활용’이나 ‘수리비 급여 신설’ 등은 재정 및 행정 부담으로 의견 일치율이 상대적으로 낮았다. 또한 이용자의 특성 및 환경을 고려한 전문 상담 체계 부재, 과학적 근거에 기반한 품목 등재 미흡, 품질 및 안전관리 부족, 사후관리 서비스 부족, 전문인력 활용 미흡, 통합적 데이터 관리 미비 등이 지적되었다.
정책 및 학술적 시사점으로는 본 연구는 단일 제도 분석을 넘어 장애인 보조기기 보험급여 제도와 복지용구 급여 제도를 통합적 시각에서 분석하여, 기존 제도의 구조적 문제를 체계적으로 규명한 점에서 학술적 의의가 있다. 다각적인 방법론(실무자 설문조사, 전문가 델파이 조사, 정책담당자 심층 면담)을 통해 현장의 현실과 정책대안의 실행 가능성을 확보하였다. 연구에서 도출된 개선 방안은 정부가 추진 중인 복지용구 급여 제도 개편과 맥락을 같이하며, 급여 품목 확대, 가격 기준 재정비, 전문인력 활용 등이 도출되었다. 또한 정책화 단계에서 상위계획과의 정합성, 재정 영향, 단계별 도입(시범사업) 및 이해관계자 수용성을 고려해야 한다는 점도 제시되었다. 연구 결과를 토대로 이용자 중심의 서비스 전달체계 구축을 위한 여덟 가지 정책 대안을 제시하였다.
첫째, 급여품목의 확대와 기능 중심 분류체계 도입이다. 지체장애 위주 품목을 이동‧자세 유지, 시각‧청각, 인지‧학습, 일상생활 보조기기, 로봇기술 기반 기기로 확대하고, 예비급여 시범사업을 통해 안전성과 유효성을 검증한 후 본 급여로 전환해야 한다.
둘째, 기준금액과 사용 가능 기간의 현실화이다. 제품 생산원가, 기술성, 서비스 비용 등을 고려해 기준금액을 현실화하고, 가격 조정 시스템을 마련한다. 내구연한은 실제 사용 환경의 소모도를 반영해 단축하며, 사용자의 상태 변화에 따라 유연한 재지급을 허용해야 한다.
셋째, 정보 제공 및 상담 체계 강화이다. 건강보험공단과 지역 보조기기센터를 연계한 통합 정보 시스템을 구축해 제품 정보와 이용 방법을 맞춤형으로 제공하고, 표준화된 상담 매뉴얼과 사례관리 체계를 마련한다.
넷째, 전문인력 양성과 전달체계 재편이다. 전문인력 배치와 공단 등록업소 종사자 의무교육을 제도화하고, 지역별 협력체계를 구축해 보조기기센터를 거점으로 한 서비스 네트워크를 형성해야 한다.
다섯째, 이용자 교육 및 훈련이다. 이용자와 보호자를 대상으로 안전 사용과 유지‧보수 교육 프로그램을 제공하고, 공급업체 및 복지용구사업소 종사자 등 공급·관리 인력에 대한 교육도 강화한다.
여섯째, 사후관리 및 수리비 지원 체계 마련이다. 급여체계에 수리비 급여를 포함하고, 정기적인 점검과 유지·보수를 제공하며, 사후관리 책임을 명확히 하는 제도를 마련한다.
일곱째, 품질 및 안전 관리 강화이다. 과학적 안전성·유효성 평가를 의무화하고, 대여 제품에 대한 소독 기준을 마련하며, 기준에 부합하지 않은 제품에 대한 관리 체계를 구축한다.
마지막으로 통합 데이터 기반 정책 추진이다. 중앙 데이터베이스를 구축해 급여 이용 현황과 품질·안전 데이터를 통합 관리하고, 이를 활용해 급여 품목과 기준을 주기적으로 조정하며 정책 효과를 평가한다.
본 연구는 장애인 보조기기 보험급여 제도와 복지용구 급여 제도의 구조적 한계를 확인하고, 이용자 중심의 통합 서비스 모델과 구체적 정책 개선안을 제시하였다. 급여 품목 확대, 기준금액 현실화, 사후관리 강화, 전문인력 양성, 데이터 기반 정책 추진 등을 통해 보조기기 지원체계가 단순한 보조기기 지급을 넘어 전문인력에 의한 상담 및 평가, 정보 제공, 교육 및 훈련, 사후관리가 체계적으로 제공되는 서비스를 제공해야 함을 강조한다. 이러한 개선방향은 장애인과 노인의 자립생활과 삶의 질 향상에 기여하고, 중앙정부 차원의 보조기기 지원제도의 인권적·사회적 가치를 실현하는 기반이 될 것이다.
연구가 가진 제한점에도 불구하고 전국 22개 보조기기센터의 실무자(보조공학 업무 담당자)와 전문가(센터장, 팀장)을 대상으로 전수조사를 실시하여 장애인 보조기기 보험급여 제도와 복지용구 급여 제도의 구조적 문제점 및 전달체계 개선 모형, 전문기관 연계 방안 모형 제시 등을 통합적으로 분석하였다는 데 의의가 있다. 이러한 시도는 공급자에 의한 ‘보조기기 공급’에서 벗어나, 정부의 정책 방향인 통합돌봄 정책과도 부합되는 내용으로 최근의 시대적 조류에 비추어 시의 적절하다 할 것이다. 본 연구의 결과물은 정부 및 공공기관의 보조기기 전달체계 제도 개선을 위한 기초자료로 활용될 수 있을 것이다. 아울러, 본 연구에서 제시한 개선 방안들이 실제로 장애인과 노인의 삶의 질 향상, 기능 개선, 의료비 절감 등에 어떠한 효과가 나타나는지 후속 연구를 통해 급여 품목 확대와 서비스 강화의 비용·효과성 분석을 실시하여 추가 재정 투입의 경제적 타당성을 평가하고 분석해야 할 것이다. 또한 신기술 보조기기의 도입에 따른 안전성과 유효성 평가, 장애 유형별로 맞춤형 서비스 모델을 개발 등 활발한 후속 연구를 기대한다.

The number of people worldwide who require assistive devices to manage their daily lives and social interactions is growing steadily, driven by the aging population and rising prevalence of chronic diseases. In 2016, Korea enacted the "Act on the Support and Utilization of Assistive Devices for the Disabled and the Elderly." Since 2019, the country has been regulating " Assistive Technology Professionals," who provide assistive technology services, as a national certification system. In addition, the Assistive Technology Centers, a nationwide network of specialized institutions for assistive technology services, have been set up in 17 locations across the country, but their practical impact remains limited to a small number of programs, such as the provision of assistive devices for people with disabilities.
This study focused on the two most widely used public benefit device systems in Korea: the National Health Insurance Corporation's Assistive Device Insurance Benefit System for Persons with Disabilities and the long-term care insurance system for the elderly. It systematically analyzed the structural issues with the current system and proposed improvements to the service delivery system, which would be based on partnerships with specialized assistive technology service providers. The current system is heavily skewed towards providing assistive devices, leaving out essential professional assistive technology services, including expert counseling, evaluation, and post-implementation management. This study aimed to address the common issues identified in previous research and develop an effective model for delivering assistive technology services.
To achieve the research objectives, we first analyzed relevant laws and statistics to understand the institutional framework and administrative procedures, and compared and analyzed domestic and international systems. Second, we analyzed prior research and policy data related to assistive devices to assess the system's effectiveness and identify its challenges. Third, a comprehensive survey was conducted among working-level officials at 22 Assistive Technology Centers to identify the issues and areas for improvement in the current benefit system. Fourth, a Delphi survey was conducted among the directors and team leaders at 22 Assistive Technology Centers to explore effective ways to improve the service delivery system and establish connections with specialized institutions. In a final round of discussions, we held in-depth meetings with policy officials from the Ministry of Health and Welfare and the Assistive Technology Centers to assess the feasibility of implementing the proposed system improvements and their administrative viability.
According to a survey of assistive technology practioners, 72.6%(n=53) felt that the current insurance coverage is inadequate. The most significant issue with insurance benefits was the high concentration of items related to physical disabilities, brain injuries, and assistive devices, which accounted for 51%(n=27) of the total. A significant 76.7%(n=56) felt that the scope of insurance benefits needed to be expanded, with the most pressing areas being mobility and daily life, including walking and moving around, bathing, maintaining posture, and driving. In terms of the appropriateness of welfare equipment benefits, 56.1%(n=41) felt that the items were not suitable, citing that the limited number of items prevented the representation of users' diverse characteristics, which was a major issue for 57.8%(n=26), and the restricted diversity and variety of benefits 42.2%(n=19) were found. A majority of 60.9%(n=44) believed that expanding the list of welfare equipment benefits was necessary. To meet the disability and age-related health conditions with diverse and complex characteristics, a comprehensive overhaul of the insurance benefit system and welfare equipment benefit system is needed, one that prioritizes functional and environmental assessments based on demand.
The standard amount and durability of insurance benefit items were found to be generally lower than market prices for manual wheelchairs, electric wheelchairs, and electric scooters, with a relatively longer durability period. Among the welfare equipment benefits, respondents reported that the lifespan of items such as portable toilets, adult walkers, bath chairs, electric beds, and manual beds is relatively long. The unrealistic benefit standards have been found to be a major factor in increasing the out-of-pocket expenses of users and hindering the use of suitable assistive devices. Therefore, it is necessary to improve the benefit system based on systematic research to establish appropriate benefit standards for each item.
While 98.6% of respondents(n=72) considered the provision of insurance benefit systems and item information to be important, only 15%(n=11) believed that the current system effectively provides information to users. The welfare equipment benefit system survey found that 89.1%(n=65) considered information provision crucial, but only 65.8%(n=48) believed that the current system effectively delivers information to users. This clearly underscores the urgent need for systematic institutional reform to provide more effective information to users of insurance benefits and welfare equipment.
In the follow-up management section of the insurance benefit system, 97.2% (n=71) said they needed follow-up services. However, a significant majority of respondents(74%, n=54) felt that follow-up management services were not being systematically implemented. In contrast, only a small minority(5.5%, n=4) believed that services were being handled in a systematic way. The results suggest that the overall follow-up management system needs improvement, with 37%(n=20) calling for a more structured management system and 33.3%(n=18) advocating for the introduction of repair cost reimbursement. In the follow-up management section of the welfare equipment benefit system, 72.6%(n=53) felt that follow-up management was not being done effectively. To improve follow-up management services, respondents suggested "education and training for proper use"(37%) and "establishing a systematic follow-up management system"(31.5%). Respondents felt that follow-up management services were inadequate in both the insurance benefit system and the welfare equipment benefit system. Notably, the welfare equipment benefit system was found to have a low level of management adequacy for safety accidents, highlighting the need for specialized education and training, as well as a safety management system, to ensure safe use.
The delivery system of the insurance benefit system has several issues, including the lack of a systematic case management system(35.6%), a unified service delivery system that has not been established(27.4%), and a delivery system that focuses on device suppliers(19.2%). Moreover, 87.7% of respondents(n=64) believe that services should be provided by professionals. The majority of respondents, 76.6%(n=49), felt that professional services tailored to an individual's unique characteristics and needs were essential. In the welfare equipment benefit system survey, 85%(n=62) said that professional staff are needed to provide welfare equipment benefit services, citing the need for specialized services such as recommending welfare equipment, as well as providing education and training.
The results showed a strong need for 83.1% of the population, while 10.2% believed that a regional base management system was necessary. The need for services provided through the mediation of professionals in the insurance benefit system and welfare equipment benefit system was highlighted, as well as the necessity of building a regional network centered on the Assistive Technology Centers and reforming the service delivery system.
The Delphi survey of experts at the Assistive Technology Centers concluded that the system for providing insurance benefits for assistive devices for people with disabilities and welfare equipment should be revamped into a comprehensive service process that covers information provision, counseling and assessment, education and training, and follow-up management. Notably, the highest level of agreement among experts was seen in "mandatory education for workers at registered industrial complexes(94.1%)" and "developing standardized counseling and evaluation manuals(88.2%)." In contrast, consensus was relatively low for initiatives like "utilizing online resources" and "introducing repair fee payments," which were hindered by financial and administrative burdens. Furthermore, it was noted that the lack of a professional counseling system tailored to users' characteristics and circumstances, inadequate item registration based on scientific evidence, insufficient quality and safety management, inadequate follow-up management services, underutilization of professional staff, and poor integrated data management were all identified as areas for improvement.
This study has significant policy and academic implications, as it goes beyond a single institutional analysis to integrate the insurance benefits system for assistive devices and the welfare equipment benefits system, thereby systematically addressing the structural issues of existing systems. Through a multi-faceted approach(including practitioner surveys, expert Delphi surveys, and in-depth interviews with policymakers), we were able to verify the realities on the ground and assess the feasibility of our policy options. The study's proposed improvements align with the government's ongoing efforts to revamp the welfare equipment benefit system, which involves expanding the range of eligible items, revising price standards, and leveraging specialized expertise. The policy implementation stage also requires consideration of consistency with higher-level plans, financial implications, phased implementation (pilot projects), and stakeholder acceptance. Based on the research findings, eight policy alternatives were proposed to establish a user-centered service delivery system.
First, the expansion of benefit items and the introduction of a function-based classification system. To enhance the effectiveness of assistive devices, the scope of items covered should be expanded to include mobility and posture support, visual and auditory aids, cognitive and learning aids, daily living aids, and robot-based devices. These should be tested for safety and efficacy through a pilot project for preliminary benefits, after which they should be transitioned to full benefits.
Second, the practical implementation of benefit amounts and allowable periods of use. To set a realistic benchmark, we will factor in production costs, technological capabilities, and service expenses, and establish a pricing adjustment system. The durability limit should be adjusted to reflect the actual usage environment's consumption rate, and flexible repayment should be allowed based on the user's changing status.
Third, we will strengthen our information provision and counseling system. To provide tailored product information and usage guidance, we will establish an integrated information system linking the National Health Insurance Service and local Assistive Technology Centers, and develop standardized consultation manuals and case management systems.
Fourth, it involves training and developing professional personnel and reconfiguring the delivery system. To establish a service network centered on the Assistive Technology Centers, it's essential to institutionalize the deployment of professional staff and mandatory training for employees of registered businesses, and build a regional cooperation framework.
Fifth, user education and training are provided. We offer safety usage and maintenance training programs for users and their caregivers, and also enhance training for our suppliers, welfare equipment staff, and other personnel involved in supply and management.
Sixth, the establishment of a system for follow-up management and repair cost support. The system will incorporate repair costs into the benefit package, offer regular inspections and maintenance, and clearly define follow-up management responsibilities.
Seventh, the strengthening of quality and safety management. The government will make scientific safety and efficacy evaluations mandatory, establish disinfection standards for rental products, and set up a management system for products that don't meet these standards.
Ultimately, this is about driving policy initiatives through integrated data. We have established a central database to integrate and manage benefit usage status, quality, and safety data. This enables us to regularly review and adjust benefit items and standards, and assess the impact of our policies.
This study examined the structural limitations of the insurance coverage for assistive devices for people with disabilities and the welfare equipment benefit system, and proposed a user-centered integrated service model and concrete policy improvements. Expanding the scope of benefits, aligning standard amounts
with real-world conditions, enhancing follow-up monitoring, developing professional expertise, and implementing data-driven policies are essential.
The support system for assistive devices should go beyond just providing basic devices, and instead offer a comprehensive service that includes expert consultation and evaluation, information provision, education and training, and post-service management. These improvements will help enhance the independence and quality of life of people with disabilities and the elderly, and serve as a foundation for realizing the human and social values of the central government's support system for assistive devices.
Notwithstanding the limitations of the study, a comprehensive analysis was conducted on the structural issues and delivery system improvement models of the insurance benefit system for assistive devices and the welfare equipment benefit system, as well as models for linking with specialized institutions, through a nationwide survey of 22 Assistive Technology Centers and their staff(Assistive technology practioners and experts(Center directors and team leaders). This attempt is timely and aligns with the current trend, moving away from the supplier-driven "supply of assistive devices" and aligning with the government's integrated care policy. The findings of this study can serve as a foundation for improving the system of delivering assistive devices to government and public institutions. Furthermore, this study's proposed improvements will be evaluated through follow-up research to assess their impact on enhancing the quality of life, functional capabilities, and reducing medical expenses for people with disabilities and the elderly, ultimately informing the cost-effectiveness of expanding benefits and strengthening services. To assess the economic viability of additional financial investment, an analysis should be conducted.
In addition, the proposed improvements will be evaluated through follow-up research, including safety and efficacy assessments following the introduction of new assistive devices, as well as the development of tailored service models for specific disability types.
